Posterior cervical necrotising fasciitis: a multidisciplinary endeavour in surgery.

ABSTRACT: A 62-year-old male was diagnosed to the Emergency Department with 5-cm posterior neck carbuncle, and was subsequently discovered to have severe necrotising fasciitis intraoperatively during saucerization of the carbuncle. Subsequently, the patient was admitted to the intensive care unit and underwent combined debridement by the General Surgery, Neurosurgery and Plastic Surgery team. The large defect necessitated a trapezius flap reconstruction for coverage. Three months post-surgery, the patient had recovered well with the full range of movement of his neck.

Project description:Necrotising fasciitis is insidiously advancing soft tissue infection, characterized by widespread fascial necrosis. One of the most common conditions encountered in the clinical practice and which required clinical suspicion in patients who have less resistance to these infections. Especially in diabetic patients who are more prone for such infection due to low immunity and other reasons like early detection of these infections helps these patients to have a better recovery by aggressive surgical management and other measures required for the same. Literature has enough evidence on how serious this entity. Hence this study is designed in that aspect to help in early diagnosis and appropriate treatment. This study will help to determine the clinical presentation, the polymicrobial involved. The organisms most closely linked to necrotizing fasciitis are group A beta-hemolytic streptococci, although the disease may also be caused by other bacteria and their sensitivity patterns will help us to install appropriate antibiotics with the surgical management. And to compare our mortality rate with the other studies. A prospective descriptive study including all type II diabetic patients with necrotizing fasciitis over 3 years. Patients were evaluated in detail with respect to patient's history, examination, culture sensitivity and chart reviews and followed up using standard evaluation tools to measure the various outcomes. In 62 patients, the commonest clinical presentation was necrotic patch of skin and cellulites. The organisms cultured being beta hemolytic streptococci and E.Coli. The culture sensitivity pattern is to broad-spectrum antibiotics. The average stay in the hospital was about 13.74 days for the study group and the time delay in presentation is 6 days. Wagner's score is a useful tool for aiding the diagnosis. Patients without foot care had extensive lesions. And a mortality rate of 1.6 % in these patients. In an acutely swollen limb in a diabetic patient a high degree of suspicion of necrotizing fasciitis is required. A detailed foot care is warranted in these patients. An early extensive debridement is mandatory and key for a favorable outcome. And to start on broad- spectrum antibiotics is advisable. Wagner's scoring is helpful to predict the chances of a second surgery. With the above entities in mind the mortality rate of necrotizing fasciitis in type ii diabetes mellitus can be contained.

Project description:BackgroundNecrotising fasciitis is a rapidly progressing soft-tissue infection with a low incidence that carries a relevant risk of morbidity and mortality. Although necrotising fasciitis is often fatal in adults, its case fatality rate seems to be lower in children. A highly variable clinical presentation makes the diagnosis challenging, which often results in misdiagnosis and time-delay to therapy.MethodsWe conducted a protocol-based systematic review to identify specific features of necrotising fasciitis in children aged one month to 17 years. We searched 'PubMed', 'Web of Science' and 'SCOPUS' for relevant literature. Primary outcomes were incidence and case fatality rates in population-based studies, and skin symptoms on presentation. We also assessed signs of systemic illness, causative organisms, predisposing factors, and reconstructive procedures as secondary outcomes.ResultsWe included five studies reporting incidence and case fatality rates, two case-control studies, and 298 cases from 195 reports. Incidence rates varied between 0.022 and 0.843 per 100,000 children per year with a case-fatality rate ranging from 0% to 14.3%. The most frequent skin symptoms were erythema (58.7%; 175/298) and swelling (48%; 143/298), whereas all other symptoms occurred in less than 50% of cases. The majority of cases had fever (76.7%; 188/245), but other signs of systemic illness were present in less than half of the cohort. Group-A streptococci accounted for 44.8% (132/298) followed by Gram-negative rods in 29.8% (88/295), while polymicrobial infections occurred in 17.3% (51/295). Extremities were affected in 45.6% (136/298), of which 73.5% (100/136) occurred in the lower extremities. Skin grafts were necessary in 51.6% (84/162) of the pooled cases, while flaps were seldom used (10.5%; 17/162). The vast majority of included reports originate from developed countries.ConclusionsClinical suspicion remains the key to diagnose necrotising fasciitis. A combination of swelling, pain, erythema, and a systemic inflammatory response syndrome might indicate necrotising fasciitis. Incidence and case-fatality rates in children are much smaller than in adults, although there seems to be a relevant risk of morbidity indicated by the high percentage of skin grafts. Systematic multi-institutional research efforts are necessary to improve early diagnosis on necrotising fasciits.

Project description:ObjectiveRobot-assisted (RA) techniques have been widely investigated in thoracolumbar spine surgery. However, the application of RA methods on cervical spine surgery is rare due to the complex morphology of cervical vertebrae and catastrophic complications. Thus, the feasibility and safety of RA cervical screw placement remain controversial. This study aims to evaluate the feasibility and safety of RA screw placement on cervical spine surgery.MethodsA comprehensive search on PubMed, Cochrane Library, Embase Database, Web of Science, Chinese National Knowledge Databases, and Wanfang Database was performed to select potential eligible studies. Randomized controlled trials (RCTs), comparative cohort studies, and case series reporting the accuracy of cervical screw placement were included. The Cochrane risk of bias criteria and Newcastle-Ottawa Scale criteria were utilized to rate the risk of bias of the included literatures. The primary outcome was the rate of cervical screw placement accuracy with robotic guidance; subgroup analyses based on the screw type and insertion segments were also performed.ResultsOne RCT, 3 comparative cohort studies, and 3 case series consisting of 160 patients and 719 cervical screws were included in this meta-analysis. The combined outcomes indicated that the rates of optimal and clinically acceptable cervical screw placement accuracy under robotic guidance were 88.0% (95% confidence interval [CI], 84.1%-91.4%; p = 0.073; I2 = 47.941%) and 98.4% (95% CI, 96.8%-99.5%; p = 0.167; I2 = 35.954%). The subgroup analyses showed that the rate of optimal pedicle screw placement accuracy was 88.2% (95% CI, 83.1%-92.6%; p = 0.057; I2 = 53.305%); the rates of optimal screw placement accuracy on C1, C2, and subaxial segments were 96.2% (95% CI, 80.5%-100.0%; p = 0.167; I2 = 44.134X%), 89.7% (95% CI, 80.6%-96.6%; p = 0.370; I2 = 0.000X%), and 82.6% (95% CI, 70.9%-91.9%; p = 0.057; I2 = 65.127X%;), respectively.ConclusionRA techniques were associated with high rates of optimal and clinically acceptable screw positions. RA cervical screw placement is accurate, safe, and feasible in cervical spine surgery with promising clinical potential.
