Perceived Occupational Noise Exposure and Depression in Young Finnish Adults.

ABSTRACT: We investigated the association between perceived occupational noise exposure and depressive symptoms in young Finnish adults and whether noise sensitivity moderates this association. This study was based on an ongoing longitudinal twin study. We included those who had been working daily (n = 521) or weekly (n = 245) during the past 12 months (mean age 22.4, SD 0.7, 53% female). We asked about occupational noise exposure at age 22 and assessed depressive symptoms using the General Behavior Inventory (GBI) at age 17 and 22. Noise sensitivity and covariates were used in linear regression models. Perceived daily occupational noise exposure was associated, as a statistically independent main effect with depressive symptoms at age 22 (beta 1.19; 95% CI 0.09, 2.29) among all, and separately for females (beta 2.22; 95% CI 0.34, 4.09) but not males (beta 0.22; 95% CI -1.08, 1.52). Noise sensitivity was independently associated with depressive symptoms among all (beta 1.35; 95% CI 0.54, 2.17), and separately for males (beta 1.96; 95% CI 0.68, 3.24) but not females (beta 1.05; 95 % CI -0.04, 2.13). Noise sensitivity was independent of perceived occupational noise exposure. Pre-existing depressive symptoms at age 17 were predictive of perceived occupational noise exposure, suggesting complex interactions of noise and depression.

Project description:The primary aim of this study was to investigate whether auditory brainstem response (ABR) and speech perception in noise (SPiN) were associated with occupational noise exposure in normal hearing young factory workers. Forty young adults occupationally exposed to noise and 40 non-exposed young adults (control group) from Zhejiang province in China were selected. All participants presented with normal hearing thresholds and distortion product otoacoustic emissions. Participants were evaluated with the Mandarin Bamford-Kowal-Bench (BKB) test and ABR. The latter was obtained for click stimulus at 50, 60, 70, 80, and 90 dBnHL. Peak-to-trough amplitudes and latencies for waves I and V were obtained. The ABR wave I amplitude, the wave I/V amplitude ratio, the slope of the wave I amplitude growth as a function of stimulus intensity (AMP-ISlope), and the wave V latency shift with ipsilateral noise (LAT-VSlope) were used as ABR outcomes. Finally, equivalent continuous average sound pressure level normalized to 8 h (LAeq.8h) and cumulative noise exposure (CNE) were obtained for noise-exposed participants. No significant differences between groups were found for any ABR outcomes. Noise-exposed participants exhibited worse BKB scores than control group participants. A multivariate regression model showed that 23.3% of the variance in BKB scores was explained by group category (exposed vs. non-exposed) and hearing thresholds. However, since none of the ABR outcomes exploring cochlear synaptopathy were associated with noise exposure, we cannot conclude that cochlear synaptopathy was the contributing factor for the differences between groups for BKB scores. Factors that go beyond sensory processing may explain such results, especially given socio-economic differences between the noise-exposed and control groups. We conclude that in this sample of participants, occupational noise exposure was not associated with signs of cochlear synaptopathy as measured by ABR and BKB.

Project description:ObjectivesThe aim of this study was to create a quantitative job-exposure matrix (JEM) for noise including a large set of measurements for the Swedish workforce, a detailed exposure-level assessment, spanning over an extensive time period from 1970 to 2014.MethodsThe JEM was developed by 2 teams, each with an experienced occupational hygienist and an occupational safety engineer. Each pair assessed the exposure using measurements performed and reported by occupational hygienists, occupational safety engineers, or similar, from 1970 to 2014. The measurements included either the original LAeq(8h) measurements or an LAeq(8h) levels calculated from partial measurements of the working day, provided that the measurement targeted a regular task usually performed during a full workday. The collection of measurement reports was done in 2008 and 2012 by contacting clinics working in the area of occupational health or occupational safety engineers and their submitted reports were added to our own material. Noise exposure assessments were inserted at the appropriate time period for the relevant job family. The final matrix was developed in a consensus procedure and the validity was investigated by comparison of the 2 team's individual results.ResultsThe noise JEM contains 321 job families with information regarding occupational noise from 1970 to 2014. The time-period label has a 5-yr scale starting in 1970. The estimated average 8 h (TWA) noise level in decibels [dB(A)] for every job family and 5-yr period was coded as 1: <70 dB(A), 2: 70 to 74 dB(A), 3: 75 to 79 dB(A), 4: 80 to 84 dB(A) or 5: 85(+) dB(A). The validation showed no systematic difference in relative position and very high agreement in the ordering of paired ordinal classifications. The JEM has also successfully been applied in several epidemiological studies.ConclusionsWe present a JEM for occupational noise using Swedish data from 1970 to 2014 with a higher degree of sensitivity in assessed noise exposure compared with the previously existing version. Repeated application of the JEM, in epidemiological studies, has shown consistent results and contributed to yielding important findings.
