Adherence to topical anti-glaucoma medications in Sidama regional state, Southern Ethiopia.

ABSTRACT:

Purpose

To assess the proportion of adherence to topical anti-glaucoma medications and associated factors among glaucoma patients in Sidama regional state, Ethiopia.

Methods

An institution based cross-sectional study was conducted from May 30 to July 15, 2022 at Hawassa University comprehensive specialized and Yirgalem General Hospitals in Sidama regional state, Ethiopia. A systematic random sampling method was used to select 410 study participants. An adapted eight-item self-reported questionnaire was used to assess adherence. Binary logistic regression was used to identify factors associated with adherence to topical anti-glaucoma medications. Variables with a p-value of <0.05 under multivariable analysis were taken as statistically significant factors for adherence. The strength of the association was measured by using an adjusted odds ratio with 95% confidence interval.

Results

A total of 410 participants were included giving a response rate of 98.3%. Those who were adherent to their medications were 221(53.9%) (95% CI: 48.8-58.5). Urban residence (AOR = 2.81: 95% CI = 1.34-5.87), higher educational level (AOR = 3.17: 95% CI = 1.24-8.09), monthly follow-up frequency (AOR = 3.30: 95% CI = 1.79-6.11) and normal vision (AOR = 6.58: 95% CI = 3.03-10.84) were significantly associated with adherence.

Conclusion

More than half of glaucoma patients attending at Hawassa University comprehensive specialized and Yirgalem general hospitals were adherent to their topical anti-glaucoma medications. Urban residence, educational level, follow-up frequency, and normal vision were associated with adherence.

Project description:BackgroundFor developing countries such as Ethiopia, coffee is a commodity of great economic, social, and environmental importance. No detailed investigations have been performed on the contents of essential and toxic metals in coffee beans and soil in this study area.MethodsThe levels of essential metals (Na, K, Ca, Zn, Mn, Cu, Co, Cr, Ni) and toxic elements (Pb and Cd) were investigated in coffee beans (coffee growing farmland and coffee washed plants) and soil samples (from farmland) using flame atomic absorption spectrometry (FAAS) and flame emission atomic spectroscopy. We selected six (20%) administrative units (kebele) with purposive sampling techniques based on their coffee production capacity in Dale Woreda for soil testing. After coffee sample preparation in a microwave system with HNO3and H2O2 reagents, the accuracy of the optimized procedure was evaluated by analysing the digest of the spiked samples. Soil samples were abridged with a slight revision of the EPA 3050B acid digesting method. ANOVA was used to determine the significant differences in the mean concentration of metal within coffee beans from farmland at the various sampled sites at the p < 0.05 significance level. To correlate the effect of one metal concentration on other metals in the coffee bean samples, Pearson correlation matrices were used.ResultsCalcium had the highest concentration (1,355 ± 18.02 mg kg-1) of macroelements in soil samples, followed by K (681.43 ± 1.52 mg kg-1). Similarly, Na (111.63 ± 0.35 mg kg-1), Cu (49.96 ± 0.99 mg kg-1), Co (5.43 ± 0.31 mg kg-1), Mn (0.62 ± 0.238 mg kg-1), Ni (0.194 ± 0.01 mg kg-1), and Zn (0.163 ± 0.007 mg kg-1) were detected among the microelements in the soil samples. Pb and Cr were not detected in all soil samples. Potassium (K) was found to have the highest concentration (99.93 ± 0.037 mg kg-1), followed by Ca (17.23 ± 0.36 mg kg-1), among the macroelements in coffee beans from farmers' farms. Similar to coffee beans from farmland, samples from washed plants also contained the highest K (77.93 ± 0.115 mg kg-1), followed by Ca (4.33 ± 0.035 mg kg-1). Metal levels in coffee bean samples from farmland are in the following order: K>Na>Ca >Mn>Cu> Ni>Zn. Metal levels were found to be K>Na>Ca >Mn>Cu> Zn>Ni in coffee beans from the washed plants. Co, Cr, Pb and Cd were no detected in all coffee bean samples. Except for calcium, potassium and manganese, the levels of metals in coffee beans from farmland and washed plants were not significantly different at the 95% confidence level within a kebele.ConclusionsWe observed permitted levels of macro- and trace elements in coffee beans from farmlands and washed plants. Only in the soil samples are cadmium concentrations higher than those permitted for agricultural soil recommended by the WHO and FAO. Overall, there is no health danger linked with the use of coffee beans due to detrimental and trace heavy metals.

Project description:BackgroundHealth disparities, inconsistent outcomes, and underwhelming health services continue to be problems for all nurses and doctors. Studies from a variety of nations have found that doctors and nurses have little familiarity with evidence-based practice. There is a knowledge vacuum about the degree of evidence-based nursing practice and its contributing factors in Ethiopia as well as the current study region.ObjectiveThe study's objective was to evaluate how healthcare professionals working in government hospitals in the Sidama regional state of 2022 are implementing evidence-based clinical practice and to identify associated factors.MethodsFrom October 1 to December 30/2022, a cross-sectional institution-based study with a mix of quantitative and qualitative methods was carried out. A total of 422 healthcare workers were randomly selected to participate in the current study from 6 randomly selected public hospitals in the region. This study included 25 key informant interviews. The Friedman test and Funk's BARRIER scale were adapted into a semi-structured questionnaire quantitative. Multivariable logistic regression was used to assess the significance of the association between the dependent and independent variables. A pretest was conducted on 22 healthcare workers from Shashmane Hospital.ResultThis study found that more than half, 51.7%, of the participants had a total implementation of evidence-based practice score below the mean score (40.4). only Access to the internet [having smart phone] (Adjusted Odds Ratio (AOR) = 3.03: 95% Confidence Interval (CI): (1.75-5.26))., favorable Attitude toward EBP of participants. (Adjusted Odds Ratio (AOR) = 1.84Confidence Interval (1.12-2.70)), inadequate Self-efficacy of Evidence-based practice skills (Adjusted Odds Ratio (AOR) = 0.29 Confidence interval = 0.19-0.46), and Knowledge of evidence-based practice (Adjusted Odds Ratio (AOR) = 0.45 Confidence interval = 0.30-0.77)were factors significantly associated with EBP implementation.ConclusionMore than half of the participants,51.7%, do not implement evidence-based practice in the care of their clients, which is unacceptable. The use of EBP has been significantly associated with access to the internet (having a smartphone), participants' positive attitudes toward it, their level of skill efficacy in finding and accessing it, and their knowledge of it. Therefore, stakeholders ought to think about addressing these obstacles to the adoption of EBP.

Project description:IntroductionCOVID-19 incidence is increasing and different measures have been adopted to control the spread of the pandemic in Ethiopia. Among these measures, enhancing the knowledge, positive attitudes, and proper practices of prevention measures about the disease is a basic strategy to control it. However, community compliance to control measures is largely dependent on their knowledge, attitudes, and practices (KAP) towards COVID-19.ObjectiveTo assess the current level of KAP towards COVID-19 pandemic and predictors among the rural dwellers in Sidama regional state, Southern Ethiopia; 2020.MethodsThis community-based prospective cross-sectional study was carried out from May 1-30, 2020 on a sample of 1,278 adult populations in Sidama regional state, Southern Ethiopia. A multi-stage sampling technique was used to choice the study participants. The data were collected using a structured interviewer-administered questionnaire. We have entered data using Epi data version 3.1 and all analyses were done using SPSS version 25. KAPs scores of study participants based on their independent variables were compared using Chi-square test, t-test or one-way analysis of variance (ANOVA) as required. Bi-variable and multivariable logistic regression analyses were used to identify factors associated with KAP. The important assumptions of the logistic regression model were checked to be satisfied. Adjusted odds ratios (AOR) with a 95% confidence interval (CI) were calculated to assess the existence and strength of associations.ResultsFrom a total of 1,214 study participants, the overall attained knowledge, attitude and practice score about COVID-19 were 90%, 82.4% and 65%, respectively. Among these, 43.9%, 37.5%, and 24.4% of the study participants had demonstrated good knowledge, high attitude and proper practice, respectively. The mean knowledge scores were significantly different between sex, categories of marital status, educational levels, main occupation, and the monthly income quintiles of the study participants (p<0.05). Similarly, the mean attitude scores significantly varied across educational levels, main occupations and marital status (p<0.05). Based on multivariable logistic regression analysis, main occupation of the government employees, education level of diploma and above, highest and second highest wealth rank were positively associated with COVID-19 prevention and control practice.ConclusionsThe majority of study participants had showed good knowledge and optimistic attitude toward COVID-19. But, the level of practice lower than that expected to maximize effective control measures. Further public education interventions and community sensitization campaigns are required for rural adult population in the Sidama regional state, Ethiopia.

Project description:BackgroundGlobally, at least 1 billion people have a vision impairment that could have been easily prevented or easily treated. Cataract is the leading preventable and most treatable causes of blindness and bilateral low vision among adults. Despite being the leading cause of preventable and most treatable blindness, the lack of knowledge about the disease and its option of treatment is still a major barrier in reducing the blindness owing to cataract in the developing countries particularly in Ethiopia. Hence, the aim of this study is to determine the level of knowledge about cataract and associated factors among adults in Yirgalem Town, Sidama National Regional State, Southern Ethiopia, 2020.MethodsA community-based cross-sectional study design was conducted among randomly selected 599 adult's age 18 years and above from May 10-30, 2020. A multi-stage sampling technique was used to select the study participants. Data were collected using pre-tested and structured face-to-face interview questionnaires. The collected data were entered to Epi data version 3.1 and then exported to SPSS version 21 for analysis. Bi-variable and multivariable logistic regression was used to identify associated factors of knowledge about cataract. Adjusted Odds Ratio (AOR) together with 95% Confidence Interval (CI) was used to declare the statistical association between dependent and independent variables.ResultsOf the total study participants, 379 (64.7%), [(95% CI: 60.7-68.6%)] of them had good knowledge about cataract. Age (≥40 years) [AOR = 2.29(95% CI 1.18-4.44)], Elementary school completed [AOR = 2.31(95% CI 1.30-4.10)], High school & above [AOR = 5.55(95% CI 2.81-10.89)], governmental and non-governmental employed [AOR = 5.62 (95% CI 2.78-11.38)], Merchant [AOR = 1.72(95% CI 1.03-2.88)], Positive Attitude [AOR = 3.85(95% CI 2.94-6.47)] were positively significantly associated with knowledge about cataract. Whereas, rural residence [AOR = 0.19 (95% CI: 0.12-0.31)] was negatively associated with knowledge about cataract.ConclusionsMore than one third of the participants still had poor knowledge about cataract. This implies that health facilities should be engaged and raises the awareness of the community and empowers people about eye care needs.

Project description:IntroductionDisrespect and abusive care is a violation of women's basic human rights and it is serious global problem that needs urgent intervention. Poor quality client-provider interaction is commonly reported from family planning programmes. In Ethiopia, disrespect and abusive care is very common (21-78%) across health facilities.ObjectiveTo assess the status of respectful family planning service (client-provider interaction) in Sidama zone, south Ethiopia.MethodologyHealth facility-based cross-sectional study was conducted from June to August 2018. Data were collected from 920 family planning clients recruited from 40 randomly selected health facilities. The Mother on Respect index (MORi) questionnaire was used to collect the data through client exit interview. Partial proportional odds ordinal regression was employed to identify determinants of respectful family planning service.ResultAmong family planning clients, the level of respectful family planning service was found to be zero (0%) in the very low respect category, 75(18.5%) in the low respect category, 382(41.52%) in moderate respect category and 463(50.33%) in high respect category. Being a short acting method client (AOR = 0.30, 95%CI [0.12, 0.72]), being an uneducated client (AOR = 0.39, 95%CI [0.25, 0.61]) or a client with elementary education (AOR = 0.41, 95%CI [0.23, 0.73]), client's poverty (AOR = 0.75, 95%CI [0.56, 0.99]), and long waiting time (AOR = 0.46, 95%CI [0.30, 0.69])significantly reduced the odds of moderate and high respect compared to low respect. Conversely, preference of male service providers, service providers' work satisfaction and health workers' prior training on respectful care significantly increased the odds of moderate and high respect.ConclusionConsidering the current strategy of zero tolerance for disrespect and abuse in Ethiopia, the level of respectful care in this study is sub-optimal. Short term training for service providers on respectful care seems valuable to enhance the level of respectful care for family planning clients irrespective of their socioeconomic background.

Project description:PurposeThe present study aims to estimate the prevalence of non-compliance and improper drop administration technique among glaucoma patients and describe common obstacles to medication compliance.MethodsA hospital-based cross-sectional study, using standardized questionnaire and direct observation by study personnel was conducted among glaucoma patients aged 18 years and above at a tertiary care charitable eye hospital in North India. 151 consecutive glaucoma patients on medical therapy following up at the glaucoma clinics for at least 6 months were recruited. Non-compliance was defined as missing at-least one drop of medication per week and (or) the inability to accurately describe the medication regimen. Study personnel also assessed drop administration technique during application of eye drops by patients treating ophthalmologist-provided information, including measures of disease stability. Factors such as socioeconomic status, presence of caregiver, and number of medications with their effect on compliance were studied using chi-square statistics.ResultsAmong 151 patients interviewed, around 49% of patients reported problems in using glaucoma medications, with 16% of them reporting total non-compliance. 35% of patients demonstrated improper drop administration technique. Forgetfulness was cited as the main reason for being non-compliant and had a significant association with non-compliance (P = 0.00). Paying patients were more compliant as compared to subsidized patients (P = 0.05). Disease was more stable in compliant patients compared to non-compliant patients (P = 0.05). No other factor had significant association with compliance (P > 0.05).ConclusionsOver 50% of the patients surveyed were non-compliant, and 35% demonstrated improper administration technique. Glaucoma patients should be educated on the importance of compliance and aids that minimize forgetfulness, and delivery systems facilitating the delivery of medications to the eye could be considered to enhance patient adherence.

Project description:BackgroundContraceptive use is an important and cost-effective intervention to prevent unwanted pregnancies. People with disabilities face discrimination when it comes to using contraception and are doubly burdened by unwanted pregnancies. However, the status of contraceptive use and associated factors among reproductive-aged females with disabilities was not adequately determined in Ethiopia.ObjectiveThis study aimed to assess contraceptive use and associated factors among reproductive-age females with disabilities in Dale and Wonsho districts and Yirgalem city administration of central Sidama National Regional State, Ethiopia.MethodsA community-based cross-sectional study was conducted among randomly selected 620 reproductive-age females with disabilities living in the selected districts from June 20 to July 15, 2022. The data were collected through face-to-face interviewing techniques using a structured questionnaire. A multilevel logistic regression analysis model was employed to analyze the data. The adjusted odds ratio (AOR) with a 95% confidence interval (CI) was used to report the measures of associations.ResultsIn this study, 27.3% (95% CI [23.8%-31.0%]) of the reproductive-age females with disabilities were current contraceptive users. Regarding the methods, 82 (48.5%) of the reproductive-age females with disabilities used implants. Having good contraceptive knowledge (AOR = 9.03; 95% CI [4.39-18.6]), transport accessibility to health facilities (AOR = 2.28; 95% CI [1.32-3.94]), being an adult (25 to 34 years old) (AOR = 3.04; 95% CI [1.53-6.04]), having a hearing disability (AOR = 0.38; 95% CI [0.18, 0.79]), having paralysis of the extremities (AOR = 0.06; 95% CI [0.03-0.12]), and wheel-chaired disability (AOR = 0.10; 95% CI [0.05-0.22]) were factors associated with contraceptive use.ConclusionContraceptive use among reproductive-age females with disabilities is low. Transport accessibility, contraceptive knowledge, being in the age groups of 25 to 34 years, and the types of disability determine their contraceptive use. Therefore, designing appropriate strategies to provide contraceptive education and information and provide contraceptive services in their homes is important to enhance contraceptive use.
