Project description:PurposeTo report the clinical outcomes of quadriceps tendon repair using adjustable cortical fixation devices at a minimum 2-year follow-up.MethodsA retrospective chart review identified patients who underwent quadriceps tendon repair using adjustable cortical fixation devices between January 2017 and March 2020. Patients with a partial tendon rupture were excluded. Demographic and injury-specific variables were gathered preoperatively and postoperatively from the electronic medical record and patient-reported outcomes (Lysholm Knee Questionnaire, Lower Extremity Functional Scale, and SF-12) were collected via telephone at a minimum of 2 years postoperatively.ResultsFourteen quadriceps tendon repairs were included in a total of 13 patients. The average time to follow-up was 3.5 ± 1.2 years with a range of 1.9 to 5.7 years. The mean age of this cohort was 55.7 ± 11.6 years, and the mean body mass index was 32.9 ± 6.0. Ten injuries (71.4%) were sustained by mechanical fall, 2 patients (14.3%) suffered a direct blow to the knee, and 2 patients (14.3%) reported a noncontact injury mechanism. Thirteen quadriceps ruptures (13/14, 92.9%) underwent surgery within 10 days of their injury. One knee (7.1%) had a postoperative extensor lag of 5°, whereas another knee (7.1%) required a reoperation for manipulation under anesthesia and arthroscopic lysis of adhesions at 3 months' postoperatively. None of the included patients (0.0%) developed a tendon re-rupture, venous thromboembolism, delayed wound healing, surgical-site infection, neuropraxia or nerve injury, hardware irritation, patella fracture, or heterotopic ossification.ConclusionsIn this study, adjustable cortical fixation was a safe and effective surgical technique for quadriceps tendon repair, with adequate restoration of quadriceps function and a low rate of adverse events at 2 years postoperatively.Level of evidenceLevel IV, therapeutic case series.

Project description:Quadriceps tendon ruptures are devastating injuries that impair the extensor mechanism of the knee. Complete tears require prompt surgical intervention in order to ensure optimal clinical outcomes. Chronic, neglected ruptures- marked by severe extensor lag, muscular atrophy, and tendon retraction- are difficult to treat. Further, severe patella baja complicates surgical planning. While there is no consensus on optimal treatment for these injuries, many different techniques have been proposed. Unlike many of these approaches, our technique focuses on restoration of patellar height via patellar tendon lengthening. Once patellar height is restored, quadriceps tendon repair can be performed using the native quadriceps tendon. Both the patellar tendon lengthening and the quadriceps tendon repair are augmented with bioinductive implants to ensure optimal healing environments, enabling us to reestablish native extensor mechanism function.

Project description:Recently, although some studies of open repair of the tendon of the quadriceps femoris have been published, there have been no reports in the literature on primary arthroscopic repair. In our present study, we present two cases of quadriceps tendon injury arthroscopically repaired with excellent results. Case 1 involved a 68-year-old man who was injured while shifting his weight to prevent a fall. MRI showed complete rupture at the insertion of the patella of the quadriceps tendon. The rupture was arthroscopically repaired using both suture anchor and pull-out suture fixation methods via bone tunnels (hereafter, pull-out fixation). Two years after surgery, retearing was not observed on MRI and both Japan Orthopedic Association (JOA) Knee and Lysholm scores had recovered to 100. Case 2 involved a 50-year-old man who was also injured when shifting his weight to prevent a fall. MRI showed incomplete superficial rupture at the insertion of the patella of the quadriceps tendon. The rupture was arthroscopically repaired using pull-out fixation of six strand sutures. One year after surgery, MRI revealed a healed tendon and his JOA and Lysholm scores were 95 and 100, respectively. Thus, arthroscopic repair may be a useful surgical method for repairing quadriceps tendon injury.

Project description:Proximal hamstring injuries are a common sports and recreational injury among the active patient population. Surgical fixation of the tendons of the hamstring muscle complex, as opposed to conservative treatment alone, has shown improved patient outcomes, prompting the evolution of the suture anchors utilized in these repairs. Previous studies investigating the biomechanical properties of hamstring repair anchors have focused on double-row knotless techniques, in which the fixation of the overall construct relies on each individual anchor to maintain fixation. While these constructs have demonstrated biomechanical strength and clinical durability, each suture anchor represents a potential point of failure for the entire construct due to the crossed stitch anchor configuration. To address this limitation, recent tensionable knotless all-suture anchor designs have been implemented with success due to their smaller size and biomechanical strength. The aim of this technical note is, thus, to describe a technique for proximal hamstring repair using a tensionable knotless all-suture anchor construct that has 5 independent mattress sutures and, in doing so, employs the biomechanical strength of knotless fixation but eliminates the potential single point of failure seen with current knotless suture anchor designs.

Project description:Acute, traumatic distal biceps tendon injuries are common among the middle-aged athletic male population. Surgical repair of distal biceps tendon remains the most effective means to restore maximal strength of forearm supination and elbow flexion with relief of antecubital pain. To date, no consensus exists on the optimal fixation method during distal biceps tendon repair and multiple techniques are accepted, including 1- and 2-incision approaches and tendon fixation with suture anchors, transosseous sutures, interference screws, and cortical buttons. In this Technical Note, we demonstrate a distal biceps tendon repair technique using a single-incision approach with bicortical, tensionable suspensory button fixation.

Project description:End-stage renal disease with spontaneous quadriceps tendon rupture (QTR) is a specific condition that differs from classic QTR. The tissue quality of the quadriceps tendon (QT), the rupture site, the mechanism of injury, and the pathophysiology of the rupture mechanism all have an effect on conventional QT repair procedures, with a higher likelihood of rerupture or failed repair construction. We believe that our technique provides repair-site stability, strong repair construction, increased contact surface healing, and a reduced chance of rerupture after QT repair. Furthermore, in most patients who have end-stage renal disease with QTR, misdiagnosis and/or underestimation occurs, resulting in proximal retraction of the QT and poor results; however, this technique can be performed with alternative procedures such as augmentation or QT lengthening. The suture bridge transosseous QT repair technique relies on biomechanics knowledge for better stability. Suture bridge repair concept can achieve better healing of all layers of the QT until returning to normal activity with no disability and an improved quality of life. Technique Video Video 1 The patient is positioned supine on the operating table, with the affected leg hanging in 90° of knee flexion. The midline incision is made around 8 cm from the proximal part of the quadriceps tendon (QT) to the patella. We debride the frayed soft tissue, refresh the good-quality tendon ends, and roughen the upper pole of the patella. Five No. 2.0 pins are used to drill transosseous patellar tunnels longitudinally, which comprise 3 anterior drill holes and 2 posterior drill holes in the patellar bone. No. 2 Ethibond is inserted in the 3 anterior patellar drill holes to prepare for shuttling of FiberTape, which will be sutured to the QT. Polydioxanone (PDS) is inserted in the 2 posterior patellar drill holes for shuttling of the No. 2 Ethibond. Two FiberTape sutures are placed through the middle part of the torn tendon by the Kessler repair technique. The distance of the suture is measured 2 to 3 cm proximal to the tear site. The 4 suture tails are passed through 3 parallel longitudinal tunnels within the patella. In the 2 posterior drilling tunnels of the patella, the 4 limbs of the No. 2 Ethibond suture are sutured in a retrograde manner from the deep part to the superficial part of the QT, where they are placed at a distance 2 cm proximal to the tear site. The anterior peritendinous portion of the QT is sutured with No. 1-0 Vicryl along the anterior of the patella with a figure-of-8 suture technique. The 4 FiberTape limbs are secured by a knot at the inferior pole of the patella with the knee in full extension. All the suture limbs from the posterior patellar drill holes are tied at the inferior patellar bone, which acts like a suture bridge. The retinaculum is sutured with No. 1-0 Vicryl by a figure-of-8 suture technique. After the repair, stability is confirmed when knee flexion of up to 90° can be achieved intraoperatively without causing tension.

Project description:Medial patellofemoral ligament reconstruction risks patellar fracture with the osseous violation necessary for patellar attachment. Anatomic studies identify an entire medial patellofemoral complex of structures responsible for medial restraint to patellar lateral instability. One specific component of this complex is the medial quadriceps tendon femoral ligament (MQTFL). This note presents the technique, pearls and pitfalls, and critical surgical anatomy necessary for successful MQTFL reconstruction-a treatment strategy for patellar instability with no increased risk for patellar fracture. An autograft hamstring tendon or allograft tendon is fixed to the anatomically identified femoral origin and passed deep to the vastus medialis obliquus to then weave around the distal medial quadriceps tendon. This simulates the native anatomic interdigitation of the MQTFL with the quadriceps tendon and provides a stable restraint to prevent lateral patellar subluxation or dislocation.

Project description:Medial quadriceps tendon-femoral ligament (MQTFL) reconstruction for prevention of recurrent patella dislocation is an alternative to medial patellofemoral ligament (MPFL) reconstruction. Because the reconstruction graft attaches to the quadriceps tendon, no patella drill hole is required, thereby eliminating iatrogenic fracture risk. The procedure remains anatomically accurate and early results are comparable to MPFL reconstruction for preventing patella dislocation. The MQTFL reconstruction graft is brought up under the vastus medialis obliquus distal to the patella apex, such that its orientation is directed toward the medial patellofemoral complex (MPFC) midpoint, also known as Tanaka's point. The graft is then secured by looping it around the vastus medialis and rectus femoris tendons, after which optimal graft length is easily established by cycling the knee, after which it is sutured securely into the deep quadriceps tendon precisely at the anatomic midpoint of the MPFC. Anatomic reconstruction of the MQTFL-in which graft orientation is crucial-confers reliable patellofemoral joint stability in the surgical treatment of patients with recurrent patella dislocations.

Project description:Quadriceps tendon ruptures compromise the knee extensor mechanism and cause an inability to ambulate and significant functional limitations. Therefore, the vast majority of quadriceps tendon ruptures are indicated for operative intervention to restore patient mobility and function. Although these injuries were traditionally repaired using a transosseous repair technique, recent literature has shown that suture anchor repair may offer biomechanical advantages. Additionally, research in other areas of orthopaedics has found that a double-row suture anchor construct can offer additional biomechanical strength to tendinous repair. This technical note describes a safe and effective quadriceps tendon repair using a double-row suture anchor construct.
