Impact of the COVID-19 pandemic on cancer patients in western Pennsylvania: rural-urban disparities.

ABSTRACT:

Purpose

Disparities in cancer care persist between patients living in rural versus urban areas. The COVID-19 pandemic may have impacted concerns related to care and personal health differently in rural cancer patients. Using survey data collected from cancer patients in western Pennsylvania, we examined pandemic-related distress, concerns related to cancer care, impact on personal health, and the extent to which these differed by urban-rural residence.

Methods

Patients filled out an initial survey in August-December 2020; a second survey was completed in March 2021. The following patient concerns related to the pandemic were evaluated: threat of COVID-19 to their health, pandemic-related distress, perceptions of cancer care, and vaccine hesitancy. Multivariable logistic regression models were used to examine relationships between these outcomes and urban-rural residence as well as patient-related factors, including anxiety symptoms and social support.

Results

The study sample included 1,980 patients, 17% resided in rural areas. COVID-19 represented a major or catastrophic threat to personal health for 39.7% of rural and 49.0% of urban patients (p = 0.0017). Patients with high general anxiety were 10-times more likely to experience pandemic-related distress (p < 0.001). In the follow-up survey (n = 983), vaccine hesitancy was twice as prevalent among rural patients compared to urban (p = 0.012).

Conclusions

The extent to which perceptions of the threat of COVD-19 to personal health and vaccine hesitancy exacerbates rural-urban disparities in cancer care and prognosis warrants further study. Cancer patients may be vulnerable to heightened anxiety and distress triggered by the pandemic.

Project description:BackgroundVaccination plays an important role in preventing COVID-19 infection and reducing the severity of the disease. There are usually differences in vaccination rates between urban and rural areas. Measuring these differences can aid in developing more coordinated and sustainable solutions. This information also serves as a reference for the prevention and control of emerging infectious diseases in the future.ObjectiveThis study aims to assess the current coverage rate and influencing factors of COVID-19 (second booster) vaccination among Chinese residents, as well as the disparities between urban and rural areas in China.MethodsThis cross-sectional study used a stratified random sampling approach to select representative samples from 11 communities and 10 villages in eastern (Changzhou), central (Zhengzhou), western (Xining), and northeast (Mudanjiang) Mainland China from February 1 to February 18, 2023. The questionnaires were developed by experienced epidemiologists and contained the following: sociodemographic information, health conditions, vaccine-related information, information related to the Protective Motivation Theory (PMT), and the level of trust in the health care system. Vaccination rates among the participants were evaluated based on self-reported information provided. Binary logistic regression models were performed to explore influencing factors of vaccination among urban and rural participants. Urban-rural disparities in the vaccination rate were assessed using propensity score matching (PSM).ResultsA total of 5780 participants were included, with 53.04% (3066/5780) being female. The vaccination rate was 12.18% (704/5780; 95% CI 11.34-13.02) in the total sample, 13.76% (341/2478; 95% CI 12.40-15.12) among the rural participants, and 10.99% (363/3302; 95% CI 9.93-12.06) among the urban participants. For rural participants, self-reported health condition, self-efficacy, educational level, vaccine knowledge, susceptibility, benefits, and trust in the health care system were independent factors associated with vaccination (all P＜.05). For urban participants, chronic conditions, COVID-19 infection, subjective community level, vaccine knowledge, self-efficacy, and trust in the health care system were independent factors associated with vaccination (all P＜.05). PSM analysis uncovered a 3.42% difference in vaccination rates between urban and rural participants.ConclusionsThe fourth COVID-19 vaccination coverage rate (second booster) among the Chinese population was extremely low, significantly lower than the previous vaccine coverage rate. Given that COVID-19 infection is still prevalent at low levels, efforts should focus on enhancing self-efficacy to expand the vaccine coverage rate among the Chinese population. For rural residents, building awareness of the vaccine's benefits and improving their overall health status should be prioritized. In urban areas, a larger proportion of people with COVID-19 and patients with chronic illness should be vaccinated.

Project description:The COVID-19 pandemic that has hit the entire world since the beginning of 2020 is an unimaginable phenomenon. The pandemic is disastrous because it has caused loss of life and livelihood for a large part of the population. People lose their jobs, spaces for social interaction are closed, and social relationships are disrupted. Several studies show that social solidarity should be a major concern for people to sustain the future quality of their lives. Social solidarity dimensions in this study include gotong royong (local culture of mutual help), marriage banjars (local association), cooperation, and sharing of information. This study aims to examine the existence of social solidarity during the pandemic in rural and urban areas and to know the level of community exposure to COVID-19 in Lombok Island, Indonesia. This research adopted a quantitative approach to identify and analyse the existence of social solidarity in rural and urban communities during the COVID-19 pandemic. A set of questionnaires was distributed and shared by enumerators with 1,100 targeted-respondents from Lombok Island. The survey was carried out from 14th October to 28th October 2021, that is, during a recovery period from the pandemic while restrictions implemented by the government were still in full force. Chi square statistical analysis was used to test whether there were differences in social solidarity between those who lived in rural areas and those who lived in urban areas. This research found the existence of social solidarity in both rural and urban communities during the pandemic. The level of social solidarity in rural areas is higher than in urban areas. While related to the number of those who were exposed and died, conditions were higher in rural areas than in urban areas. However, in terms of the death rate, the level of death rate in urban areas is higher than in rural areas. This condition indicates differences in the characteristics of rural and urban communities, which can be considered in implementing policies during a future pandemic. This research provides evidence for local governments in formulating policies with a social solidarity perspective by taking into account the different characteristics of rural and urban communities.
