A flexible optoacoustic blood 'stethoscope' for noninvasive multiparametric cardiovascular monitoring.

ABSTRACT: Quantitative and multiparametric blood analysis is of great clinical importance in cardiovascular disease diagnosis. Although there are various methods to extract blood information, they often require invasive procedures, lack continuity, involve bulky instruments, or have complicated testing procedures. Flexible sensors can realize on-skin assessment of several vital signals, but generally exhibit limited function to monitor blood characteristics. Here, we report a flexible optoacoustic blood 'stethoscope' for noninvasive, multiparametric, and continuous cardiovascular monitoring, without requiring complicated procedures. The optoacoustic blood 'stethoscope' features the light delivery elements to illuminate blood and the piezoelectric acoustic elements to capture light-induced acoustic waves. We show that the optoacoustic blood 'stethoscope' can adhere to the skin for continuous and non-invasive in-situ monitoring of multiple cardiovascular biomarkers, including hypoxia, intravascular exogenous agent concentration decay, and hemodynamics, which can be further visualized with a tailored 3D algorithm. Demonstrations on both in-vivo animal trials and human subjects highlight the optoacoustic blood 'stethoscope''s potential for cardiovascular disease diagnosis and prediction.

Project description:In the present global context, continuous blood pressure (BP) monitoring is paramount in addressing the global mortality rates attributed to hypertension. Achieving precise insights into the human cardiovascular system necessitates accurate measurement of BP, and the accuracy depends on the faithful recording of oscillations or pulsations. This task ultimately depends on the caliber of the pressure sensor embedded in the BP device. In this context, we have fabricated a flexible resistive pressure sensor based on reduced graphene oxide (rGO) and a polydimethylsiloxane (PDMS) sponge that is highly flexible and sensitive. The designed device operates effectively with a minimal bias voltage of 500 mV, at which point it showed its maximum relative change in current, reaching approximately 25%. Additionally, the sensing device showed a notable change in resistance values, exhibiting almost 100% change in resistance when subjected to a pressure of 400 mmHg and high sensitivity of 0.27 mmHg-1. After promising outcomes were obtained during static pressure measurement, the sensor was used for BP monitoring in humans. The sensor accurately traced the oscillometric waveform (OMW) for distinct systolic blood pressure (SBP) and diastolic blood pressure (DBP) combinations to cover a range of medical situations, including hypotension, standard or normal, and hypertension. The values of SBP, DBP, and MAP were derived from the sensor's output using the MAA technique, and the errors in these values concerning the simulator and the traditional BP monitor follow the universal AAMI/ESH/ISO protocols. Bland-Altman (B&A) correlation and scatter plots were used to compare the sensor's results and further validate the proposed sensor. The sensor showed the mean and standard deviation error in the SBP, DBP, and MBP of -0.2 ± 5.9, -0.5 ± 7, and -0.9 ± 4.7 mmHg when compared with the noninvasive blood pressure (NIBP) simulator. The pulse rate (PR) was also calculated from the same OMW for the specified value of 80 beats per minute (bpm) given by the simulator and reported a mean PR value of ∼81 bpm, close to the reference value. The findings show that the flexible resistive sensing device can accurately measure BP and replace the existing sensors of BP devices.

Project description:BackgroundCurrent blood glucose monitoring (BGM) methods are often invasive and require repetitive pricking of a finger to obtain blood samples, predisposing individuals to pain, discomfort, and infection. Noninvasive blood glucose monitoring (NIBGM) is ideal for minimizing discomfort, reducing the risk of infection, and increasing convenience.ObjectiveThis review aimed to map the use cases of artificial intelligence (AI) in NIBGM.MethodsA systematic scoping review was conducted according to the Arksey O'Malley five-step framework. Eight electronic databases (CINAHL, Embase, PubMed, Web of Science, Scopus, The Cochrane-Central Library, ACM Digital Library, and IEEE Xplore) were searched from inception until February 8, 2023. Study selection was conducted by 2 independent reviewers, descriptive analysis was conducted, and findings were presented narratively. Study characteristics (author, country, type of publication, study design, population characteristics, mean age, types of noninvasive techniques used, and application, as well as characteristics of the BGM systems) were extracted independently and cross-checked by 2 investigators. Methodological quality appraisal was conducted using the Checklist for assessment of medical AI.ResultsA total of 33 papers were included, representing studies from Asia, the United States, Europe, the Middle East, and Africa published between 2005 and 2023. Most studies used optical techniques (n=19, 58%) to estimate blood glucose levels (n=27, 82%). Others used electrochemical sensors (n=4), imaging (n=2), mixed techniques (n=2), and tissue impedance (n=1). Accuracy ranged from 35.56% to 94.23% and Clarke error grid (A+B) ranged from 86.91% to 100%. The most popular machine learning algorithm used was random forest (n=10) and the most popular deep learning model was the artificial neural network (n=6). The mean overall checklist for assessment of medical AI score on the included papers was 33.5 (SD 3.09), suggesting an average of medium quality. The studies reviewed demonstrate that some AI techniques can accurately predict glucose levels from noninvasive sources while enhancing comfort and ease of use for patients. However, the overall range of accuracy was wide due to the heterogeneity of models and input data.ConclusionsEfforts are needed to standardize and regulate the use of AI technologies in BGM, as well as develop consensus guidelines and protocols to ensure the quality and safety of AI-assisted monitoring systems. The use of AI for NIBGM is a promising area of research that has the potential to revolutionize diabetes management.
