Does photobiomodulation on the root surface decrease the occurrence of root resorption in reimplanted teeth? A systematic review of animal studies.

ABSTRACT: This review aimed to answer the following question "Does photobiomodulation treatment of the root surface decrease the occurrence of root resorption in reimplanted teeth?" Electronic searches were performed in the MEDLINE/PubMed, Cochrane Library, Scopus, Web of Science, Embase, and Grey Literature Report databases. Risk of bias was evaluated using SYRCLE Risk of Bias tool. The Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) tool was used to assess the certainty of evidence. In total, 6 studies were included. Five studies reported a reduced occurrence of root resorption in teeth that received photobiomodulation treatment of the root surface prior to replantation. Only 1 study reported contradictory results. The photobiomodulation parameters varied widely among studies. GRADE assessment showed a low certainty of evidence. It can be inferred that photobiomodulation treatment of the root surface prior to replantation of teeth can reduce the occurrence of root resorption. Nonetheless, further clinical studies are needed.

Project description:BackgroundControversial results have been reported regarding the impact of photobiomodulation (PBM) on orthodontically induced inflammatory root resorption (OIIRR). The aim of this study was to evaluate the influence of two PBM protocols, one of them requiring a high application frequency (on days 0, 3, 7, 14, then every 2 weeks), while the second requires less frequent applications (every 3 weeks), on OIIRR accompanying orthodontic treatment.MethodsTwenty female patients were recruited for this randomized controlled trial, requiring the therapeutic extraction of maxillary first premolars, and they were randomly divided into 2 equal groups. In Group A, one side of the maxillary arch randomly received PBM on days 0, 3, 7, 14, and every 2 weeks thereafter, while in Group B, one side was randomly chosen to receive PBM every 3 weeks. The laser applied was a Diode laser with a wavelength of 980 nm, in a continuous mode. Canine retraction in both groups was carried out using closed-coil springs, delivering 150 g of force, and the force level was checked every 3 weeks, over a 12-week study period. Pre-retraction and post-retraction cone-beam computed tomography (CBCT) was done for the evaluation of OIIRR.ResultsNo significant differences in the amount of OIIRR have been reported between the laser and control sides in both groups A and B. Also, no significant differences have been reported between the laser sides in both groups.ConclusionsPhotobiomodulation does not affect OIIRR, whether by increasing or decreasing its occurrence, with both laser application protocols. Therefore, it can be stated that PBM does not result in root resorption less than the commonly observed range elicited with conventional orthodontic treatment, and that it has no effect on OIIRR. Project description:BackgroundOrthodontically induced root resorption (OIRR) is a common side effect of orthodontic treatment. This study compares the degree of OIRR between root-filled teeth (RFT) and vital pulp teeth (VPT), and analyzes relevant study variables.MethodsWe conducted a retrospective study on 69 patients who had undergone orthodontic treatment. Using Cone-beam computed tomography (CBCT), we measured changes of root length before and after treatment through a unique method involving three-dimensional (3D) image registration and superimposition. Factors related to the OIRR such as gender, type of treatment, tooth type, age, duration of treatment and distance of root movement were considered.ResultsThe sample included 55 females and 14 males aged 27.19 ± 6.08 years. On the basis that there was no significant difference in the root movement distance between RFT and VPT, RFT showed significantly less OIRR than VPT (P < 0.05). Gender did not significantly impact on OIRR for either RFT or VPT group (P > 0.05). In women specifically, RFT displayed less resorption than VPT (P < 0.05). For treatment type, extraction cases demonstrated a lower degree of OIRR in RFT than VPT (P < 0.05), and notable greater OIRR in with-extraction group compared to no-extractions group was found in RFT (P < 0.05), but not in VPT (P > 0.05). Tooth type did not yield significant differences in OIRR overall; however, upper teeth and premolars experienced lower resorption in RFT than in VPT (P < 0.05). Cases treated with fixed appliance had higher OIRR in both RFT and VPT than those with clear aligners (P < 0.05). Age did not correlate significantly with OIRR for either group (P > 0.05). And duration of treatment positively correlated with OIRR for both types (RFT: r = 0.5506, P < 0.0001; VPT: r = 0.4371, P = 0.0002), so did root movement distance (RFT: r = 0.2955, P = 0.0140; VPT: r = 0.2790, P = 0.0206).ConclusionsRFT exhibit significantly less OIRR than VPT after orthodontic treatment. Treatment type, appliance type, duration of treatment and root movement distance are significant factors influencing OIRR. Personalized orthodontic treatment plans and vigilant monitoring are crucial to mitigate OIRR risks.

Project description:BackgroundThe long-term use of contraceptive methods that contain estrogens, progestogens or combinations of the above among women aged 15 to 49 years is extensive. Both estrogens and progestogens affect bone metabolism.ObjectiveTo systematically investigate and appraise the quality of the available evidence from animal studies regarding the impact of exogenous administration of female sex hormones on the rate of orthodontic tooth movement and root resorption.Search methodsSearch without restriction in seven databases (including grey literature) and hand searching were performed until May 2021.Selection criteriaWe looked for controlled animal studies investigating the effect from exogenous administration of formulations containing female sex hormones on the rate of orthodontic tooth movement and root resorption.Data collection and analysisAfter study retrieval and selection, relevant data was extracted, and the risk of bias was assessed using the SYRCLE's Risk of Bias Tool. The quality of available evidence was assessed with the Grades of Recommendation, Assessment, Development, and Evaluation.ResultsThree studies were identified, all being at unclear risk of bias. Overall, administration of progesterone and the combinations of estradiol with norgestrel and desogestrel were shown to significantly decrease the rate of orthodontic tooth movement when given for longer periods (>3 weeks). Inconsistent information was detected for shorter periods of consumption. Estradiol, with desogestrel use, resulted in less root resorption. The quality of the available evidence was considered to be low.ConclusionsExogenous administration of female sex hormones may decelerate in the long term the rate of tooth movement and decrease orthodontically induced root resorption in animals. Until more information becomes available, an orthodontist should be able to identify a patient consuming such substances and understand the potential clinical implications and adverse effects that may arise.RegistrationPROSPERO: CRD42017078208; https://clinicaltrials.gov/.
