Patient safety culture is an influencing factor that affects the second-victim pain and turnover intention. Among them, the non-punitive response to errors, open communication, cooperation between different departments, organizational learning and promotion has a statistically significant influence on the second-victim pain and turnover intention.

Project description:ObjectivesThe unrelenting migration trend of Filipino nurses to other countries has threatened the quality of patient care services in the country. This study explored the extent of nurses' organizational commitment and turnover intention in the Philippines. Furthermore, predictors of nurses' organizational commitment and turnover intention were identified.MethodsA cross-sectional research design was adopted for this study. Two hundred nurses from nine rural hospitals in the Central Philippines were asked to participate in the study and 166 nurses responded (an 83% response rate). Two standardized instruments were used: the Organizational Commitment Questionnaire and the Six-item Turnover Intention Inventory Scale.ResultsFindings revealed that Philippine nurses were moderately committed (3.13 ± 0.24) to and were undecided (2.42 ± 0.67) whether or not to leave their organization. Nurses' age (P = 0.006), gender, (t = -2.25, P = 0.026), education (t = 2.38, P < 0.001), rank(t = 4.38, P < 0.001), and work experience (t = 2.18, P = 0.031) correlated significantly with organizational commitment, while nurses' age (P = 0.028) and education (t = 1.99, P = 0.048) correlated significantly with turnover intention. An inverse relationship was identified between the organizational commitment and turnover intention (r = -0.22, P = 0.005).ConclusionThe findings of this study highlight the need for formulation and implementation of interventions to promote life-long commitment in nurses and to reduce turnover rates.

Project description:BACKGROUND:A strong patient safety culture (PSC) may be associated with improved patient outcomes in hospitals. The mechanism that explains this relationship is underexplored; missed nursing care may be an important link. PURPOSE:The purpose of this study was to describe relationships among PSC, missed nursing care, and 4 types of adverse patient events. METHODS:This cross-sectional study employed primary survey data from 311 nurses from 29 units in 5 hospitals and secondary adverse event data from those same units. Analyses include analysis of variance and regression models. RESULTS:Missed nursing care was reported to occur at an occasional level (M = 3.44, SD = 0.24) across all 29 units. The PSC dimensions explained up to 30% of the variance in missed nursing care, 26% of quality of care concerns, and 15% of vascular access device events. Missed care was associated with falls (P < .05). CONCLUSIONS:Prioritized actions to enhance PSC should be taken to reduce missed nursing care and adverse patient outcomes.

Project description:In times of global demographic changes, strategies are needed for improving nursing staff retention. We examined the association of care setting (nursing homes and home care) with geriatric nurses' intention to leave their job and their profession. Thus far, it is unclear why nurses' turnover intention and behaviour do not differ between care settings, although working conditions tend to be better in home care. We used the Job Demands-Resources model to explain indirect and buffering effects by job demands (time pressure, social conflicts) and resources (task identity, supervisor support, and co-worker support) via nurses' perceived health and job satisfaction on nurses' leaving intentions. The present cross-sectional questionnaire study was conducted with a sample of N = 278 registered nurses and nursing aides in German geriatric care. As expected, there was no direct relationship between care setting and leaving attitudes. Demands and resources predicted the intention to leave with job satisfaction as mediator. We found more demands in nursing homes but no differences in resources. Serial mediation effects of care setting on intentions to leave via demands/resources and health/job satisfaction as mediators were found only for time pressure and social conflicts. Unexpectedly, there were no clear differences between intention to leave the job and the profession. As hypotheses were only partly confirmed, other buffering and detrimental effects on leaving intentions are discussed. The present data suggest that detailed concepts for personnel and career planning in geriatric care are needed.

Project description:AimTo explore the prevalence of turnover intentions among emergency nurses across the globe, decision-makers should be offered evidence-based assistance.Background and introductionCompared with those of general nurses, the unique work environment and pressure significantly impact emergency nurses' turnover intention. High personnel turnover intention often hinders the provision of high-quality emergency services.MethodsThis study was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Published and unpublished papers were identified through electronic searches of PubMed, Web of Science, EMBASE, CINAHL, and the Cochrane Library from their establishment until February 1, 2023. The literature included in this study may encompass cross-sectional studies and longitudinal studies. Two researchers independently screened the literature, extracted data, and assessed the quality of the included studies while using the tool developed by Hoy and colleagues in 2012. Stata 17.0 was used for all the statistical analyses.ResultsThis study included 12 articles by screening 744 articles, which included a total of 4400 nurses. All studies included in the analysis were cross-sectional. The overall prevalence of turnover intention among emergency nurses was 45%. Further analysis revealed that the turnover intention prevalence among emergency nurses in Asia was 54%, whereas in other regions, it was 38%. The turnover intention among younger nurses (61%) was significantly greater than that among older nurses (30%). Compared with the published scale, the self-developed scale resulted in a higher turnover intention rate of 52%, which was 41%.ConclusionThe prevalence of emergency nurses' turnover intention is relatively high and shows an increasing trend, with noticeable variations across different regions and age groups. Notably, Asian nurses and those younger than 35.6 years exhibit a greater intention to turnover.Patient or public contributionThere is no patient or public involvement, as this article is a meta-analysis.Implications for nursing and health policyNursing managers, administrators, and policymakers must recognize the seriousness of high turnover intentions among emergency nurses and develop effective prevention strategies to address this issue globally.

Project description:BackgroundNurse burnout and turnover intention significantly impact global healthcare systems, especially intensified by the COVID-19 pandemic. This study employs network analysis to explore these phenomena, providing insights into the interdependencies and potential intervention points within the constructs of burnout and turnover intention among nurses.MethodsA cross-sectional study was conducted with 560 nurses from three tertiary hospitals in Hangzhou, China. Data were collected via online questionnaires, including the Maslach Burnout Inventory-General Survey (MBI-GS) and the Turnover Intention Questionnaire (TIQ). Network analysis was performed using Gaussian graphical models to construct the network model and calculate related metrics.ResultsThe network analysis revealed that items related to personal accomplishment and emotional exhaustion were central, indicating significant roles in influencing nurses' turnover intentions. Specifically, perceived meaningful work and self-efficacy emerged as pivotal nodes, suggesting that enhancing these can mitigate turnover intentions. The network's stability and accuracy were confirmed through bootstrapping methods, emphasizing the robustness of the findings.ConclusionThe study underscores the importance of addressing nurse burnout by focusing on core elements like personal accomplishment and self-efficacy to reduce turnover intentions. These insights facilitate targeted interventions that could improve nurse retention and stability within healthcare systems. Future research should expand to multi-center studies to enhance the generalizability of these findings.

Project description:BackgroundEstablishing a positive safety-culture environment is essential in healthcare settings to enhance patient care. This study aimed to determine the relationship between critical care nurses' perceptions of patient safety culture and adverse events.MethodsA cross-sectional study was conducted among 200 nurses working in critical care units in the Damanhour Governorate in Egypt. Data were collected using a self-administered questionnaire, including the Hospital Survey of Patients' Safety Culture (HSOPSC) and information on adverse events (AEs).ResultsThe study revealed areas for improvement in patient safety culture, with low positive response rates in staffing (26.6%), non-punitive response to errors (38%), handoffs and transitions (39.4%), teamwork across and within units (42.3%), and overall perception of patient safety (49.3%). The majority of critical care nurses had a moderate to high level of overall perception of patient safety at 42.5% and 42.0%, respectively. The most frequent adverse events reported daily were complaints from patients or their families (65.5%). Adverse drug events and patient falls occurred several times per week in 56.5% and 57.0% of patients, respectively. A significant association was found between low safety culture perception and higher rates of patient falls (p = .008), adverse drug events (p = .005), and patient/family complaints (p = .030).ConclusionThe findings of the study indicate that nurses' perceptions of patient safety culture are moderate. Adverse medication responses, falls, and complaints from patients or their families were noted. Female nurses aged 31 to 40, especially divorced nurses, had more experience, worked fewer than 8 h daily, and had a higher education level, which appeared to influence overall safety culture perceptions. Furthermore, there was a correlation between the prevalence of adverse events and patient safety culture, with cooperation being the key factor.

Project description:ObjectiveThis study aimed to review the relationship between job embeddedness and turnover intentions among nurses and explore the effects of the Job Embeddedness Scale, number of years in the career, education, and marital status on this relationship.MethodsThe review was conducted by searching the China Knowledge Resource Integrated Database (CNKI), Weipu Database (CQVIP), China Biology Medicine (CBM), Wanfang Database, PubMed, Web of Science, Embase, CINAHL, and APA-PsycNet for articles on nurses' job embeddedness and turnover from intention up to March 2024. The research quality was evaluated using the Agency for Healthcare Research and Quality (AHRQ) assessment criteria. The review protocol has been registered on PROSPERO [CRD42023483947].ResultsThe results of this review included 47 studies consisting of 15,742 nurses from seven countries worldwide. A moderate negative correlation was found between job embeddedness and turnover intention (r = -0.487). Furthermore, on-the-job embeddedness (r = -0.527) was more negatively associated with turnover intention than off-the-job embeddedness (r = -0.234). The highest negative correlation was found between sacrifice and turnover intention (r = -0.460), while the lowest was for the link (r = -0.185). Furthermore, the relationship between job embeddedness and its dimensions with turnover intention was affected by different job embeddedness scales, number of years in the career, education, and marital status (P < 0.05).ConclusionThis systematic review and meta-analysis analyzed the relationships between nurses' job embeddedness, dimensions, and turnover intention. Meanwhile, subgroup analysis and meta-regression explored the factors influencing these relationships. It is an important reference for nurse managers to promote nurse retention.

Project description:BackgroundThis study aimed to investigate the turnover intention among nurses in eastern China and explore the association between turnover intention and personal characteristics, family factors, and work-related factors.MethodsA total of 2504 nurses participated in a cross-sectional survey administered in 26 hospitals in Eastern China from October to November 2017. In December 2021, a survey was conducted on nurses who resigned between December 2017 and November 2021.ResultsThe turnover intention score of in-service nurses was 15 (12-17), and 43% of nurses had a high turnover intention, which was mainly due to the following reasons: age < 40 years, raising two or more children, monthly income of USD786.10-1572.20 or < USD786.10, occupation was assigned or selected according to parental wishes, ≤ 1 or ≥ 2-night shifts per week, contractual or third-party personnel agents, full-time nurses with part-time jobs, and high job stress. Among 102 retired nurses, 80.4% reported family reasons for leaving, 39.2% for work reasons, and 21.6% for other personal reasons.ConclusionNurses' intention to leave their occupation is high in Eastern China. Age < 40 years old, > 1 child, low income, involuntary career selection, frequent night shifts, informal employment, part-time, and high job stress are significant factors associated with nurses' willingness to leave. Government and hospital administrators should consider ways to address these factors to retain nurses in hospitals in eastern China and improve the quality of nursing services.

Project description:ObjectiveThis study aims to determine the mediating effects of psychological empowerment on abusive supervision and turnover intention as perceived by nurses to provide information to change the status of nurse turnover.MethodsA cross-sectional survey (a questionnaire examining perceptions of abusive supervision, measurement of psychological empowerment, and questionnaire for turnover intention) was used to collect data. A total of 1127 clinical nurses, who were recruited through convenience sampling, participated in the survey.ResultsNurses' average perceived abusive supervision, psychological empowerment, and turnover intention scores were 1.62 ± 0.95, 3.24 ± 0.83, and 14.17 ± 3.78, respectively. Psychological empowerment was found to mediate the relationship between abusive supervision and turnover intention (P < 0.01). Turnover intention tends to be stronger and psychological empowerment reduced when nurse managers adopt an abusive leadership style.ConclusionsNurses' psychological empowerment is an intermediary variable that predicts the relationship between abusive supervision and turnover intention. Nurse managers should manage abusive supervision to increase nurses' psychological empowerment and decrease turnover intention.

Project description:BackgroundThe turnover intention of nurses from their current positions have become a critical global public health challenge. In Iran, as a country facing with several of healthcare challenges, understanding the rate of nurses' turnover intention is of paramount importance. This study aimed to determine the pooled proportion of turnover intention among Iranian hospital nurses through systematic review and meta-analysis.MethodsThis systematic review and meta-analysis adhered to PRISMA guidelines. Relevant studies were identified through a comprehensive search of English and Persian databases, including Web of Science, PubMed, Scopus, Science Information Database, IranMedex, and Magiran from January 2000 to November 2024. The Joanna Briggs Institute checklist was employed to assess the methodological quality of included studies. Pooled mean and standard error with 95% CI were calculated using a random effect model. Data analysis was performed using Comprehensive Meta-Analysis software, with the I² statistic used to evaluate heterogeneity among studies and Egger's test and visual inspection of funnel plots used to investigate the publication bias.ResultsOverall, 42 studies were included in this systematic review and meta-analysis. In total, 12,898 nurses participated in the 42 included studies. The overall turnover intention was 2.96 ± 0.056 (95% CI: 2.85-3.07, P = 0.001). Based on subgroup analysis, the turnover intention was 2.95 ± 0.123 and 2.96 ± 0.064 before and after the implementing of health transformation plan (HTP). Also, subgroup analysis by COVID-19 pandemic showed that the turnover intention was 2.97 ± 0.072 and 2.94 ± 0.095 after and before COVID-19 pandemic, respectively. Based on visual inspection of the funnel plot, and egger's test (2.04 ± 2.96, p = 0.45), no evidence of publication bias was observed.ConclusionNurses turnover intention in Iran are at a moderate level, in general and, before and after the implementation of the HTP and the COVID-19 pandemic. These results support the need to formulate evidence-based policies and plans to enhance nurse retention, contributing to the overall stability of the Iranian health system.
