Frameworks for cultural adaptation of psychosocial interventions: A systematic review with narrative synthesis.

ABSTRACT:

Introduction

Psychosocial dementia interventions may be less effective when used with populations for whom they were not initially intended. Cultural adaptation of interventions aims to increase effectiveness of interventions by enhancing cultural relevance. Use of theoretical frameworks may promote more systematic cultural adaptation. The aim of this review was to provide a comprehensive synthesis of published cultural adaptation frameworks for psychosocial interventions to understand important elements of cultural adaptation and guide framework selection.

Method

Five scientific databases, grey literature and reference lists were searched to January 2023 to identify cultural adaptation frameworks for psychosocial interventions. Papers were included that presented cultural adaptation frameworks for psychosocial interventions. Data were mapped to the framework for reporting adaptations and modifications to evidence-based interventions, then analysed using thematic synthesis.

Results

Twelve cultural adaptation frameworks met inclusion criteria. They were mostly developed in the United States and for adaptation of psychological interventions. The main elements of cultural adaptation for psychosocial interventions were modifying intervention content, changing context (where, by whom an intervention is delivered) and consideration of fidelity to the original intervention. Most frameworks suggested that key intervention components must be retained to ensure fidelity, however guidance was not provided on how to identify or retain these key components. Engagement (ways to reach and involve recipients) and cultural competence of therapists were found to be important elements for cultural adaptation.

Conclusions

Comprehensive frameworks are available to guide cultural adaptation of psychosocial dementia interventions. More work is required to articulate how to ensure fidelity during adaptation, including how to identify and retain key intervention components.

Project description:BackgroundMental health crisis presentations are common in those who have a diagnosis of borderline personality disorder (BPD), and psychosocial interventions should be provided. However, there is limited evidence outlining what a crisis-focused psychosocial intervention for this population should include.AimsTo conduct a systematic review and narrative synthesis of crisis-focused psychosocial interventions for people diagnosed with BPD.MethodThree databases (MEDLINE, Embase, PsycInfo) were searched for eligible studies. Studies were included if they were quantitative studies comparing a crisis-focused intervention with any control group and they included adults (18+ years of age) who were diagnosed with BPD (or with equivalent experiences). A narrative synthesis was undertaken to analyse results.ResultsA total of 3711 papers were initially identified, 95 full texts were screened and 5 studies were included in the review. Two of five studies reported on the same trial, so four individual trials were included. Overall moderate risk of bias across studies was identified. The review tentatively demonstrated that crisis-focused psychosocial interventions are feasible and acceptable to people with BPD and that they have potential impacts on outcomes such as self-harm and number of days spent in hospital. There is limited consensus on what outcome measures should be used to assess the impact of interventions.ConclusionsThere is presently insufficient data to recommend any specific psychosocial crisis intervention for people with BPD. Given the relationship between BPD and the high frequency of crises experienced by this group, further large-scale trials examining crisis-focused psychosocial interventions are required.Statement on languageWe acknowledge that the term personality disorder can be controversial and stigmatising. As the term borderline personality disorder has been retained in DSM-5 and is used in the research evidence base we have decided to use this term throughout this review. However, we recognise that this term may not be acceptable to all.

Project description:BACKGROUND:Paediatric burns are highly painful and traumatising injuries that are overrepresented among Aboriginal and Torres Strait Islander people. Paediatric burn patients' pain remains poorly managed by pharmacological interventions, leading to increased anxiety, distress, and trauma in patients and their caregivers. Non-pharmacological psychosocial interventions have been suggested as effective in reducing pain and psychological morbidities among paediatric burn patients and their caregivers; however, their degree of effectiveness and appropriateness for Aboriginal and Torres Strait Islander people is unclear. METHODS:A non-date restricted systematic review was conducted through four databases. Studies published in English assessing psychosocial interventions on paediatric burn patients' physical pain along with theirs and/or their caregiver's anxiety, distress, or trauma symptoms were identified and included in this review. Included studies were assessed for their ability to reduce one of the outcomes of interests and for their reflection of Aboriginal and Torres Strait Islander peoples' perspectives of health. RESULTS:Of the 3178 identified references, 17 were eligible. These include distraction based techniques (n = 8), hypnosis/familiar imagery (n = 2), therapeutic approaches (n = 4), and patient preparation/procedural control (n = 3). Distraction techniques incorporating procedural preparation reduced pain, while discharge preparation and increased 'patient control' reduced patient and caregiver anxiety; and internet based Cognitive Behaviour Therapy reduced short-term but not long-term post-traumatic stress symptoms. No interventions reflected Aboriginal and Torres Strait Islander peoples' perspectives of health; and few targeted caregivers or focused on reducing their symptoms. CONCLUSIONS:The development and assessment of psychosocial interventions to appropriately meet the needs of Aboriginal and Torres Strait Islander paediatric burn patients is required.

Project description:IntroductionAgricultural workplaces have a high number of incidents and fatalities, with the majority occurring from machinery use. Farmers' behaviour plays a critical role in maintaining safety, as improper or unsafe practices often lead to injuries and fatalities. This review categorises interventions targeting farm machine safety, examining both the behaviour change techniques (BCTs) used and their reported outcomes to understand how the techniques influence safety practices and outcomes on farms.MethodsThe systematic review is reported in accordance with the Preferred reporting items for systematic reviews and meta-analysis guidelines. Seven electronic databases were searched for relevant studies published before June 2024, and the quality of included studies was assessed using Cochrane risk of bias assessment tools. Analysis of intervention behavioural components was guided by the behaviour change wheel framework and BCT taxonomy (v1). The findings were synthesised using a narrative review.ResultsNine studies were included and a total of 21 BCTs were identified. The most frequently coded BCTs were 4.1 (instruction on how to perform the behaviour), 10.8 (incentive [outcome]), and 16.3 (vicarious consequences) (each n = 6). Reported outcomes included reductions in injury rates, improved adoption of safety devices, implementation of safety measures, and positive shifts in safety norms and perceptions. However, due to variations in intervention design and reporting, assessing the direct impact of specific BCTs on these outcomes proved challenging.DiscussionThe use of BCT taxonomy provided a common language for describing intervention components and enabled the standardisation of intervention content analysis. While patterns were observed regarding the commonly used BCTs, their implementation and outcomes, the heterogeneity and limited details provided by studies limited our ability to discern their effectiveness. Providing (i) greater transparency in reporting active intervention components and (ii) clearer connections between components and specific outcomes, will enable enhanced comparisons of future studies, and facilitate a greater understanding of how to support safe machine-related behaviours on farms.
