The role of military service in preventing depression in China: evidence from a nationally representative longitudinal survey.

ABSTRACT: Despite recognition in the West that military veterans experience more mental health issues than the general population, little research has focused on this subject in China. This study examined the associations between male veterans' military experience and depression in China. A sample of 12,914 men including 669 veterans was included in the final analysis and propensity score matching, multivariable regressions and fixed effect model were used. The military experience was associated with a lower likelihood of depression in male veterans. In the subgroup analysis, military experience was associated with a lower likelihood of depression among married and urban male veterans. Military experience was also associated with a lower likelihood of depression in both "junior college and above" and "below junior college" groups. In contrast, evidence was lacking regarding the associations between military experience and depression for unmarried and rural veterans. Individual characteristics could influence the relationship between military experience and depression in male veterans, and the mental health of veterans should be paid more attention and guaranteed.

Project description:The decline in healthy behavior in young people is a concern for public health in general and for country's defense. The aim of this study is to identify and compare health behaviors and psychological distress between male conscripts enlisted and rejected for military service. This cross-sectional study included 1243 men aged 19-26 years (mean age 22.50 ± 2.43 years). We assessed health behaviors (physical activity, adherence to healthy eating patterns, cigarette smoking, and alcohol consumption) and psychological distress. Among all conscripts, 44.7% were physically inactive, 50.2% had low adherence to healthy nutrition, 9.6% were heavy drinkers, 62.3% were current smokers, and 9.1% had high psychological distress level. Compared with physically inactive conscripts, physically active conscripts were more likely to be enlisted (adjusted odds ratio (OR) = 1.42; 95% confidence interval (CI) 1.11-2.03). Compared with current nonsmokers, current smokers were less likely to be enlisted (OR = 0.58; CI 0.39-0.86). Compared with conscripts with a high distress level, those with a low distress level were almost four times more likely to be enlisted (OR = 0.26; 95% CI 0.12-0.55). Adherence to guidelines for healthy eating and alcohol consumption was not significantly related to enlistment. These findings suggest that health behaviors in male conscripts are unsatisfactory. That is, about half are physically inactive, have a poor diet, and smoke, and nearly one in 10 is a heavy drinker and has a high psychological distress level. The enlisted conscripts were more likely to be sufficiently physically active and less likely to be a current smoker or have a high distress level. Early intervention programs to provide a heathier population of young men for conscription should focus on mental well-being and target health-related behaviors such as physical activity and not smoking. Preferably, these should be implemented as health education programs in schools to help prevent the development of adverse health behaviors among young men. Governmental policies and strategies are required to enable intersectional collaboration and shared responsibility among the education, military and health sectors.

Project description:BackgroundSingle time-point assessments of psychological distress are often used to indicate chronic mental health problems, but the validity of this approach is unclear. The aims of this study were to investigate how a single assessment of distress relates to longer-term assessment and quantify misclassification from using single measures to indicate chronic distress.MethodsData came from the Household, Income and Labour Dynamics in Australia Survey, a nationally representative study of Australian adults. Psychological distress, measured with the Kessler10 and categorised into low (scores:10- < 12), mild (12- < 16), moderate (16- < 22) and high (22-50), has been assessed in the Survey biennially since wave 7. Among respondents who were aged ≥25 years and participated in all waves in which distress was measured, we describe agreement in distress categories, and using a mixed linear model adjusting for age and sex we estimate change in scores, over a two-, four-, six- and eight-year follow-up period. We applied weights, benchmarked to the Australian population, to all analyses.ResultsTwo-years following initial assessment, proportions within identical categories of distress were 66.0% for low, 54.5% for mild, 44.0% for moderate and 50.3% for high, while 94.1% of those with low distress initially had low/mild distress and 81.4% with high distress initially had moderate/high distress. These patterns did not change materially as follow-up time increased. Over the full eight-year period, 77.3% of individuals with high distress initially reported high distress on ≥1 follow-up occasion. Age-and sex- adjusted change in K10 scores over a two-year period was 1.1, 0.5, - 0.7 and - 4.9 for low, mild, moderate and high distress, respectively, and also did not change materially as follow-up time increased.ConclusionIn the absence of repeated measures, single assessments are useful proxies for chronic distress. Our estimates could be used in bias analyses to quantify the magnitude of the bias resulting from use of single assessments to indicate chronic distress.

Project description:BackgroundChildhood health significantly influences mental well-being in later life, but this relationship remains underexplored in China. This study aims to investigate the long-term associations between childhood health status and depressive symptoms in older adults, emphasizing the need for early interventions to promote lifelong mental health.MethodsWe utilized data from a nationally representative survey conducted across 28 provinces of China, comprising 15,581 adults aged 45 years and older. Depressive symptoms were assessed using the Center for Epidemiological Studies Depression scale. Logistic regression and decision tree models were applied to examine the association between childhood health and depressive symptoms in later life.ResultsThe overall prevalence of depressive symptoms among participants was 33.75%. Those reporting excellent or very good childhood health had a lower prevalence (29.99%), while those with poor childhood health reported a markedly higher rate (47.57%). After adjusting for demographics and lifestyle factors, poor childhood health was associated with a higher likelihood of depressive symptoms (AOR 1.83, 95% CI 1.56-2.14, P < 0.0001). Other risk factors included aged 65-74 years (AOR 1.23, 95% CI 1.10-1.38, P = 0.0009), female gender (AOR 2.01, 95% CI 1.78-2.28, P < 0.0001), and never drinking (AOR 1.24, 95% CI 1.11-1.38, P = 0.0012). Protective factors included tertiary education (AOR 0.45, 95% CI 0.32-0.65, P = 0.0030), higher BMI (BMI 24-27.9 kg/m2: AOR 0.72, 95% CI 0.60-0.87 kg/m2, P = 0.0033; BMI ≥ 28 kg/m2: AOR 0.69, 95% CI 0.56-0.85 kg/m2, P = 0.0030), sleep duration of seven or more hours (7-10 h sleep: AOR 0.52, 95% CI 0.48-0.57, P < 0.0001; ≥10 h sleep: AOR 0.50, 95% CI 0.43-0.58, P < 0.0001), never smoked (AOR 0.77, 95% CI 0.68-0.86, P < 0.0001), and urban residency (AOR 0.57, 95% CI 0.50-0.64, P < 0.0001). The decision tree model highlighted key factors associated with depressive symptoms, including childhood health, non-communicable diseases, sleep duration, residency, alcohol consumption, and smoking status.ConclusionsOur findings suggest that childhood health may influence mental well-being in later life. Promoting healthy behaviors from early childhood could help reduce depression risk in older age. However, the reliance on self-reported data and a cross-sectional design limit causal interpretation. Preventive care and targeted interventions for vulnerable children should be prioritized to improve long-term mental health outcomes.
