Factors Impacting Retention of Aged Care Workers: A Systematic Review.

ABSTRACT: Retention of care support workers in residential aged care facilities and home-based, domiciliary aged care is a global challenge, with rapid turnover, low job satisfaction, and poorly defined career pathways. A mixed-methods systematic review of the workforce literature was conducted to understand the factors that attract and retain care staff across the aged care workforce. The search yielded 49 studies. Three studies tested education and training interventions with the aim of boosting workforce retention and the remaining 46 studies explored opinions and experiences of care workers in 20 quantitative, four mixed-methods and 22 qualitative studies. A range of factors impacted retention of aged care staff. Two broad themes emerged from the analysis: individual and organisational factors facilitating retention. Individual factors related to personal satisfaction with the role, positive relationships with other staff, families, and residents, and a cooperative workplace culture. Organisational factors included opportunities for on-the-job training and career development, appropriate wages, policies to prevent workplace injuries, and job stability. Understaffing was often cited as a factor associated with turnover, together with heavy workloads, stress, and low job satisfaction. With global concerns about the safety and quality of aged care services, this study presents the data associated with best practice for retaining aged care workers.

Project description:BackgroundIt is estimated that over 930 million people live in fragile and conflict-affected situations (FCAS) worldwide. These regions, characterized by violence, civil unrest, and war, are often governed by corrupt administrations who are unwilling to provide their citizens with basic human rights. Individuals living in FCAS face health inequities; however, women are disproportionally affected and face additional barriers to accessing sexual and reproductive services, including antenatal care (ANC). This systematic review aims to identify the factors that impact ANC usage in the 37 countries or regions classified as FCAS in 2020 by The World Bank.MethodsUsing the PRISMA guidelines, a systematic search of five databases (SCOPUS, Web of Science, PubMed, EMBASE, and CINAHL) was conducted. Results were limited to human studies, written in English, and published between January 2002 and January 2022. Studies that identified factors affecting utilization of ANC or maternal health services were included for review and critically appraised using the National Institute of Health's Quality Assessment Tools. Findings were summarized using a narrative synthesis approach.ResultsThe database search yielded 26,527 studies. After title, abstract and full-text review, and exclusion of duplicate articles, 121 studies remained. Twenty-eight of the 37 FCAS were represented in the included studies. The studies highlighted that women in FCAS' are still not meeting the World Health Organization's 2002 recommendation of four ANC visits during pregnancy, a recommendation which has since been increased to eight visits. The most cited factors impacting ANC were socioeconomic status, education, and poor quality of ANC. Despite all studies being conducted in conflict-affected regions, only nine studies explicitly identified conflict as a direct barrier to accessing ANC.ConclusionThis review demonstrated that there is a paucity in the literature examining the direct and indirect impacts of conflict on ANC utilization. Specifically, research should be conducted in the nine FCAS that are not currently represented in the literature. To mitigate the barriers that prevent utilization of maternal health services identified in this review, policy makers, women utilizing ANC, and global organizations should attempt to collaborate to enact policy change at the local level.

Project description:BackgroundRegular engagement over time in hypertension care, or retention, is a crucial but understudied step in optimizing patient outcomes. This systematic review leverages a hermeneutic methodology to identify, evaluate, and quantify the effects of interventions and contextual factors for improving retention for patients with hypertension.MethodsWe searched for articles that were published between 2000 and 2022 from multiple electronic databases, including MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, clinicaltrials.gov, and WHO International Trials Registry. We followed the latest version of the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guideline to report the findings for this review. We also synthesized the findings using a hermeneutic methodology for systematic reviews, which used an iterative process to review, integrate, analyze, and interpret evidence.ResultsFrom 4686 screened titles and abstracts, 18 unique studies from 9 countries were identified, including 10 (56%) randomized controlled trials (RCTs), 3 (17%) cluster RCTs, and 5 (28%) non-RCT studies. The number of participants ranged from 76 to 1562. The overall mean age range was 41-67 years, and the proportion of female participants ranged from 0% to 100%. Most (n = 17, 94%) studies used non-physician personnel to implement the proposed interventions. Fourteen studies (78%) implemented multilevel combinations of interventions. Education and training, team-based care, consultation, and Short Message Service reminders were the most common interventions tested.ConclusionsThis review presents the most comprehensive findings on retention in hypertension care to date and fills the gaps in the literature, including the effectiveness of interventions, their components, and contextual factors. Project description:BackgroundUpstream factors have been found to affect COVID-19 vaccination rates and coverage globally. However, there are inadequate details within the African context. This systematic review aims to close this research gap by investigating upstream factors influencing COVID-19 vaccination rates in Africa.MethodsA literature search will be systematically conducted utilizing various databases including: MEDLINE, EMBASE, SCOPUS, CINAHL, Web of Science, and PsycINFO. Eligible studies will include peer-reviewed articles published in the English language from 2020-2023, conducted in Africa, focused on upstream factors, and include one barrier or facilitator to COVID-19 vaccination rates. Two reviewers will use a two-step screening process to examine every article's title, abstract, and full text. A third-party reviewer will resolve disagreements between both individual reviewers. This review will focus on extracting data from published studies to explain the upstream factors included and their impact on COVID-19 vaccination rates across Africa. Data and records will be managed using Covidence. Preferred Reporting Items for Systematic Reviews and Meta-Analyses [PRISMA] framework will be used as the basis for reporting. To reduce bias, the researchers will use the Mixed Methods Appraisal Tool to assess the studies chosen for review. Results will be compiled utilizing four tables to summarize articles and group determinants based on the Consolidated Framework for Implementation Research (CFIR).DiscussionUpstream factors have been cited as affecting population health, vaccination programs, and COVID-19, yet a large-scale systematic review has not been conducted to investigate these factors in relation to COVID-19 vaccination disparities faced in Africa. This review aims to analyze the root causes of African vaccination disparities by focusing on upstream factors. Understanding these factors is vital to help explain why these disparities occur and for designing effective interventions for future vaccinations. The results are expected to provide insights for researchers, policymakers, health systems, and individuals by identifying how resources and efforts can be better utilized to improve vaccination uptake and access.Trial registrationSystematic review registration: CRD42024501293.
