Project description:Children's sleep is essential for healthy development, yet over a third of children in the United States experience inadequate sleep. Environmental factors can influence sleep: greenspace exposure can promote better sleep, while heat exposure can disrupt sleep. As global climate change raises nighttime and daytime temperatures, greenspace may mitigate the negative effects of heat stress on sleep. We examined the direct effects of neighborhood greenspace and extreme heat exposure on sleep and the statistical interaction between greenspace and heat exposure on sleep outcomes among a nationally representative, four-year longitudinal sample of 8580 U.S. children ages 9-10 years at baseline. Hierarchical linear models incorporated a neighborhood greenspace measure: percent open park space within individual child census tracts, a measure of extreme neighborhood heat exposure during the summer months, and extensive individual and neighborhood-level covariates to test main and interaction effects on child sleep quality. Neighborhood open park space was related to better sleep quality, after controlling for covariates. Additionally, neighborhood extreme heat exposure was associated with worse sleep quality. A two-way interaction was found between neighborhood open park space and neighborhood heat exposure on sleep quality, suggesting open park space mitigated the negative effects of heat on sleep. The results indicate the potential contribution of open greenspace to improve child sleep and enhance resilience to extreme heat, which is an adverse outcome of climate change.

Project description:Both low family socioeconomic status (SES) and low neighborhood SES have been associated with higher levels of childhood obesity. However, little is known about how these two factors operate together. The purpose of this study was to determine if the association between neighborhood SES and obesity varies across household SES. We used the first-grade round of the Early Childhood Longitudinal Study, Kindergarten Class of 2011 (ECLS-K:2011). Household SES was defined based on income, education, and occupation. Neighborhood SES was defined by the percent of households living in poverty in the child's school district. Log-binomial regression models estimated the association between neighborhood SES and obesity and tested whether this association varied by household SES. We found the association between neighborhood SES and obesity varied significantly by household SES (p-interaction = 0.002). For children in the lowest tertile of neighborhood SES, prevalence of obesity was not statistically significantly different comparing children with low, middle or high household SES (Predicted probability (PP)lowest 0.20 (95% CI: 0.17, 0.23), PPmiddle 0.21 (95%CI: 0.18, 0.24), PPhighest 0.16 (95%CI: 0.12, 0.20)). Conversely, within the highest and the middle tertiles of neighborhood SES, children with high household SES have significantly lower prevalence of obesity compared to children with the lowest household SES (PP: 0.09 (95%CI: 0.07, 0.11) vs 0.19 (0.16, 0.21) and (PP: 0.07 (95%CI: 0.05, 0.09) vs 0.17 (0.13, 0.21) for highest vs lowest household SES in middle and high neighborhood SES, respectively). Hence, low-SES in either variable is enough to be associated with increased prevalence of obesity.

Project description:Recent evidence suggests that living in a neighborhood with a greater percentage of older adults is associated with better individual health, including lower depression, better self-rated health, and a decreased risk of overall mortality. However, much of the work to date suffers from four limitations. First, none of the U.S.-based studies examine the association at the national level. Second, no studies have examined three important hypothesized mechanisms - neighborhood socioeconomic status and neighborhood social and physical characteristics - which are significantly correlated with both neighborhood age structure and health. Third, no U.S. study has longitudinally examined cognitive health trajectories. We build on this literature by examining nine years of nationally-representative data from the Health and Retirement Study (2002-2010) on men and women aged 51 and over linked with Census data to examine the relationship between the percentage of adults 65 and older in a neighborhood and individual cognitive health trajectories. Our results indicate that living in a neighborhood with a greater percentage of older adults is related to better individual cognition at baseline but we did not find any significant association with cognitive decline. We also explored potential mediators including neighborhood socioeconomic status, perceived neighborhood cohesion and perceived neighborhood physical disorder. We did not find evidence that neighborhood socioeconomic status explains this relationship; however, there is suggestive evidence that perceived cohesion and disorder may explain some of the association between age structure and cognition. Although more work is needed to identify the precise mechanisms, this work may suggest a potential contextual target for public health interventions to prevent cognitive impairment.

Project description:BackgroundThe rapid diffusion of the internet has decreased consumer reliance on health care providers for health information and facilitated the patients' ability to be an agent in control of their own health. However, empirical evidence is limited regarding the effects of health-related internet use among older adults, which is complicated by the proliferation of online health and medical sources of questionable scientific accuracy.ObjectiveWe explore the effects of health-related internet use, education, and eHealth literacy on medical encounters and patient-reported outcomes. Patient-reported outcomes are categorized into two dimensions: (1) self-reported health problem and (2) affective distress (feeling worried and anxious) due to information obtained. We were particularly interested in whether education and eHealth literacy moderate the association between perceived strain in medical encounters and patient-reported outcomes.MethodsOur study sample consisted of online panel members who have used the internet as a resource for health information, randomly drawn from one of the largest probability-based online research panels. This paper specifically reports results obtained from older panel members (age≥60 years: n=194). First, we examined descriptive statistics and bivariate associations (Pearson correlations and independent samples t tests). We used hierarchical ordinary least squares regression analyses by running separate regressions for each patient-reported outcome. In model 1, we entered the main effects. In model 2, technology and medical encounter variables were included. Model 3 added the statistical interaction terms.ResultsAge (β=-.17; P=.02), gender (β=-.22; P=.01), and medical satisfaction (β=-.28; P=.01) were significant predictors of self-reported health problems. Affective distress was positively predicted by gender (β=.13; P=.05) and satisfaction with medical encounters (β=.34; P<.001) but negatively predicted by education (β=-.18; P=.03) and eHealth literacy (β=-.32; P=.01). The association between experiencing a health problem in relation to health-related internet use and perception of strained medical encounters was greater among respondents with lower levels of education (β=-.55; P=.04). There was also a significant interaction between education and eHealth literacy in predicting the level of affective distress (β=-.60; P=.05), which indicated that higher levels of education predicted lower averages of feeling anxiety and worry despite lower eHealth literacy. Older women reported higher averages of affective distress (β=.13; P=.05), while older men reported higher averages of experiencing a self-reported health problem (β=-.22; P=.01).ConclusionsThis study provides evidence for the effect of health-related internet use on patient-reported outcomes with implications for medical encounters. The results could be used to guide educational and eHealth literacy interventions for older individuals.

Project description:In 2015, we conducted a cross-sectional, Internet-based, U.S. nationally representative probability survey of 2,021 adults (975 men, 1,046 women) focused on a broad range of sexual behaviors. Individuals invited to participate were from the GfK KnowledgePanel®. The survey was titled the 2015 Sexual Exploration in America Study and survey completion took about 12 to 15 minutes. The survey was confidential and the researchers never had access to respondents' identifiers. Respondents reported on demographic items, lifetime and recent sexual behaviors, and the appeal of 50+ sexual behaviors. Most (>80%) reported lifetime masturbation, vaginal sex, and oral sex. Lifetime anal sex was reported by 43% of men (insertive) and 37% of women (receptive). Common lifetime sexual behaviors included wearing sexy lingerie/underwear (75% women, 26% men), sending/receiving digital nude/semi-nude photos (54% women, 65% men), reading erotic stories (57% of participants), public sex (≥43%), role-playing (≥22%), tying/being tied up (≥20%), spanking (≥30%), and watching sexually explicit videos/DVDs (60% women, 82% men). Having engaged in threesomes (10% women, 18% men) and playful whipping (≥13%) were less common. Lifetime group sex, sex parties, taking a sexuality class/workshop, and going to BDSM parties were uncommon (each <8%). More Americans identified behaviors as "appealing" than had engaged in them. Romantic/affectionate behaviors were among those most commonly identified as appealing for both men and women. The appeal of particular behaviors was associated with greater odds that the individual had ever engaged in the behavior. This study contributes to our understanding of more diverse adult sexual behaviors than has previously been captured in U.S. nationally representative probability surveys. Implications for sexuality educators, clinicians, and individuals in the general population are discussed.

Project description:Regular physical activity is a key modifiable non-pharmacological treatment to enhance sleep quality, a key predictor of optimal health and wellbeing. Most of the evidence on physical activity and sleep quality is based on studies assessing the effects of aerobic moderate-to-vigorous physical activity (e.g. brisk walking, cycling, jogging). Emerging clinical evidence suggests that muscle-strengthening exercise (e.g. push-ups, using weight machines) may also be beneficial for sleep quality. However, since no studies have examined the associations between muscle-strengthening exercise with sleep quality among a large sample of community-dwelling adults, it is currently unknown how these findings translate to the population level. This study describes the associations between the weekly frequency of muscle-strengthening exercise and sleep quality among a nationally representative sample of German adults. Data were drawn for the 2014 German Health Update Survey (n = 23,635; ≥18 years). Validated survey items assessed muscle-strengthening exercise (times/week) and sleep quality (assessed via 4 point-scale: 'very poor' to 'good'). Poisson regression with robust error variance was used to calculate prevalence ratios of (PR) across weekly muscle-strengthening exercise frequency (None [reference]; 1, 2, 3-4 and ≥ 5 times/week), adjusting for potential confounders (e.g. age, sex, socioeconomic status, self-rated health, smoking, alcohol, aerobic physical activity). Compared with those reporting none, any muscle-strengthening exercise was associated with a reduced prevalence of 'poor' (PR range: 0.77-0.83) and 'very poor' (PR range: 0.57-0.70) quality sleep. Future health behavior modification strategies to enhance sleep quality at the population-level should consider promoting muscle-strengthening exercise.

Project description:Despite increased awareness and concern about children with developmental disabilities wandering away from adult supervision, there is a paucity of research about elopement. This is the first study to examine and report the prevalence and correlates of elopement in a nationally representative sample of school-age children in the United States with an autism spectrum disorder (ASD) and/or cognitive impairment. Data were obtained from the CDC's "Pathways" Survey, a follow-up telephone survey of the parents of 4,032 children with a developmental condition. 3,518 children that had ASD, intellectual disability (ID), and/or developmental delay (DD) at the time of survey administration were included for analysis. Children were divided into three condition groups: ASD-only; ID/DD-only; ASD+ID/DD. Logistic regression analyses were used to compare the prevalence of elopement and rates of preventive measure use (barriers and/or electronic devices) across condition groups, and to examine the clinical and demographic correlates of elopement. T-tests were also performed to compare scores on the Children's Social Behavior Questionnaire (CSBQ) between wanderers and non-wanderers. Overall, 26.7% of children had reportedly eloped within the previous year, most commonly from public places. Children with ASD-only and ASD+ID/DD were more likely to have eloped than those with ID/DD-only. Across all groups, wanderers scored higher than non-wanderers on five out of six CSBQ subscales; they were more likely not to realize when there is danger, to have difficulty distinguishing between strangers and familiar people, to show sudden mood changes, to over-react to everything/everyone, to get angry quickly, to get lost easily, and to panic in new situations or if change occurs. Even after controlling for elopement history, parents of children in the ASD+ID/DD group were more likely than those in the other condition groups to report using physical or electronic measures to prevent wandering.

Project description:The association between obstructive sleep apnea (OSA) and hypertension by race/ethnicity has not been well characterized in a national sample. Adult participants in the 2007-2008 National Health and Nutrition Examination Survey were reviewed by self-report of sleep apnea diagnosis, snorting, gasping or stopping breathing during sleep, and snoring to derive whether OSA was probable (pOSA). Multivariable logistic regression determined whether pOSA predicted hypertension in the overall cohort, and by body mass index (BMI) group and ethno-racial strata. pOSA predicted hypertension in several groups: (1) Within BMI strata, there was a significant association among overweight individuals [odds ratio [OR], 1.82; 95% confidence interval [CI], 1.26-2.62); (2) In race/ethnicity subgroups, the association was significant among Hispanic/Latinos (OR, 1.69; 95% CI, 1.13-2.53) and whites (OR, 1.40; 95% CI, 1.07-1.84); (3) In models stratified by both race/ethnicity and BMI, pOSA predicted hypertension among overweight black/African Americans (OR, 4.74; 95% CI, 1.86-12.03), overweight whites (OR, 1.65; 95% CI, 1.06-2.57), and obese Hispanic/Latino participants (OR, 2.01; 95% CI, 1.16-3.49). A simple, self-report tool for OSA was strongly associated with hypertension, and may serve as a potential future opportunity for OSA diagnosis.

Project description:Emerging evidence has shown that self-perception of overweight/obese status is associated with unfavorable cardiometabolic outcomes, above and beyond actual body weight. Given the lack of research among Asian populations, we examined the association between weight perception and metabolic syndrome (MetS) and cardiometabolic risks among Koreans. Data from the 2010-2015 Korea National Health and Nutrition Examination Survey, including women (N = 12,181) and men (N = 9,448) aged 19-65 years, were analyzed. Weight status perception was measured by participants' self-evaluation of their body size ("very/slightly obese," "normal," and "very/slightly thin"). Overall, 23.2% of women and 28.7% of men had MetS. Our cross-sectional multilevel logistic analyses showed a significant positive association between self-perceived obesity (vs. perceived normal weight) and MetS, independent of BMI and sociodemographic/behavioral/medical conditions, with a stronger association detected among men (OR = 1.38, p < 0.05) than women (OR = 1.22, p < 0.05), confirmed by a statistically significant interaction. Additionally, perceived obesity was associated with high blood pressure (OR = 1.27, p < 0.05) and high triglycerides (OR = 1.38, p < 0.05) among men and low high-density lipoprotein cholesterol (OR = 1.15, p < 0.05) among women. While further prospective research is needed, our findings suggest that perception of being obese may be an unfavorable indicator of cardiometabolic health among Koreans regardless of actual body weight.

Project description:The purpose of this research was to examine age, sex, and racial differences in the prevalence of harsh physical punishment in childhood in a nationally representative sample of the United States. Data were from the National Epidemiologic Survey on Alcohol and Related Conditions (NESARC) collected in 2004 and 2005 (n=34,653). Logistic regression analyses were conducted to examine age, sex, and racial differences in the prevalence of harsh physical punishment. Results suggest that the prevalence of harsh physical punishment has been decreasing among more recently born age groups; however, there appear to be sex and racial differences in this trend over time. The magnitude of the decrease appears to be stronger for males than for females. By race, the decrease in harsh physical punishment over time is only apparent among Whites; Black participants demonstrate little change over time, and harsh physical punishment seems to be increasing over time among Hispanics. Prevention and intervention efforts that educate about the links of physical punishment to negative outcomes and alternative non-physical discipline strategies may be particularly useful in reducing the prevalence of harsh physical punishment over time.
