Influencing Factors on the Quality of Lymph Node Dissection for Stage IA Non-Small Cell Lung Cancer: A Retrospective Nationwide Cohort Study.

ABSTRACT: Lymphadenectomy is an essential part of complete surgical operation for non-small cell lung cancer (NSCLC). This retrospective, multicenter cohort study aimed to identify factors that influence the lymphadenectomy quality. Data were obtained from the Polish Lung Cancer Study Group Database. The primary endpoint was lobe-specific mediastinal lymph node dissection (L-SMLND). The study included 4271 patients who underwent VATS lobectomy for stage IA NSCLC, operated between 2007 and 2022. L-SMLND was performed in 1190 patients (27.9%). The remaining 3081 patients (72.1%) did not meet the L-SMLND criteria. Multivariate logistic regression analysis showed that patients with PET-CT (OR 3.238, 95% CI: 2.315 to 4.529; p < 0.001), with larger tumors (pT1a vs. pT1b vs. pT1c) (OR 1.292; 95% CI: 1.009 to 1.653; p = 0.042), and those operated on by experienced surgeons (OR 1.959, 95% CI: 1.432 to 2.679; p < 0.001) had a higher probability of undergoing L-SMLND. The quality of lymphadenectomy decreased over time (OR 0.647, 95% CI: 0.474 to 0.884; p = 0.006). An analysis of propensity-matched groups showed that more extensive lymph node dissection was not related to in-hospital mortality, complication rates, and hospitalization duration. Actions are needed to improve the quality of lymphadenectomy for NSCLC.

Project description:BackgroundCurrent guidelines recommend anatomical resection and mediastinal lymph node resection for stage I to IIIA pulmonary carcinoids (PCs). The role of wedge resection in stage IA PCs remains controversial, previous studies focused on typical carcinoids (TCs) while differentiating histological subtypes preoperatively is not easy. We aimed to study the effect of wedge resection and lymph node examination (LNE) in patients with stage IA PCs.MethodsPatients who underwent anatomical and wedge resection for stage T1N0M0 lung carcinoid tumors between 2004 and 2019 were identified from the Surveillance, Epidemiology, and End Results (SEER) database. Patients were also divided into a non-LNE group and an LNE group. Kaplan-Meier analysis and the log-rank test were used to calculate and compare overall survival (OS). Propensity score matching (PSM) and inverse probability of treatment weighting (IPTW) were used to balance the variables between groups. Univariate and multivariate Cox proportional hazard models were developed to determine prognostic factors.ResultsA total of 2,029 patients with bronchopulmonary carcinoid tumors were included in this study, 1,450 underwent lobectomy, 147 underwent segmentectomy and 432 underwent wedge resection. Initially, 5-year survival differed marginally between wedge and anatomical resection (91% vs. 95%, P=0.051), but lost significance after adjustment. LNE improved 5-year survival (95% vs. 89%, P=0.003), and this remained significant after adjustment. In multivariate cox analysis, LNE remained a significant variable while extent of resection was not. This result also remained consistent after adjustment. OS was comparable between wedge resection and anatomical resection when at least 1 lymph node was examined.ConclusionsFor early-stage PC, wedge resection was not inferior to anatomical resection in terms of OS, while LNE significantly increased the survival in both multivariate and matched studies. The relationship between surgical extent and survival in the unadjusted study may be attributed to the lower rate of LNE in wedge resection. Our findings support wedge resection with emphasis on LNE in early-stage PCs.

Project description:BackgroundWith the popularization of lung cancer screening, more early-stage lung cancers are being detected. This study aims to compare three types of N classifications, including location-based N classification (pathologic nodal classification [pN]), the number of lymph node stations (nS)-based N classification (nS classification), and the combined approach proposed by the International Association for the Study of Lung Cancer (IASLC) which incorporates both pN and nS classification to determine if the nS classification is more appropriate for early-stage lung cancer.MethodsWe retrospectively reviewed the clinical data of lung cancer patients treated at the Cancer Hospital, Chinese Academy of Medical Sciences between 2005 and 2018. Inclusion criteria was clinical stage IA lung adenocarcinoma patients who underwent resection during this period. Sub-analyses were performed for the three types of N classifications. The optimal cutoff values for nS classification were determined with X-tile software. Kaplan‒Meier and multivariate Cox analyses were performed to assess the prognostic significance of the different N classifications. The prediction performance among the three types of N classifications was compared using the concordance index (C-index) and decision curve analysis (DCA).ResultsOf the 669 patients evaluated, 534 had pathological stage N0 disease (79.8%), 82 had N1 disease (12.3%) and 53 had N2 disease (7.9%). Multivariate Cox analysis indicated that all three types of N classifications were independent prognostic factors for prognosis (all P < 0.001). However, the prognosis overlaps between pN (N1 and N2, P = 0.052) and IASLC-proposed N classification (N1b and N2a1 [P = 0.407], N2a1 and N2a2 [P = 0.364], and N2a2 and N2b [P = 0.779]), except for nS classification subgroups (nS0 and nS1 [P < 0.001] and nS1 and nS >1 [P = 0.006]). There was no significant difference in the C-index values between the three N classifications (P = 0.370). The DCA results demonstrated that the nS classification provided greater clinical utility.ConclusionThe nS classification might be a better choice for nodal classification in clinical stage IA lung adenocarcinoma.

Project description:IntroductionLung cancer is the primary cause of cancer-related deaths worldwide, with high rates of morbidity and mortality. The most effective treatment for early stage (I-II) non-small cell lung cancer (NSCLC) is surgical resection. However, the extent of mediastinal lymph nodes removal required and the impact of their removal remains controversial. This systematic review and meta-analysis aimed to evaluate the postoperative complications in patients with stage I-II NSCLC who received mediastinal lymph node dissection (MLND) or mediastinal lymph node sampling (MLNS).Methods and analysisAccording to the predefined inclusion criteria, we will conduct a comprehensive search for randomized controlled trials (RCTs) and observational studies examining the postoperative complications of MLND compared to MLNS in patients with stage I-II NSCLC. The search will be performed across multiple databases including PubMed, Embase, the Cochrane Library, CNKI, WanFang, Sinomed, VIP, Duxiu, and Web of Science from inception to February 2024. Additionally, relevant literature references will be retrieved and hand searching of pertinent journals will be conducted. Screening, data extraction, and quality assessment will be performed by two independent reviewers. Review Manager 5.4 will be applied in analyzing and synthesizing. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) will be used to assess the quality of evidence for the whole RCTs and used Newcastle-Ottawa scale to assess the methodologic quality of observational studies.Ethics and disseminationThis study did not include personal information. Ethical approval was not required for this study. This study is based on a secondary analysis of the literature, so ethical review approval is not required. The final report will be published in a peer-reviewed journal.ConclusionThis systematic review will contribute to compare the safety and survival benefits of these two surgical techniques for the treatment of early stage NSCLC, to further guide the selection of surgical approaches.Trial registrationThe protocol of the systematic review has been registered on Open Science Framework, with a registration number of DOI https://doi.org/10.17605/OSF.IO/N2Y5D.
