Implantable, Bioresorbable Radio Frequency Resonant Circuits for Magnetic Resonance Imaging.

ABSTRACT: Magnetic resonance imaging (MRI) is widely used in clinical care and medical research. The signal-to-noise ratio (SNR) in the measurement affects parameters that determine the diagnostic value of the image, such as the spatial resolution, contrast, and scan time. Surgically implanted radiofrequency coils can increase SNR of subsequent MRI studies of adjacent tissues. The resulting benefits in SNR are, however, balanced by significant risks associated with surgically removing these coils or with leaving them in place permanently. As an alternative, here the authors report classes of implantable inductor-capacitor circuits made entirely of bioresorbable organic and inorganic materials. Engineering choices for the designs of an inductor and a capacitor provide the ability to select the resonant frequency of the devices to meet MRI specifications (e.g., 200 MHz at 4.7 T MRI). Such devices enhance the SNR and improve the associated imaging capabilities. These simple, small bioelectronic systems function over clinically relevant time frames (up to 1 month) at physiological conditions and then disappear completely by natural mechanisms of bioresorption, thereby eliminating the need for surgical extraction. Imaging demonstrations in a nerve phantom and a human cadaver suggest that this technology has broad potential for post-surgical monitoring/evaluation of recovery processes.

Project description:BackgroundWe aimed to demonstrate the feasibility of generating high-resolution human brain magnetic resonance imaging (MRI) at 5 Tesla (T) using a quadrature birdcage transmit/48-channel receiver coil assembly.MethodsA quadrature birdcage transmit/48-channel receiver coil assembly was designed for human brain imaging at 5T. The radio frequency (RF) coil assembly was validated by electromagnetic (EM) simulations and phantom imaging experimental studies. The simulated B1+ field inside a human head phantom and inside a human head model generated by the birdcage coils driven in circularly polarized (CP) mode at 3T, 5T and 7T was compared. Signal-to-noise ratio (SNR) maps, the inverse g-factor maps for evaluation of parallel imaging performance, anatomic images, angiography images, vessel wall images and susceptibility weighted images (SWI) were acquired using the RF coil assembly at 5T and compared to those acquired using a 32-channel head coil on a 3T MRI scanner.ResultsFor the EM simulations, 5T MRI provided less RF inhomogeneity compared to that of 7T. In the phantom imaging study, the distributions of the measured B1+ field were consistent with the distributions of the simulated B1+ field. In the human brain imaging study, the average SNR value of the brain in the transversal plane at 5T was 1.6 times of that at 3T. The 48-channel head coil at 5T had higher parallel acceleration capability than the 32-channel head coil at 3T. The anatomic images at 5T also showed higher SNR than those at 3T. Improved delineation of the hippocampus, lenticulostriate arteries, and basilar arteries was observed at 5T compared to 3T. SWI with a higher resolution of 0.3 mm ×0.3 mm ×1.2 mm could be acquired at 5T, which enabled better visualization of small blood vessels compared to that at 3T.Conclusions5T MRI can provide significant SNR improvement compared to that of 3T with less RF inhomogeneity than that of 7T. The ability to obtain high quality in vivo human brain images at 5T using the quadrature birdcage transmit/48-channel receiver coil assembly has significant in clinical and scientific research applications.

Project description:ObjectivesCardiac implantable electronic device (CIED)-induced metal artefacts possibly significantly diminish the diagnostic value of magnetic resonance imaging (MRI), particularly cardiac MR (CMR). Right-sided generator implantation, wideband late-gadolinium enhancement (LGE) technique and raising the ipsilateral arm to the generator during CMR scanning may reduce the CIED-induced image artefacts. We assessed the impact of generator location and the arm-raised imaging position on the CIED-induced artefacts in CMR.MethodsWe included all clinically indicated CMRs performed on patients with normal cardiac anatomy and a permanent CIED with endocardial pacing leads between November 2011 and October 2019 in our institution (n = 171). We analysed cine and LGE sequences using the American Heart Association 17-segment model for the presence of artefacts.ResultsRight-sided generator implantation and arm-raised imaging associated with a significantly increased number of artefact-free segments. In patients with a right-sided pacemaker, the median percentage of artefact-free segments in short-axis balanced steady-state free precession LGE was 93.8% (IQR 9.4%, n = 53) compared with 78.1% (IQR 20.3%, n = 58) for left-sided pacemaker (p < 0.001). In patients with a left-sided implantable cardioverter-defibrillator, the median percentage of artefact-free segments reached 87.5% (IQR 6.3%, n = 9) using arm-raised imaging, which fell to 62.5% (IQR 34.4%, n = 9) using arm-down imaging in spoiled gradient echo short-axis cine (p = 0.02).ConclusionsArm-raised imaging represents a straightforward method to reduce CMR artefacts in patients with left-sided generators and can be used alongside other image quality improvement methods. Right-sided generator implantation could be considered in CIED patients requiring subsequent CMR imaging to ensure sufficient image quality.Key points• Cardiac implantable electronic device (CIED)-induced metal artefacts may significantly diminish the diagnostic value of an MRI, particularly in cardiac MRIs. • Raising the ipsilateral arm relative to the CIED generator is a cost-free, straightforward method to significantly reduce CIED-induced artefacts on cardiac MRIs in patients with a left-sided generator. • Right-sided generator implantation reduces artefacts compared with left-sided implantation and could be considered in CIED patients requiring subsequent cardiac MRIs to ensure adequate image quality in the future.
