Medial Meniscus Posterior Root Transtibial Pullout Repair With Progressively Tensioning Subcortical Fixation Button.

ABSTRACT: We describe a surgical technique to repair medial meniscus posterior root tears through a transtibial pullout repair with a subcortical button for tibial fixation. This technique allows progressive tensioning of the repaired root without losing tension both during suturing of the knots above the button and after the procedure, owing to the specific button configuration.

Project description:Meniscal root tears are recognized as an important pathology. Failure to recognize and to treat this pathology could lead to early-onset osteoarthritis, similar to a total meniscectomy. Surgical treatment is essential to restore meniscal function and to normalize compartment contact pressures, whenever there is joint overload and not severe cartilaginous damage. Still, the biological healing property of the medial meniscus is poor, and failure of the surgical procedure may occur. This Technical Note proposes a technical procedure for revision of medial meniscus posterior root reconstruction with a split-gracilis autograft tendon anatomical transtibial pullout technique.

Clinical evaluation of suture materials for transtibial pullout repair of medial meniscus posterior root tear.

Project description:BackgroundThere are no recommendations for specific suture materials in transtibial pullout repair of medial meniscus posterior root tears. This study aimed to evaluate the clinical outcomes of transtibial pullout repair of medial meniscus posterior root tears using ultrahigh-molecular-weight polyethylene sutures and suture tape.MethodsWe retrospectively reviewed the data of 36 patients (27 women and 9 men, mean age 64.1 years) who had undergone transtibial pullout repair of medial meniscus posterior root tears between November 2018 and December 2019. Two groups of 18 patients each received either two different cord-like sutures or suture tape. Clinical parameters were assessed preoperatively and on second-look arthroscopy (mean postoperative period 12 months). The meniscal healing status was assessed using a previously published scoring system (ranging from 0 to 10), and the incidence rate of suture cut-out was assessed on second-look arthroscopy.ResultsAll clinical scores significantly improved in both groups, with no significant between-group differences on second-look arthroscopy. The arthroscopic meniscal healing scores significantly differed between sutures (mean 6.7 points) and suture tape (mean 7.4 points; p = 0.044). No significant between-group difference in the suture cut-out rate was observed.ConclusionsThis study found no significant differences in the clinical outcomes between ultrahigh-molecular-weight polyethylene sutures and suture tape. Favorable clinical outcomes were obtained using both types of suture; however, the usefulness of suture tape appears to be limited.

Transtibial Pullout Repair of Lateral Meniscus Posterior Root Tear with Tissue Loss: A Case with Anterior Cruciate Ligament Injury and Medial Meniscus Tear.

Project description:Lateral meniscus (LM) posterior root tear (LMPRT) is mainly caused by trauma, especially trauma associated with anterior cruciate ligament (ACL) injuries. Although a transtibial pullout repair or a side-to-side repair is commonly performed for LMPRT, to the best of our knowledge, there is no clinical report of LMPRT with tissue loss using the pullout technique. Thus, the purpose of this report was to describe a clinical, radiographic, and arthroscopic outcome after pullout repair for a case of LMPRT with a large defect with a chronic ACL tear and complex medial meniscus (MM) tears. A 31-year-old man complained of knee pain and restricted range of motion after twisting his knee when he stepped on an iron pipe. The patient had a football-related injury to his right knee 14 years before presentation, and since then, the patient's knee has given out more than 10 times but was left unassessed. Magnetic resonance imaging showed LMPRT with tissue loss, ACL tears, and complex MM tears. Transtibial pullout repair of the LMPRT with ACL reconstruction and MM repairs were performed. Following the pullout repair of the LMPRT, an approximately 6 mm gap remained between the LM posterior root and root insertion. However, magnetic resonance imaging and second-look arthroscopy at 1 year postoperatively revealed meniscal healing, gap filling with some regeneration tissue, of the LM posterior root. Furthermore, the lateral meniscus extrusion in the coronal plane improved from 3.1 mm (preoperative) to 1.6 mm (1 year postoperatively). Transtibial pullout repair with the remaining gap could be a viable treatment option for LMPRT with tissue loss, combined with ACL reconstruction.

Medial Meniscus Posterior Root Repair Using a Transtibial Technique.

Project description:The meniscal roots are critical in maintaining the normal shock absorbing function of the meniscus. If a meniscal root tear is left untreated, meniscal extrusion can occur rendering the meniscus nonfunctional resulting in degenerative arthritis. Two main repair techniques are described: (1) suture anchors (direct fixation) and (2) sutures pulled through a tibial tunnel (indirect fixation). Meniscal root repair using a suture anchor technique is technically challenging requiring a posterior portal and a curved suture passing device that can be difficult to manipulate within the knee. We present a technique for posterior medial meniscus root repair using 3 sutures (1 leader, 2 cinch), standard arthroscopy portals, and transtibial fixation. Overall, this technique simplifies a challenging procedure and allows for familiarity and efficiency.

Posterior Meniscal Root Repair With Transtibial Double Tunnel Pullout Technique and Anchor Fixation.

Project description:Meniscus root tears are tears that occur within 1 cm of the meniscus root attachment to the tibia. If not repaired, root tears will lead to altered knee biomechanics and accelerated degenerative changes in the knee. In this surgical technique, we demonstrate repair of a meniscus root tear with an all-inside suture repair of the posterior meniscus that is then passed through 2 transtibial tunnels and then fixed with an anchor. After surgery, patients are non-weightbearing for 4 weeks and can return to activity by 6 months.

Association between postoperative meniscal extrusion and clinical outcomes of the pullout fixation technique for medial meniscus posterior root tear.

Project description:ObjectiveTo evaluate the correlation between meniscal extrusion in the coronal plane with clinical and radiological outcomes at a minimum of 1 year after transtibial pullout fixation for medial meniscus posterior root tear (MMPRT).MethodsData from 30 patients (five males, 25 females) with MMPRT who underwent arthroscopic root fixation by the transtibial pullout technique between 2011 and January 2015 were retrospectively analyzed. The mean patient age was 60.7 ± 8.3years. The median time between injury and surgery was 3.8 months (range, 0.5-15 months). Medial meniscus radial displacement was assessed on magnetic resonance imaging performed preoperatively and at 1 year after fixation. The patients were divided into two groups based on the presence on magnetic resonance imaging of decreased meniscal extrusion (group A) or increased meniscal extrusion (group B) at 1 year postoperatively compared with preoperatively. The two groups were compared regarding factors including age at surgery, sex, body mass index, time between injury and surgery, and the Lysholm score and Kellgren-Lawrence (K-L) grade preoperatively and at 1 year postoperatively.ResultsThere were seven knees in group A, and 23 in group B. Both groups had a significantly improved Lysholm score postoperatively compared with preoperatively (p < 0.001). Although the postoperative K-L grade was significantly worse than the preoperative K-L grade in group B (p < 0.001), the postoperative K-L grade did not worsen postoperatively in group A.ConclusionThe K-L grade did not worsen postoperatively in the group with decreased meniscal extrusion at 1 year after transtibial pullout fixation for MMPRT compared with preoperatively.

Medial Meniscus Posterior Root Tear Repair Using a 2-Simple-Suture Pullout Technique.

Project description:Medial meniscus posterior root tear is one of the underestimated knee injuries in terms of incidence. Despite its grave sequelae, using simple but effective technique can maintain the native knee joint longevity. In the current note, a 2-simple-suture pullout technique was used to effectively reduce the meniscus posterior root to its anatomic position. The success of the technique depended on proper tool selection as well as tibial tunnel direction that allowed easier root suturing and better suture tensioning, without inducing any iatrogenic articular cartilage injury or meniscal tissue loss. Using anterior knee arthroscopy portals, anterolateral as a viewing portal and anteromedial as a working portal, a 7-mm tibial tunnel starting at Gerdy tubercle and ending at the medial meniscus posterior root bed was created. The 2 simple sutures were retrieved through the tunnel and tensioned and secured over a 12-mm-diameter washer at the tibial tunnel outer orifice. Anatomic reduction of the medial meniscus posterior root tear was confirmed arthroscopically intraoperatively and radiologically by postoperative magnetic resonance imaging.

Plate Hooking Transtibial Medial Meniscus Posterior Root Pull-Out Repair with Medial Open-Wedge High Tibial Osteotomy for Knee Osteoarthritis with Medial Meniscus Posterior Root Tear.

Project description:Medial meniscus posterior root tears (MMPRT) are a risk factor for knee osteoarthritis (KOA) because it involves disruption of the hoop structure of the meniscus. In recent years, MMPRT pullout repair has been performed to restore the native anatomy of the meniscus attachment. In addition, medial open-wedge high tibial osteotomy (MOWHTO) has been performed with MMPRT pullout repair to reduce the pressure on the medial compartment. However, it has been noted that when MMPRT pullout repair and HTO are performed simultaneously in a patient, the locking screws of the plate interfere with the tibial bone tunnel, which damages the suture. In this technical note, we developed a method to reduce the probability of interference by placing the plate further anteroinferior when digging the bone tunnel from the PM position. This technical note aimed to provide a comprehensive description of the safety of transtibial MMPRT repair with MOWHTO for medial KOA with MMPRT.

Augmentation of the Pullout Repair of a Medial Meniscus Posterior Root Tear by Arthroscopic Centralization.

Project description:The meniscus roots are critical for meniscus function in preserving correct knee kinematics and avoiding meniscus extrusion and, consequently, in the progression of osteoarthritis. Several techniques exist for medial meniscus posterior root tear repair; however, current surgical techniques have been proved to fail to reduce meniscus extrusion, which has been shown to be associated with development of osteoarthritis, although significant improvements in the postoperative clinical findings have been achieved. This Technical Note describes an arthroscopic technique for the medial meniscus posterior root tear in which a pullout repair is augmented by a centralization technique to restore and maintain the medial meniscus function by efficiently reducing meniscus extrusion.

Distal Fixation of Meniscal Root Repair by Transtibial Pullout Technique via Transtibial Tubercle Fixation Without Hardware

Project description:The meniscal roots are critical in maintaining the normal shock-absorbing function of the meniscus. If a meniscal root tear is left untreated, meniscal extrusion can occur, rendering the meniscus nonfunctional, resulting in degenerative arthritis. Preservation of meniscal tissue with restoration of meniscal continuity is becoming the standard for meniscal root pathology. Not all patients are candidates for root repair; however, repair is indicated in active patients after acute or chronic injury with no significant osteoarthritis and malalignment. Two main repair techniques have been described: suture anchor (direct fixation) and transtibial pullout (indirect fixation). The most common root repair technique is a transtibial technique. In this technique, sutures are placed into the torn meniscal root and then shuttled down through the tunnel in the tibia to tie the repair distally. The option adopted in our technique is to fix the meniscal root distally by wrapping threads of FiberTape (Arthrex) around the tibial tubercle through a transverse tunnel posterior to the tibial tubercle with buried knots inside the transverse tunnel without the use of metal buttons or anchors. This technique provides secure tension for repair without loosening of knots and tension that occur when using metal buttons and avoiding irritation caused by metal buttons and knots in patients.
