Kinesiophobia in patients after cardiac surgery: a scoping review.

ABSTRACT:

Background

This paper reviews the scope of research on kinesiophobia in patients after cardiac surgery. Further, it reviews the current situation, evaluation tools, risk factors, adverse effects, and intervention methods of kinesiophobia to provide a reference for promoting early rehabilitation of patients after cardiac surgery.

Methods

Guided by the scoping methodology, the Web of Science, PubMed, CINAHL, Cochrane Library, China Biomedical Literature Database, VIP Database, Wanfang Database, CNKI, and other databases were searched from database inception until July 31, 2024. The studies obtained were screened, summarised and systematically analysed by two researchers.

Results

Eighteen studies (16 cross-sectional studies, one qualitative study, and one randomised controlled trial) were included. The incidence of kinesiophobia in patients after cardiac surgery was 39.20-82.57%, and the Tampa Scale for Kinesiophobia Heart (TSK-SV Heart) was used to evaluate this incidence. The influencing factors of kinesiophobia in patients after cardiac surgery included demographic characteristics, pain severity, frailty, exercise self-efficacy, disease-related factors, and psychosocial factors. Kinesiophobia led to adverse health outcomes such as reduced recovery, prolonged hospital stays, and decreased quality of life in patients after cardiac surgery, and there were few studies on intervention methods for postoperative kinesiophobia.

Conclusion

The kinesiophobia assessment tools suitable for patients after cardiac surgery should be improved, and intervention methods to promote the early recovery of patients after major clinical surgery and those with difficult and critical diseases should be actively researched.

Project description:BackgroundFamily-centered care (FCC) has been widely implemented in various medical settings, its application in cardiac surgery remains underexplored. Given the complexity of cardiac surgery and the significant impact on both patients and families, understanding FCC's role in this context is essential to improving patient outcomes and family well-being.ObjectivesTo identify the current state of the application of FCC in the families of cardiac surgery.MethodsThis study followed Arksey and O'Malley's methodology and the PRISMA extension for Scoping Reviews. A systematic search was conducted across ten databases between July and August 2024. Two researchers independently retrieved literature, extracted data, and cross-checked findings following predefined inclusion and exclusion criteria. The results were organized and synthesized using a structured spreadsheet.ResultsA total of 23 studies were included, 95.65% of which were quantitative, spanning nine countries. FCC interventions focused on collaboration, communication, negotiation, and support, primarily delivered by healthcare professionals. The main intervention formats were training sessions (100.00%), written materials (71.43%), and visual media (42.86%). FCC was linked to improved psychological well-being and greater satisfaction, with anxiety (52.38%) being the most commonly assessed outcome using validated scales. Qualitative findings highlighted the importance of FCC in patient recovery, emphasizing the need for systematic support to alleviate caregiving burden.ConclusionsThis scoping review highlights the core characteristics and interventions of FCC for cardiac surgery patients, emphasizing the need for a multidisciplinary approach. While FCC has shown benefits in improving emotional well-being and satisfaction, there is a lack of robust longitudinal studies and objective outcome measures. Future research should focus on interactive, personalized interventions, integrate technology, and include a broader range of outcome indicators to better assess FCC's clinical effectiveness.

Project description:IntroductionContinually rising life expectancy and a shift towards an ageing population are resulting in an increasing population of nonagenarians. By 2030, the global population of nonagenarians is expected to exceed 30 million. The incidence of symptomatic cardiac disease is reported to occur in 25% of those aged over 75 years. Therefore, the number of nonagenarians undergoing cardiac surgery is also expected to increase. A linear relationship between advanced age and surgical risk has previously been demonstrated; however, it is not yet known whether this knowledge extends to the perioperative course and mortality of nonagenarians undergoing cardiac surgery. This scoping review aims to review the literature, assess whether a deficiency exists in the published literature and potentially identify knowledge gaps to guide future efforts to improve the understanding of nonagenarians undergoing cardiac surgery.Methods and analysisFollowing the relevant aspects of the Preferred Reporting Items for Systematic Review and Meta-Analysis Extension for Scoping Review guidelines, electronic databases of MEDLINE, EMBASE and the Cochrane Library will be systematically searched, with additional reference tracking of eligible studies. Studies reporting the outcomes of nonagenarians undergoing open cardiac surgery or minimally invasive cardiac surgery requiring cardiopulmonary bypass will be included. Screening and data extraction will be performed by two reviewers independently. The data will be analysed and summarised descriptively with a narrative approach. Qualitative data that capture quality-of-life outcomes will be subjected to thematic analysis where feasible. Additionally, reporting results will highlight similarities and differences in nonagenarian selection for surgery.Ethics and disseminationEthics approval was not required. The findings will be disseminated through professional networks, conference presentations and publications in scientific journals.
