Effect of breast feeding on intelligence in children: prospective study, sibling pairs analysis, and meta-analysis.

ABSTRACT:

Objective

To assess the importance of maternal intelligence, and the effect of controlling for it and other important confounders, in the link between breast feeding and children's intelligence.

Design

Examination of the effect of breast feeding on cognitive ability and the impact of a range of potential confounders, in particular maternal IQ, within a national database. Additional analyses compared pairs of siblings from the sample who were and were not breast fed. The results are considered in the context of other studies that have also controlled for parental intelligence via meta-analysis.

Setting

1979 US national longitudinal survey of youth.

Subjects

Data on 5475 children, the offspring of 3161 mothers in the longitudinal survey.

Main outcome measure

IQ in children measured by Peabody individual achievement test.

Results

The mother's IQ was more highly predictive of breastfeeding status than were her race, education, age, poverty status, smoking, the home environment, or the child's birth weight or birth order. One standard deviation advantage in maternal IQ more than doubled the odds of breast feeding. Before adjustment, breast feeding was associated with an increase of around 4 points in mental ability. Adjustment for maternal intelligence accounted for most of this effect. When fully adjusted for a range of relevant confounders, the effect was small (0.52) and non-significant (95% confidence interval -0.19 to 1.23). The results of the sibling comparisons and meta-analysis corroborated these findings.

Conclusions

Breast feeding has little or no effect on intelligence in children. While breast feeding has many advantages for the child and mother, enhancement of the child's intelligence is unlikely to be among them.

Project description:IntroductionThe recommendations of most health organisations encourage mothers to keep exclusive breast feeding during the first 6 months and combining breast feeding with complementary feeding at least during the first and second years, due to the numerous immunologic, cognitive developmental and motor skill benefits that breast feeding confers. Although the influence of breast feeding on motor development during childhood has been studied, the findings are inconsistent, and some studies have even reported no effect. This manuscript presents a protocol for a systematic review and meta-analysis, with the aim of reviewing the relationship between breast feeding and motor skill development in children in terms of duration, exclusivity or non-exclusivity of breast feeding.Methods and analysisThe search will be conducted using Medline (via PubMed), EMBASE, Web of Science and Cochrane Library from inception to December 2019. Observational studies (cross-sectional and follow-up studies) written in English or Spanish that investigate the association between breast feeding and motor development in children will be included. This systematic review and meta-analysis protocol follows the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Protocols. The Critical Appraisal Checklist for Analytical Cross-Sectional Studies and The Newcastle-Ottawa Quality Assessment Scale for longitudinal studies will be used to assess the quality of included studies. The effect of breast feeding on motor skill development will be calculated as the primary outcome. Subgroup analyses will be carried out based on the characteristics of motor skill development and the population included.Ethics and disseminationEthical approval is not required because the data used will be obtained from published studies, and there will be no concerns about privacy. The findings from this study will be relevant information regarding the association of breast feeding with motor development in children and could be used encourage to improve breastfeeding rates. Project description:ObjectivesTo investigate the association between breast feeding and infant development during the first year of life using sibling comparison.DesignNationwide prospective birth cohort study with sibling pair analysis.Setting15 regional centres that participated in the Japan Environment and Children's Study.ParticipantsThis study included 77 119 children (singleton, term birth and no malformation/severe diseases) whose mothers were registered between January 2011 and March 2014, including 3521 duos or trios of siblings.Primary outcome measuresThe primary outcome was developmental delay at 6 and 12 months of age, assessed using the Japanese translation of the Ages and Stages Questionnaires, third edition. Multivariable regression analyses adjusted for confounders were performed to estimate the risk ratios of delay associated with any or exclusive breast feeding. Pairs of siblings discordant for statuses were selected, and conditional regression analyses were conducted with a matched cohort design.ResultsDevelopmental delay was identified in 6162 (8.4%) and 10 442 (14.6%) children at 6 and 12 months of age, respectively. Any breast feeding continued until 6 months or 12 months old was associated with reduced developmental delay at 12 months of age (adjusted risk ratio (95% CI): 0.81 (0.77 to 0.85) and 0.81 (0.78 to 0.84), respectively). Furthermore, exclusive breast feeding until 3 months was associated with reduced developmental delay at 12 months of age (adjusted risk ratio, 0.86 (95% CI 0.83 to 0.90)). In sibling pair analysis, the association between any breast feeding until 12 months and reduced developmental delay at 12 months of age persisted (adjusted risk ratio, 0.64 (95% CI 0.43 to 0.93)).ConclusionsThe present study demonstrated the association of continuous breast feeding with reduced developmental delay at 1 year of age using sibling pair analysis, in which unmeasured confounding factors are still present but less included. This may provide an argument to promote breastfeeding continuation.

Project description:Empirical sciences in general and psychological science in particular are plagued by replicability problems and biased published effect sizes. Although dissemination bias-related phenomena such as publication bias, time-lag bias, or visibility bias are well-known and have been intensively studied, another variant of effect distorting mechanisms, so-called decline effects, have not. Conceptually, decline effects are rooted in low initial (exploratory) study power due to strategic researcher behavior and can be expected to yield overproportional effect declines. Although decline effects have been documented in individual meta-analytic investigations, systematic evidence for decline effects in the psychological literature remains to date unavailable. Therefore, we present in this meta-meta-analysis a systematic investigation of the decline effect in intelligence research. In all, data from 22 meta-analyses comprising 36 meta-analytical and 1,391 primary effect sizes (N = 697,000+) that have been published in the journal Intelligence were included in our analyses. Two different analytic approaches showed consistent evidence for a higher prevalence of cross-temporal effect declines compared to effect increases, yielding a ratio of about 2:1. Moreover, effect declines were considerably stronger when referenced to the initial primary study within a meta-analysis, yielding about twice the magnitude of effect increases. Effect misestimations were more substantial when initial studies had smaller sample sizes and reported larger effects, thus indicating suboptimal initial study power as the main driver of effect misestimations in initial studies. Post hoc study power comparisons of initial versus subsequent studies were consistent with this interpretation, showing substantially lower initial study power of declining, than of increasing effects. Our findings add another facet to the ever accumulating evidence about non-trivial effect misestimations in the scientific literature. We therefore stress the necessity for more rigorous protocols when it comes to designing and conducting primary research as well as reporting findings in exploratory and replication studies. Increasing transparency in scientific processes such as data sharing, (exploratory) study preregistration, but also self- (or independent) replication preceding the publication of exploratory findings may be suitable approaches to strengthen the credibility of empirical research in general and psychological science in particular.
