A cluster of Legionella-associated pneumonia cases in a population of military recruits.

ABSTRACT: A Legionella cluster was identified through retrospective PCR analysis of 240 throat swab samples from X-ray-confirmed pneumonia cases. These were identified among young and otherwise healthy U.S. military recruits during population-based surveillance for pneumonia pathogens. Results were confirmed by sequence analysis. Cases clustered tightly, suggesting a local environmental etiology.

Project description:All recruits reporting to a regimental centre between 01 Jan 98 to 31 Mar 99 (n=1049), were administered a questionnaire about their own and their family's tobacco use practices. The prevalence of smoking among recruits was 43%, while that of smokeless tobacco use was 34.1%. There was a significant association between parental tobacco use and peer pressure on the tobacco habits of recruits. Knowledge about harmful effects of tobacco use did have a role in restraining the tobacco habit among recruits.

TOLLIP deficiency is associated with increased resistance to Legionella pneumophila pneumonia.

Project description:Legionella pneumophila (Lp) is a flagellated, intracellular bacterium that can cause Legionnaires' disease (LD). Lp activates multiple innate immune receptors, and TOLLIP dampens MyD88-dependent signaling and may influence susceptibility to LD. We evaluated the effect of TOLLIP on innate immunity, pneumonia severity, and LD susceptibility in mouse lungs and human populations. To accomplish this, we evaluated the effect of TOLLIP on lung-specific Lp control and immune response and associated a common functional TOLLIP variant with Lp-induced innate immune responses and LD susceptibility in humans. After aerosol Lp infection, Tollip-/- mice demonstrated significantly fewer bacterial colony-forming unit and increased cytokine responses from BAL fluid. Tollip-/- macrophages also suppressed intracellular Lp replication in a flagellin-independent manner. The presence of a previously characterized, functionally active SNP associated with decreased TOLLIP mRNA transcript in monocytes was associated with increased TNF and IL-6 secretion after Lp stimulation of PBMC ex vivo. This genotype was separately associated with decreased LD susceptibility (309 controls, 88 cases, p = 0.008, OR 0.36, 95% CI 0.16-0.76) in a candidate gene association study. These results suggest that TOLLIP decreases lung-specific TLR responses to increase LD susceptibility in human populations. Better understanding of TOLLIP may lead to novel immunomodulatory therapies.

STRESS FRACTURES OF THE FEMORAL SHAFT IN MILITARY RECRUITS.

Project description:Stress fractures amongst military recruits are limited to the lower extremities; yet involvement of the shaft of the femur is unusual. Seven such cases in a series of 352 stress fractures are presented. The importance of early recognition and management is emphasized with a view to prevent bony disruption in an otherwise easily treatable condition.

Trends in pre-military sexually transmitted infections and associated risk behaviours in Canadian Armed Forces recruits.

Project description:Background:Sexually transmitted infections (STIs) have historically been problematic for militaries. Recent reports indicating that rates of STIs among young male Canadian Armed Forces (CAF) members are higher than civilians prompted a need to better understand CAF members' reported rates of STIs and their behavioural risk factors for STIs. This study examined the prevalence of self-reported pre-military sexual behaviours (i.e. number of sexual partners and frequency of condom use) and history of a STI diagnosis among CAF recruits attending basic military training using data collected from the Recruit Health Questionnaire. Methods:Data came from 50,603 recruits who participated in the survey between 2003 and 2018 (84.9% male, 78.6% Non-Commissioned Member candidates, 64.9% aged between 17 and 24 years). Results:Among sexually active recruits, the proportions who had more than one sexual partner in the previous year increased from 30.5% in 2003 (95% CI, 27.8-33.4) to 35.5% in 2018 (95% CI, 34.0-37.0). Of recruits who were not in an exclusive relationship at the time, the proportions who reported always using a condom decreased from 50.8% in 2003 (95% CI, 46.4-55.1) to 40.2% in 2018 (95% CI, 38.3-42.2). Overall, 5.5% (95% CI, 5.3-5.7) reported ever having received a STI diagnosis. Demographic differences by age and sex were also found. Conclusion:These observations provide an indication of the baseline, pre-enlistment STI risk behaviours, and STI history among CAF recruits, and may provide insight into avenues for targeted interventions and health promotion programming, such as education and screening initiatives.

Association of a COL1A1 polymorphism with lumbar disc disease in young military recruits.

Project description:BackgroundLumbar disc disease (LDD), one of the most common conditions for which patients seek medical care, has been associated with sequence changes of the COL genes. COL1A1, however, has not been studied in young patients with LDD; COL1A1 polymorphisms have been associated with bone mineral density (BMD) in several populations and with LDD in older adults.ObjectiveTo study COL1A1 polymorphisms in young Greek army recruits with LDD.SubjectsThese young soldiers were diagnosed with early LDD at the time of their presentation to a military training site. All patients had radiological confirmation of their disease; a control group was also studied.MethodsSp1-binding site polymorphism of the COL1A1 gene was investigated by standard methods.ResultsThere was an increased frequency of the "ss" genotype (33.3%) in LDD patients; none of the controls had this genotype. In addition, a significantly smaller number of controls was heterozygotes for this allele.ConclusionsA previously studied sequence change of the regulatory region of the COL1A1 gene, the same as has previously been associated with low BMD in many populations and LDD in older adults, showed a strong association with LDD in young male soldiers who were recently diagnosed with this disease.

Elevated transmission of upper respiratory illness among new recruits in military barracks in Thailand.

Project description:BackgroundNew recruits within military barracks present conditions favorable for the spread of respiratory pathogens. However, respiratory pathogen transmission in such confined settings in the tropics has not been well studied.MethodsRecruits in four successive Royal Thai Army basic training classes living in military barracks were monitored for the symptoms of influenza-like illness (ILI) or upper respiratory illness (URI). Classes 1 and 2 were also monitored after basic training. Nasal/throat swabs from acute illnesses were collected and tested by influenza RT-PCR (all four classes). In addition, class 1 had multiplex PCR performed along with the analysis of bed locations within the barracks.ResultsInfluenza-like illness/upper respiratory illness rates ranged from 4·7 to 6·9 per 100 recruit-weeks in the four classes and generally decreased during the course of basic training (P < 0·05 in three of four classes). Rates during basic training were 1·7 (95% CI: 1·29, 2·29) and 2·5 (95% CI: 1·5, 4·1) times higher than after basic training (classes 1 and 2, respectively). In class 1, coronavirus, parainfluenza virus, and rhinovirus were the most commonly identified respiratory pathogens; only one influenza PCR-positive infection was detected in all four classes. Bed locations of URI/ILI cases in class 1 tended to be in closer proximity to each other.ConclusionBasic training recruits in military barracks in the tropics had high rates of acute respiratory illnesses with illness patterns consistent with external seeding followed by substantial internal transmission. Our findings may contribute to control measures in similar confined settings both within and outside the military.
