Measuring Japanese mothers' perception of child abuse: development of a Japanese version of the child abuse blame scale--physical abuse (CABS-PA-J).

ABSTRACT:

Background

The Child Abuse Blame Scale - Physical Abuse (CABS-PA) was translated into Japanese and its subscale items modified by the authors according to the Japanese cultural context. The aim of the current study was to investigate the appropriateness, reliability, and clinical applicability of the CABS-PA Japanese version (CABS-PA-J). Modifications were made to enable the determination of child abuse recognition in a Japanese cultural setting and early clinical intervention in child abuse cases.

Methods

The CABS-PA text was translated into Japanese, then back translated. The appropriateness of scale item translations was verified based on e-mail discussions with the original CABS-PA author. Exploratory and confirmatory factor analyses were performed to examine the validity of CABS-PA-J responses and to confirm the validity of factor structure. Criterion-related validity was also confirmed. The Japanese scale was used to examine the characteristic differences between mothers of premature infants (< 1500 g) and those of other infants (>or= 1500 g).

Results

Exploratory and confirmatory factor analyses found the factor structure to be similar between the original scale and the translated CABS-PA-J, suggesting adequate factor validity. There was a statistically significant correlation between social support from a spouse or third party and the abuse score on a subscale, partially demonstrating criterion-referenced validity. Similarities and differences were found in the stress reactions of the mothers of premature infants (< 1500 g) and those of other infants (>or= 1500 g).

Conclusion

CABS-PA-J was shown to be appropriate and reliable. It is an effective tool for determining the recognition of child abuse among Japanese mothers.

Project description:PURPOSE:Adverse childhood experiences in sensitive periods of the developing brain render the individual at a life-long risk for a broad spectrum of aberrant health outcomes. However, there is a lack of scales for the comprehensive assessment of adverse childhood experiences providing information of various types and the age of occurrence. Based on the complete, experimental version of the Maltreatment and abuse chronology of exposure (MACE-X) scale, the present study aimed to develop and psychometrically test a Norwegian version of MACE. METHODS:The 75-item MACE-X was translated from German to Norwegian and administered as a self-report measure to 90 outpatients and 145 employees at a Division of specialized mental health care in South-Eastern Norway. The outpatients also completed the Childhood trauma questionnaire (CTQ) and the Symptom checklist 90 (SCL-90) to investigate convergent and predictive validity. To investigate test-retest reliability, outpatients completed MACE once more two weeks later. RESULTS:Rasch analysis and Anderson likelihood ratio tests on the combined outpatient and employee data resulted in a 55 item version of the Norwegian MACE. In the outpatient group, test-retest reliability of the MACE-55 was excellent for total scores (ICC ? 0.94) and good to excellent for 10 subscale scores (ICC ? 0.82). Convergent validity with the CTQ was moderate to high for both total scores (0.63 ? r ? 0.86) and subscale scores (0.56 ? r ? 0.82). As compared to CTQ total scores, a MACE total score that combined severity and duration of exposure was numerically more strongly associated with overall psychiatric symptoms and each of nine symptom domains on the SCL-90. CONCLUSIONS:The newly developed Norwegian MACE comprehensively assesses past exposure to adverse childhood experiences with high psychometric properties. This scale is a useful tool for research questions addressing sensitive periods for childhood adversities and associated health phenotypes.

Project description:Child victims' reports of psychological and physical abuse by caregivers are a fundamental source of information beyond official records and caregiver reports. However, few or no sensitive and age-appropriate child-report instruments exist that have undergone in-depth validity and reliability testing across a broad age-range. Our study addresses this gap by examining psychometric properties of a picture-based, modularized version of the Parent-Child Conflict Tactics Scale (CTSPC-R), encompassing the maltreatment subtypes of psychological and physical abuse. A sample of 904 children and adolescents aged 4-16 years from the community (n = 568), child psychiatric services (n = 159), and from Child Protective Services (CPS; n = 177) completed the CTSPC-R. Measures to test convergent (maltreatment in parent interviews and CPS records) and concurrent validity (psychiatric symptoms) were collected. The CTSPC-R comprises 22 items, arranged in three severity modules by increasing level of psychological and physical abuse by caregivers. Companion picture cards were provided for children aged 4 and 8 years. The best fit to the data was attained with a second-order factor model, assuming three inter-correlated factors corresponding to the three severity modules, and a latent second-order factor representing combined physical and psychological abuse. The three factors showed good internal consistencies. Supporting convergent validity at the global and subtype-level of maltreatment, the CTSPC-R severity scale was associated with lifetime CPS-contact, presence of caregiver-reported emotional maltreatment and physical abuse, and dimensions of chronicity and severity. Discriminant validity was supported by non-significant correlations with caregiver-reported lack of supervision, failure to provide, and sexual abuse. Bolstering concurrent validity, moderate and severe physical abuse predicted caregiver-reported internalizing and externalizing symptoms. These effects were independent of child age, gender or community vs. non-community samples. Our study supports the CTSPC-R as a scientifically and clinically sound tool for ascertaining the child's own perspective on psychological and physical abuse from an early age onwards.
