Angioma of the chest wall: A report of 2 cases.

ABSTRACT: Hemangiomas are benign congenital tumors of endothelial origin that are typically seen in childhood and that undergo spontaneous involution with time. They are usually found on the face or less commonly on the trunk. Very few cases of chest-wall angiomas have been reported. This report describes chest-wall angiomas in two children (a 7-year-old boy and a 1-year-old girl) with emphasis on their sonographic features.

Tuberculous cold abscess of the chest wall: A clinical and surgical experience. Report of 16 cases(Case series).

Project description:Background:Tuberculosis is a public health problem in developing countries. Tuberculosis of the chest wall is rare and often presents as cold abscess (to differentiate from pyogenic abscess) or pseudotumoral mass whose diagnosis is difficult and often requires a surgical biopsy. Patients and methods:The medical series of 16 patients with cold chest wall abscess treated with surgery in association to anti-tubercular therapy were analysed retrospectively for the period of 7 years between January 2011 to December 2017 at Mohamed V Military Teaching Hospital - Rabat - Morocco. Results:The clinical examination provided a correct preoperative diagnosis of the abscess in all cases. Five patients had a past history of pulmonary tuberculosis and three patients had concomitant active infection. There were 6 cases on the left side, 9 cases on the right side and one case on the anterior chest wall. All patients underwent surgical drainage and debridement with specimens for bacteriology and histology. It was not necessary to resect ribs or sternum in all cases (sample costal or sternal curettage in one case each). Anti-tubercular treatment was routinely administered (6-9 months) with drug combinations of Isoniaside, Rifampicin, Pyrazinamide and Ethambutol. The evolution was favorable in all cases without complications or recurrences. Conclusion:Drainage of chest wall abscess and complete debridement provide adequate treatment. Post-operative anti-bacillary therapy should be combined with surgical procedures to minimize local complications and recurrence of infection.

Littoral Cell Angioma of Spleen: A rare case report.

Project description:IntroductionSpleen's Littoral Cell Angioma is a rare benign vascular tumor. The main clinical presentation is isolated splenomegaly.Case presentationwe present the case of a 37 years old male patient who suffers from chronic pain in the left hypochondrium.The abdominal examination finds a painful splenomegaly related to a biological bicytopenia. The CT Scan shows a 32 cm splenomegaly.An exploratory laparotomy with splenectomy was performed. The histological and immunohistochemical study confirmed The final diagnosis of Littoral Cell Angioma.Clinical discussionIsolated splenomegaly of unknown etiology is the main clinical sign. Splenectomy is required for a diagnostic purpose and substratum for histological study.ConclusionLittoral Cell Angioma of the Spleen is a rare benign vascular tumor, however it should be highlighted by clinical and radiological features, the definitive diagnosis is made upon histological study.

Introduction to chest wall reconstruction: anatomy and physiology of the chest and indications for chest wall reconstruction.

Project description:The chest wall functions as a protective cage around the vital organs of the body, and significant disruption of its structure can have dire respiratory and circulatory consequences. The past several decades have seen a marked improvement in the management and reconstruction of complex chest wall defects. Widespread acceptance of muscle and musculocutaneous flaps such as the latissimus dorsi, pectoralis major, serratus anterior, and rectus abdominis has led to a sharp decrease in infections and mortality. Successful reconstructions are dependent upon a detailed knowledge of the functional anatomy and blood supply of the chest and the underlying pathophysiology of a particular disease process. This article will provide an overview of key principles and evidence-based approaches to chest wall reconstruction.

Three-dimensional printed polyether-ether-ketone implant for extensive chest wall reconstruction: A case report.

Project description:Three-dimensional printed (3DP) implant offers a valid option with perfect anatomic fitting in individual and skeletal reconstruction of the chest wall. Herein, we present the case of a patient with a large chest wall tumor, where an extensive chest wall defect was repaired using 3DP polyether-ether-ketone (PEEK) implants. Surgical treatment planning was performed according to the computed tomography (CT) images in DICOM format. A 3DP implant was then design and fabricated. A wide excision of the chest wall tumor was performed, including the entire sternum, 2-6 costal cartilage and ribs, and parietal pleura. Furthermore, a skeletal reconstruction was carried out using a 3DP PEEK implant. The patient recovered well without surgical complications or tumor recurrence in the following year. In general, 3DP PEEK implant is an appropriate alternative for chest wall reconstruction. KEY POINTS: SIGNIFICANT FINDINGS OF THE STUDY: Skeletal reconstruction after wide excision of the chest wall remains a challenging problem for clinicians. WHAT THIS STUDY ADDS: 3DP PEEK implant is an appropriate alternative for chest wall reconstruction.

Management of congenital chest wall deformities.

Project description:Congenital chest wall deformities are considered to be anomalies in chest wall growth. These can be categorized as either rib cage overgrowth or deformities related to inadequate growth (aplasia or dysplasia). Rib cage overgrowth leads to depression of the sternum (pectus excavatum) or protuberance of the sternum (pectus carinatum) and accounts for greater than 90% of congenital chest wall deformities. The remaining deformities are a result of inadequate growth. Evolution in the management of congenital chest wall deformities has made significant progress over the past 25 years. This article will review chest wall deformities and the current management strategies of these interesting anomalies.

Mediastinal paraganglioma successfully resected by robot-assisted thoracoscopic surgery with en bloc chest wall resection: a case report.

Project description:BACKGROUND:Robot-assisted thoracoscopic surgery (RATS) is useful for surgery in the apical region of the chest cavity, as it narrows towards the head. Here, we describe a nonfunctional, rib-invasive paraganglioma arising in the posterior mediastinum that was successfully removed using RATS combined with chest wall resection. CASE PRESENTATION:A 31-year-old woman presented with a posterior mediastinal mass on chest computed tomography (CT) scan during a medical check-up 2?years prior. Positron emission tomography/computed tomography scan with F-18 fluorodeoxyglucose revealed a mass associated with standardized uptake maximum value of 2.69. With a preoperative diagnosis of neurogenic tumor by CT-guided percutaneous fine-needle aspiration biopsy, we performed robot-assisted tumor resection combined with chest wall resection. The wristed instruments of the robotic surgical system have increased range of motion and enabled the tumor resection without organ injury in the thoracic cavity. Histopathology examination revealed a non-functional paraganglioma with rib invasion. CONCLUSIONS:RATS is a useful technique, enabling safer and easier resection of a mediastinal tumor adjacent to surrounding organs.
