The "cello technique": a new technique for ultrasound-assisted calcaneoplasty.

ABSTRACT: The "cello technique" is a new calcaneoplasty technique for the treatment of Haglund disease. It is an ultrasound-assisted technique for resection of the posterosuperior part of the calcaneus. It is possible to resect the posterosuperior part of the calcaneus under direct ultrasound control with the patient in the prone position, with a dorsally positioned ultrasound probe, in line with the Achilles tendon fibers (sagittal line), and with the abrader in the posteromedial working portal. We describe in detail the technique for this new procedure in foot and ankle surgery. This innovative technique offers the possibility of expanding the indications for ultrasound-guided surgery in other fields of orthopaedic surgery.

Establishment of Transposase-assisted Low-input m6A Sequencing Technique

Project description:As the most abundant and best-characterized internal mRNA modification, N6-methyladenosine (m6A) emerges to play a critical regulatory role in wide range of physiological and pathological processes, including gametogenesis, neuronal development, obesity and tumorigenesis. Methylated RNA immunoprecipitation coupled with next-generation sequencing (MeRIP-seq) facilitates transcriptome-wide m6A profiling, also is the most widely used technique to understand the biological significance of m6A. However, it typically requires over 100 μg of total RNA or 107 cells as input materials, hampering its application in limited samples. Here, we develop tMeRIP-seq, a transposase assisted MeRIP-seq method to achieve m6A profiling using ultra-low amount of input RNA. By marrying Tn5 tagmentation to m6A-specific immunoprecipitation, tMeRIP-seq largely improves the efficiency of library construction and reduces the input materials to as little as 60 ng total RNA or 103 cells. We apply this method on a small droplet of human blood and recapitulate the m6A profile previously reported using conventional protocol. We find tMeRIP-seq is a convenient and powerful method to examine m6A in ultra-low input material, potentially providing m6A as a new layer of bio-marker for liquid biopsy.

Sonography-assisted arthroscopic resection of volar wrist Ganglia: a new technique.

Project description:Although satisfactory arthroscopic resection of volar wrist ganglia has been reported recently, the risk of damage to arteries, nerves, and tendons remains. Furthermore, ganglia and their stalks cannot be visualized arthroscopically in many cases, and surgeons must perform a blind resection of the joint capsule until ganglion cysts or their stalks appear. Sonography has limited resolution, but recent improvements in hardware and software have made it an excellent noninvasive and dynamic imaging technique for assessing the musculoskeletal system. Ganglia, tendons, nerves, and vessels around the lesion can be clearly observed by sonography. Furthermore, the cyclic motion of the arthroscopic shaver tip makes identification by sonography easy and assists in guiding the surgeon to the lesion.

Tailoring Mesoporous Titania Features by Ultrasound-Assisted Sol-Gel Technique: Effect of Surfactant/Titania Precursor Weight Ratio.

Project description:A mesoporous titania structure has been prepared using the ultrasound-assisted sol-gel technique in order to find out a way to tailor its structure. The TiO2 obtained was compared to the same version of titania but synthesized by a conventional sol-gel method with the objective of understanding the effect of ultrasound in the synthesis process. All synthesis experiments were focused on the preparation of a titania photocatalyst. Thus, the anatase photocatalytic active phase of titania was proven by X-ray diffraction. Additionally, the ultrasonation treatment proved to increase the crystallinity of titania samples, being one of the requirements to having good photocatalytic activity for titania. The influence of surfactant/titania precursor weight ratio on the structural (XRD), textural (N2-sorption measurements), morphological (TEM), surface chemistry (FTIR) and optical properties (UVDR) was investigated. It was observed that the crystallite size, specific surface area, band gap energy and even photocatalytic activity was affected by the synergism occurring between cavitation effect and the surfactant/titania precursor weight ratio. The study yielded interesting great results that could be considered for further application of ultrasound to tailor mesoporous titania features via sol-gel soft template synthesis, against conventional sol-gel process.

Project description:Ultrasound is ideally suited for the assessment of complex anatomy and pathologies of the thumb. Focused and dynamic thumb ultrasound can provide a rapid real-time diagnosis and can be used for guided treatment in certain clinical situations. We present a simplified approach to scanning technique for thumb-related pathologies and illustrate a spectrum of common and uncommon pathologies encountered.

Ultrasound-assisted hip arthroscopy.

Project description:We describe the use of intraoperative ultrasound for the safe development of arthroscopic portals during hip arthroscopy without the requirement for fluoroscopy. We find this technique consistently accurate, allowing the safe introduction of arthroscopic instruments into the hip with a very low rate of iatrogenic injury. We have further developed the technique for application to both central- and peripheral-compartment procedures. We now have a total experience of more than 700 procedures to date. With the described technique of ultrasound guidance for portal placement, fluoroscopy is required in fewer than 2% of hip arthroscopy procedures at our institution.

Local Phase Velocity Based Imaging: A New Technique Used for Ultrasound Shear Wave Elastography.

Project description:Ultrasound shear wave elastography is an imaging modality for noninvasive evaluation of tissue mechanical properties. However, many current techniques overestimate lesions dimension or shape especially when small inclusions are taken into account. In this paper, we propose a new method called local phase velocity-based imaging (LPVI) as an alternative technique to measure tissue elasticity. Two separate acquisitions with ultrasound push beams focused once on the left side and once on the right side of the inclusion were generated. A local shear wave velocity is then recovered in the frequency domain (for a single frequency or frequency band) for both acquired data sets. Finally, a two-dimensional shear wave velocity map is reconstructed by combining maps from two separate acquisitions. Robust and accurate shear wave velocity maps are reconstructed using the proposed LPVI method in calibrated liver fibrosis tissue mimicking homogeneous phantoms, a calibrated elastography phantom with stepped cylinder inclusions and a homemade gelatin phantom with ex vivo porcine liver inclusion. Results are compared with an existing phase velocity-based imaging approach and a group velocity-based method considered as the state of the art. Results from the phantom study showed that increased frequency improved the shape of the reconstructed inclusions and contrast-to-noise ratio between the target and background.

New 'ZISIS' (Zigzag Incision-Suction-Irrigation System) Technique for Endoscopic-Assisted Forehead Tumour Excision.

Project description:Background:Endoscopic-assisted excision of forehead tumours like osteomas and lipomas is well established, but the conventional techniques suffer from many limitations like inadequate access, fogging of the endoscope and unclear vision due to collection of blood and debris. Method:Three simple modifications of the conventional endoscopic forehead technique for benign tumour excisions are described by the acronym 'ZISIS'. ZI Zigzag scalp incision increases the surface area of the opening permitting easy insertion of multiple instruments along with the endoscope.S Suction tubing made from a disposable suction catheter tube is taped along the endoscope sheath for continuous suctioning and good vision.IS Irrigation system of warm saline made with an infant feeding tube is also taped along the endoscope just opening in the front of the suction tube. Results:A total of 12 consecutive patients underwent endoscopic excision of forehead benign tumours in 2 years. This included 2 lipomas and 10 osteomas cases. All patients achieved excellent hidden scars in the scalp. All patients rated their results as excellent with respect to the hidden scar and aesthetic result. No early or late complications were reported with follow-up ranging from 6 months to 2 years. Conclusion:A new simple modification called 'ZISIS' endoscopic forehead excision technique is described for benign forehead tumours excision making it easier, efficient and ergonomic. Level of Evidence IV:Evidence obtained from multiple time series with or without the intervention, such as case studies.

Piriformis Injection: An Ultrasound-Guided Technique.

Project description:Piriformis syndrome is an elusive condition that likely is overlooked and overdiagnosed in equal proportions. Image-guided injection of the muscle is an essential element in the assessment and management of this disorder. Ultrasonography allows visualization of the anatomy of the piriformis and its relation with the sciatic nerve. Ultrasound-guided injection of the muscle can be performed in an effective, reproducible fashion and, with careful attention to the details of the procedure, can be performed with minimal morbidity to the patient. This method has the advantage of being exceptionally well tolerated by the patient and can be performed in the office with minimal patient inconvenience and the opportunity for a real-time assessment of the patient's response to the injection. Our ultrasound-guided technique of piriformis muscle injection is detailed in this article.

New diagnostic technique to evaluate hepatic steatosis using the attenuation coefficient on ultrasound B mode.

Project description:PurposeWe have developed a diagnostic technique to evaluate hepatic steatosis using the attenuation coefficient (ATT) in ultrasound B mode imaging. A controlled attenuation parameter (CAP) by vibration-controlled transient elastography (VCTE) has also been used to evaluate hepatic steatosis. As that method uses ultrasound A mode, visualizing the liver in real time is difficult. We designed this clinical study to evaluate the diagnostic advantage of our technique using ATT compared to CAP.Materials and methodsThe study group included 94 patients with chronic liver disease who had undergone both ATT and CAP assessment at the time of liver biopsy. The M-probe and XL-probe were used for CAP measurement. Data for ATT and CAP were compared as a function of the steatosis grade.ResultsThe area under the receiver operating characteristic curve (AUC-ROCs) for ATT and PAC as a function of the steatosis grade were as follows: grade 1, 0.74 and 0.81; grade 2, 0.80 and 0.85; and grade 3, 0.96 and 0.98, respectively.ConclusionThe accuracy of steatosis grade diagnosis using ATT was the same as that using CAP, with no significant differences and with the added advantage of B mode ultrasound being more convenient and rapid, compared to A mode ultrasound, particularly for patients with subcutaneous fat thickness ?2 cm.

Arthroscopy-Assisted Fabella Excision: Surgical Technique.

Project description:The fabella is an anatomic variant not seen in all individuals and can potentially be a source of chronic knee pain due to chondromalacia, osteoarthritis, fractures, or biomechanical pressure against the lateral femoral condyle. It is situated intra-articular, close to the lateral femoral condyle, the lateral gastrocnemius head tendon, and the fabellofibular ligament. Given its rarity, the diagnosis of a symptomatic fabella is often overlooked when evaluating patients with persistent posterolateral knee pain. However, this diagnosis should always be considered, especially in high-performance runners, bikers, and triathletes. Although nonoperative management can potentially resolve symptoms associated with this condition, fabella excision via arthroscopically assisted surgery is a reliable and safe alternative to treat patients who do not benefit from nonsurgical treatment. We present our technique detailing fabella excision for treatment of posterolateral knee pain, which includes an arthroscopic evaluation of the fabella to assess damage to the femoral condyle and minimize over-resection and potential damage to surrounding structures.
