Race, ideology, and the tea party: a longitudinal study.

ABSTRACT: The Tea Party movement, which rose to prominence in the United States after the election of President Barack Obama, provides an ideal context in which to examine the roles of racial concerns and ideology in politics. A three-wave longitudinal study tracked changes in White Americans' self-identification with the Tea Party, racial concerns (prejudice and racial identification), and ideologies (libertarianism and social conservatism) over nine months. Latent Growth Modeling (LGM) was used to evaluate potential causal relationships between Tea Party identification and these factors. Across time points, racial prejudice was indirectly associated with movement identification through Whites' assertions of national decline. Although initial levels of White identity did not predict change in Tea Party identification, initial levels of Tea Party identification predicted increases in White identity over the study period. Across the three assessments, support for the Tea Party fell among libertarians, but rose among social conservatives. Results are discussed in terms of legitimation theories of prejudice, the "racializing" power of political judgments, and the ideological dynamics of the Tea Party.

Project description:ImportanceThe association between incarceration and long-term mortality risk is unknown and may contribute to racial disparities in overall life expectancy.ObjectiveTo determine whether incarceration in the US is associated with an increase in mortality risk and whether this association is different for Black compared with non-Black populations.Design, setting, and participantsThis generational retrospective cohort study used data from the National Longitudinal Survey of Youth 1979, a nationally representative cohort of noninstitutionalized youths aged 15 to 22 years, from January 1 to December 31, 1979, with follow-up through December 31, 2018. A total of 7974 non-Hispanic Black and non-Hispanic non-Black participants were included. Statistical analysis was performed from October 26, 2019, to August 31, 2021.ExposuresTime-varying exposure of having experienced incarceration during follow-up.Main outcomes and measuresThe main outcome was time to death. Cox proportional hazards regression models were used to estimate hazard ratios (HRs) and 95% CIs, adjusted for baseline sociodemographic, economic, and behavioral risk factors. Models were evaluated for the full cohort and stratified by race.ResultsOf the 7974 individuals included in our sample, 4023 (50.5%) were male, and 2992 (37.5%) identified as Black (median age, 18 [IQR, 17-20] years). During a median follow-up of 35 years (IQR, 33-37 years), 478 participants were incarcerated and 818 died. Unadjusted exposure to at least 1 incarceration between 22 and 50 years of age was 11.5% (95% CI, 10.4%-12.7%) for Black participants compared with 2.5% (95% CI, 2.1%-2.9%) for non-Black participants. In the multivariable Cox proportional hazards model with the full cohort, time-varying exposure to incarceration was associated with an increased mortality rate (adjusted HR [aHR], 1.35; 95% CI, 0.97-1.88), a result that was not statistically significant. In the models stratified by race, incarceration was significantly associated with increased mortality among Black participants (aHR, 1.65; 95% CI, 1.18-2.31) but not among non-Black participants (aHR, 1.17; 95% CI, 0.68-2.03).Conclusions and relevanceIn this cohort study with 4 decades of follow-up, incarceration was associated with a higher mortality rate among Black participants but not among non-Black participants. These findings suggest that incarceration, which was prevalent and unevenly distributed, may have contributed to the lower life expectancy of the non-Hispanic Black population in the US.

Project description:IntroductionTo enter a profession is to take on a new identity. Professional identity formation can be difficult, with medical learners struggling to adopt professional norms. The role of ideology in medical socialisation may offer insight into these tensions experienced by medical learners. Ideology is the system of ideas and representations that dominates the minds of individuals or social groups and calls individuals into certain ways of being and acting in the world. In this study, we use the concept of ideology to explore residents' experiences with identity struggle during residency.MethodsWe conducted a qualitative exploration of residents in three specialties at three academic institutions in the United States. Participants engaged in a 1.5-hour session involving a rich picture drawing and one-on-one interview. Interview transcripts were coded and analysed iteratively, with developing themes compared concurrently to newly collected data. We met regularly to develop a theoretical framework to explain findings.ResultsWe identified three ways that ideology contributed to residents' identity struggle. First was the intensity of work and perceived expectations of perfectionism. Second were tensions between the developing professional identity and pre-existing personal identities. Many residents perceived messages regarding the subjugation of personal identities, including the feeling that being more than physicians was impossible. Third were instances where the imagined professional identity clashed with the reality of medical practice. Many residents described how their ideals misaligned with normative professional ideals, constraining their ability to align their practice and ideals.ConclusionThis study uncovers an ideology that shapes residents' developing professional identity-an ideology that creates struggle as it calls them in impossible, competing or even contradictory ways. As we uncover the hidden ideology of medicine, learners, educators and institutions can play a meaningful role in supporting identity development in medical learners through dismantling and rebuilding its damaging elements.
