Project description:The integration of mobile health technologies in medical practice has the potential to promote in-person, high-quality care. We examine the impact of Voalte, a healthcare-specific mobile application, on bedside rounding and care coordination. A cross-sectional survey was conducted on 71 medical ward-based nurses from a quaternary-care academic centre, capturing 183 rounding events. The frequency of physician-nurse overlap at the bedside was 50.3%, representing a >20% increase when compared with the 2018 baseline before Voalte's introduction. Our results show that mobile health technologies can strengthen inpatient medicine workflows and interdisciplinary collaboration when implemented successfully.

Project description:ObjectiveTo quantify how stress related to use of health information technology (HIT) predicts burnout among physicians.MethodsAll 4197 practicing physicians in Rhode Island were surveyed in 2017 on their HIT use. Our main outcome was self-reported burnout. The presence of HIT-related stress was defined by report of at least 1 of the following: poor/marginal time for documentation, moderately high/excessive time spent on the electronic health record (EHR) at home, and agreement that using an EHR adds to daily frustration. We used logistic regression to assess the association between each HIT-related stress measure and burnout, adjusting for respondent demographics, practice characteristics, and the other stress measures.ResultsOf the 1792 physician respondents (43% response rate), 26% reported burnout. Among EHR users (91%), 70% reported HIT-related stress, with the highest prevalence in primary care-oriented specialties. After adjustment, physicians reporting poor/marginal time for documentation had 2.8 times the odds of burnout (95% CI: 2.0-4.1; P < .0001), compared to those reporting sufficient time. Physicians reporting moderately high/excessive time on EHRs at home had 1.9 times the odds of burnout (95% CI: 1.4-2.8; P < .0001), compared to those with minimal/no EHR use at home. Those who agreed that EHRs add to their daily frustration had 2.4 times the odds of burnout (95% CI: 1.6-3.7; P < .0001), compared to those who disagreed.ConclusionHIT-related stress is measurable, common (about 70% among respondents), specialty-related, and independently predictive of burnout symptoms. Identifying HIT-specific factors associated with burnout may guide healthcare organizations seeking to measure and remediate burnout among their physicians and staff.

Project description:BackgroundAlthough personal health devices (for example, smartwatches, fitness trackers and intelligent bracelets) offer great potential to monitor personal fitness and health parameters, many users discontinue using them after a few months. Thus, it is critical to study the postadoption behaviors of current users to enhance their engagement with personal health devices and use behaviors. However, there is little empirical research on the factors affecting users' engagement in beneficial use behaviors. Mindfulness and identity are not new topics, but the applications of these concepts in the field of information systems are emerging themes. Information technology (IT) mindfulness has been conceptualized in previous studies; however, little is known about the antecedents and consequences of IT mindfulness in the mobile health (mHealth) context.ObjectiveThe main aim of this study is to explore both IT identity and IT mindfulness to develop a new ground for research in the domain of mHealth postadoption. Thus, we aim to explain why users should be fully mindful of their engagement with PHDs and what could be the consequences and implications.MethodsThis study proposes that IT mindfulness can play an important role in improving the use behaviors of users. Through a web-based survey with 450 current users of a personal health device, this paper tests the relationship between IT identity and IT mindfulness in the postadoption stage of using personal health devices.ResultsWe found that IT identity significantly shapes IT mindfulness associated with PHDs. Moreover, the IT identity-IT mindfulness relationship is negatively moderated by individuals' perceived health status (P=.003). Finally, the results of this study show that IT mindfulness can significantly predict automatic use behaviors (eg, continued intention to use), active use behaviors (eg, feature use and enhanced use behaviors), and commitment behaviors in using personal health devices (eg, positive word-of-mouth intention).ConclusionsThe findings of this study provide implications for both research and practice. This study can contribute to our current understanding of IT mindfulness by developing and empirically testing a research model that explains the determinants and outcomes of the IT mindfulness construct in the context of personal health devices. The results imply that IT mindfulness significantly helps individuals express their alertness, awareness, openness, and orientation in the present in their postadoption interactions with smart devices used for health care purposes. Finally, our findings may assist practitioners and IT developers in designing mindfulness-supporting PHDs. Owing to the impact of IT mindfulness on postadoption behaviors, its 4 dimensions could be used for developing PHD technologies. Moreover, PHD developers may need to direct their efforts toward increasing IT mindfulness by reinforcing IT identity to serve and retain a wide range of target users.

Project description:ObjectivesThe research investigated the extent to which students, residents, and faculty members in Canadian medical faculties use mobile devices, such as smartphones (e.g., iPhone, Android, Blackberry) and tablet computers (e.g., iPad), to answer clinical questions and find medical information. The results of this study will inform how health libraries can effectively support mobile technology and collections.MethodsAn electronic survey was distributed by medical librarians at four Canadian universities to medical students, residents, and faculty members via departmental email discussion lists, personal contacts, and relevant websites. It investigated the types of information sought, facilitators to mobile device use in medical information seeking, barriers to access, support needs, familiarity with institutionally licensed resources, and most frequently used resources.ResultsThe survey of 1,210 respondents indicated widespread use of smartphones and tablets in clinical settings in 4 Canadian universities. Third- and fourth-year undergraduate students (i.e., those in their clinical clerkships) and medical residents, compared to other graduate students and faculty, used their mobile devices more often, used them for a broader range of activities, and purchased more resources for their devices.ConclusionsTechnological and intellectual barriers do not seem to prevent medical trainees and faculty from regularly using mobile devices for their medical information searches; however, barriers to access and lack of awareness might keep them from using reliable, library-licensed resources.ImplicationsLibraries should focus on providing access to a smaller number of highly used mobile resources instead of a huge collection until library-licensed mobile resources have streamlined authentication processes.

Project description:BackgroundWeight management mobile applications have become increasingly popular as a tool for individuals to achieve their weight loss goals. However, key challenges with mobile weight management applications are sustaining user engagement and promoting long-term behavior change. This study examines the key functions and features of weight management mobile applications from the viewpoint of information technology (IT) experts. We have developed a framework that outlines the optimal design strategies for mobile apps to support sustainable weight management.MethodsWe conducted face-to-face interviews with five experts to evaluate and give input on the proposed framework.ResultsEight specialized functionalities have been categorized and defined based on initial framework system features and recommended features based on experts' feedback: (I) customization; (II) usability; (III) integration; (IV) online support; (V) tracking; (VI) nutrition database; (VII) gamification; and (VIII) notification. In addition, common functionalities were identified as (I) registration/sign in; (II) data privacy; (III) security; and (IV) user feedback. An online discussion indicates the system functionality in four themes: (I) configuration; (II) data analytics; (III) support/performance; and (IV) platform: iOS, Android, Native.ConclusionsThese functionalities and features will be incorporated into a framework to design mobile applications for sustainable weight management. Also, to be part of user interface development for the user experience experiments.

Project description:Complications following surgery are common and frequently occur the following discharge. Mobile and wearable digital health interventions (DHI) provide an opportunity to monitor and support patients during their postoperative recovery. Lack of high-quality evidence is often cited as a barrier to DHI implementation. This review captures and appraises the current use, evidence base and reporting quality of mobile and wearable DHI following surgery. Keyword searches were performed within Embase, Cochrane Library, Web of Science and WHO Global Index Medicus databases, together with clinical trial registries and Google scholar. Studies involving patients undergoing any surgery requiring skin incision where postoperative outcomes were measured using a DHI following hospital discharge were included, with DHI defined as mobile and wireless technologies for health to improve health system efficiency and health outcomes. Methodological reporting quality was determined using the validated mobile health evidence reporting and assessment (mERA) guidelines. Bias was assessed using the Cochrane Collaboration tool for randomised studies or MINORS depending on study type. Overall, 6969 articles were screened, with 44 articles included. The majority (n = 34) described small prospective study designs, with a high risk of bias demonstrated. Reporting standards were suboptimal across all domains, particularly in relation to data security, prior patient engagement and cost analysis. Despite the potential of DHI to improve postoperative patient care, current progress is severely restricted by limitations in methodological reporting. There is an urgent need to improve reporting for DHI following surgery to identify patient benefit, promote reproducibility and encourage sustainability.

Project description:BackgroundA pneumonia outbreak of unknown etiology took place in Wuhan, Hubei province, China and spread quickly worldwide in December 2019. Chinese Center for Disease Control and Prevention identified a novel beta-coronavirus called 2019-nCoV, now officially known as severe acute respiratory syndrome coronavirus 2 (SARS-CoV2) that is responsible for the pandemic. The coronavirus COVID-19 affected 215 countries and territories around the world and more than 99 hundred thousand people at present (Nature Nanotechnology, 2020). At present, there are no specific vaccines or treatments available for COVID-19. However, there are many ongoing clinical trials evaluating potential treatments. At this time the experts recommend precautions such as social distancing, hand washing, and wearing face masks to reduce disease transmission. This review article aims to improve the readers' awareness towards the important role of mobile technology for SARS-CoV-2.MethodologyTo achieve this objective, we performed a COVID-19 literature review from various sources that include data from the published articles as well as World Health Organization reports on coronavirus disease and how mobile technology is useful to fight against this disease.ResultsMobile technology can be helpful in mapping disease spread and provides an easy way to provide awareness that promotes safety and adoption of necessary precautions to mitigate and stop community transmission.ConclusionThe spread rate of COVID-19 is very high and until now no vaccines are available to control this disease. To this end we should leverage other avenues such as digital technologies to protect ourselves from this disease. Mobile technology such as smartphones are playing an important role in this pandemic, by launching apps to track coronavirus infected people. These apps are very easy to use and provide self-isolation guidelines as well as other safety tips.

Project description:IntroductionIn 2018, the Baltimore City Health Department launched a mobile clinic called Healthcare on The Spot, which offers low-threshold buprenorphine services integrated with health care services to meet the needs of people who use drugs. In addition to buprenorphine management, The Spot offers testing and treatment for hepatitis C, sexually transmitted infections, and HIV, as well as pre-exposure prophylaxis for HIV, wound care, vaccinations, naloxone distribution, and case management.Methods and materialsThis cohort analysis includes clinical service data from the first 15 months of The Spot mobile clinic, from September 4, 2018, to November 23, 2019. The Spot co-located with the Baltimore syringe services program in five locations across the city. Descriptive data are provided for patient demographics and services provided, as well as percent of patients retained in buprenorphine treatment at one and three months. Logistic regression identified factors associated with retention at three months.ResultsThe Spot mobile clinic provided services to 569 individuals from September 4, 2018, to November 23, 2019, including prescribing buprenorphine to 73.8% and testing to more than 70% for at least one infectious disease. Patients receiving a prescription for buprenorphine were more likely to be tested for HIV, hepatitis C, and sexually transmitted infections, as well as receive treatment for hepatitis C and preventive services including vaccination and naloxone distribution. The Spot initiated HIV treatment for four patients and HIV pre-exposure prophylaxis for twelve patients. More than 32% of patients had hepatitis C; nineteen of these patients initiated treatment for hepatitis C with eight having a documented cure. Buprenorphine treatment retention was 56.0% at one month and 26.2% at three months. Patients who were Black or receiving treatment for hepatitis C were more likely to be retained in buprenorphine treatment at three months.ConclusionsIncreasing access to integrated medical services and drug treatment through low-threshold, community-based models of care can be an effective tool for addressing the effects of drug use.

Project description:Modern theories of semantics posit that the meaning of words can be decomposed into a unique combination of semantic features (e.g., "dog" would include "barks"). Here, we demonstrate using functional MRI (fMRI) that the brain combines bits of information into meaningful object representations. Participants receive clues of individual objects in form of three isolated semantic features, given as verbal descriptions. We use machine-learning-based neural decoding to learn a mapping between individual semantic features and BOLD activation patterns. The recorded brain patterns are best decoded using a combination of not only the three semantic features that were in fact presented as clues, but a far richer set of semantic features typically linked to the target object. We conclude that our experimental protocol allowed us to demonstrate that fragmented information is combined into a complete semantic representation of an object and to identify brain regions associated with object meaning.

Project description:ObjectivePhysician characteristics may be correlated with medical treatment decisions and patient outcomes. This study examined the correlations between characteristics of infectious disease (ID) physicians and the use of the restricted antimicrobial meropenem.DesignThis was a retrospective cohort study following 27 attending ID physicians for 5 years at a large academic medical center.MethodsAll inpatient ID clinical encounters between 2013 and 2018 were assessed for physician and patient characteristics, including patient Charlson Comorbidity Index, patient sex, ID service seeing the patient, physician career stage, physician training location, and physician sex. Adjusted and unadjusted odds ratios were calculated for the receipt of meropenem on the same day as an ID clinical note.ResultsBetween 2013 and 2018, meropenem was administered on the same day as 9046 (11.1%) of 81,787 inpatient ID encounters. After adjustment for patient and practice-specific factors, physician career stage was associated with administration of meropenem. Patients seen by mid-career and late-career ID physicians were more likely to receive meropenem than those seen by early-career physicians (aOR 1.22 95% confidence interval [CI 1.13-1.31 and aOR 1.17 95% CI 1.10-1.25, respectively).ConclusionsID provider characteristics may help target future antimicrobial stewardship program interventions.
