NURTURE: development and pilot testing of a novel parenting intervention for mothers with histories of an eating disorder.

ABSTRACT: To describe the treatment development and pilot testing of a group parenting intervention, NURTURE (Networking, Uniting, and Reaching out To Upgrade Relationships and Eating), for mothers with histories of eating disorders.Based on focus group findings, extant research, and expert opinion, NURTURE was designed to be delivered weekly over 16 (1.5 h) sessions via an interactive web conferencing forum. It comprises four modules: (1) laying the foundation, (2) general parenting skills, (3) eating and feeding, and (4) breaking the cycle of risk. Pilot testing was conducted with three groups of 3-6 mothers (N = 13) who had children ages 0-3 years to determine feasibility (e.g., retention), acceptability (e.g., feedback questionnaire responses), and preliminary efficacy. Maternal satisfaction with NURTURE and changes in mother-child feeding relationship measures, maternal feeding style, maternal self-efficacy, and maternal psychopathology (eating disorder, depression, and anxiety symptoms) across three time points (baseline, post-treatment, 6-month follow-up) were examined. All outcomes were exploratory.The intervention was well tolerated with a 100% retention rate. Feedback from mothers was generally positive and indicated that the groups provided an engaging, supportive experience to participants. We observed changes suggestive of improvement in self-reported maternal self-efficacy and competence with parenting. There were no notable changes in measures of maternal feeding style or psychopathology.NURTURE is a feasible, acceptable, and potentially valuable intervention for mothers with eating disorder histories. Results of this pilot will inform a larger randomized-controlled intervention to determine efficacy and impact on child outcomes.

Project description:Background/aimsBorderline personality disorder (BPD) is a severe mental disorder characterized by an unstable sense of self, intense and rapidly changing affect, as well as impulsive and self-destructive behaviors. Interpersonal relationships of individuals with BPD are characterized by marked instability, a lack of dependability, and quick changes between love and hate. For children of individuals with BPD, this can lead to permanent stress and attachment insecurity and an increased risk of adverse physical and mental health development. To reduce dysfunctional parenting and improve positive parenting, and in turn, to promote healthy child development, a group intervention for mothers with BPD was developed. This study aims to evaluate this first disorder-specific parenting intervention for BPD in a randomized controlled trial.MethodIn a parallel-group, two-arm, randomized controlled trial, an initial N = 178 mothers diagnosed with BPD and their children aged 6 months to 6 years are assigned to either the parenting intervention or a waiting control group. If taking place, participants of both groups continue their regular treatment for BPD diagnosis (e.g., individual therapy, medication). The primary outcomes are changes in parenting from baseline (day 0) to post intervention (week 12) and follow-up (6 months after group intervention; month 9). The waiting control group can attend the group intervention at the end of all assessments. Participants allocated to the intervention group are expected to show improvement in their parenting and a reduction in child abuse potential. Maternal emotion regulation and mental distress are analyzed as secondary outcomes.DiscussionMothers with BPD may need tailored help when reporting difficulties raising their children. The first disorder-specific parenting intervention has been developed to close this gap. ProChild is part of a large government-supported consortium, which aims to investigate different aspects of abuse and maltreatment in childhood and adolescence.Trial registrationClinicalTrials.gov NCT04169048 . Project description:Food images are routinely used to investigate the cognitive and neurobiological mechanisms of eating behaviors, but there is a lack of standardized image sets for use in children, which limits cross-study comparisons. To address this gap, we developed a set of age-appropriate images that included 30 high-energy-dense (ED) foods (>2.00 kcal/g), 30 low-ED foods (<1.75 kcal/g), and 30 office supplies photographed in two amounts (i.e., "larger" and "smaller"). Preliminary testing was conducted with children (6-10 years) to assess recognition, emotional valence (1 = very sad, 5 = very happy), and excitability (1 = very bored, 5 = very excited). After the initial testing, 10 images with low recognition were replaced; thus, differences between Image Set 1 and Image Set 2 were analyzed. Thirty (n = 30, mean age 8.3 ± 1.2 years) children rated Set 1, and a different cohort of 29 children (mean age 8.1 ± 1.1 years) rated Set 2. Changes made between image sets improved recognition of low-ED foods (Set 1 = 88.3 ± 10.5% vs. Set 2 = 95.6 ± 10.6%; p < 0.0001) and office supplies (83.7 ± 10.5 vs. 93.0 ± 10.6%; p < 0.0001). For the revised image set, children recognized more high-ED foods (98.4 ± 10.6%) than low-ED foods (95.6 ± 10.6%; p < 0.05) and office supplies (93.0 ± 10.6%; p < 0.0001). Recognition also improved with age (p < 0.001). Excitability and emotional valence scores were greater for high-ED foods compared with both low-ED foods and office supplies (p < 0.0001 for both). However, child fullness ratings influenced the relationship between excitability/emotional valence and category of item (p < 0.002). At the lowest fullness level, high-ED foods were rated the highest in both excitability and emotional valence, followed by low-ED foods and then office supplies. At the highest fullness level, high-ED foods remained the highest in excitability and emotional valence, but ratings for low-ED foods and office supplies were not different. This suggests that low-ED foods were more exciting and emotionally salient (relative to office supplies) when children were hungry. Ratings of recognition, excitability, and emotional valence did not differ by image amount. This new, freely available, image set showed high recognition and expected differences between image category for emotional valence and excitability. When investigating children's responsiveness to food cues, specifically energy density, it is essential for investigators to account for potential influences of child age and satiety level.

Project description:BACKGROUND:Low adherence to dietary guidelines and a concurrent rise of obesity-related chronic diseases emphasize the need for effective interventions to promote healthy eating. There is growing recognition that behavior change interventions should draw on theories of behavior change. Online interventions grounded in theory lead to increased effectiveness for health behavior change; however, few theory-driven social media-based health promotion interventions have been described in the literature. OBJECTIVE:The objective of this study was to describe the application of the Intervention Mapping (IM) protocol to develop an evidence-informed blog to promote healthy eating among French-Canadian mothers of preschool and school-aged children. METHODS:The following six steps of the IM protocol were performed. In Step 1, a preliminary needs assessment included a literature search on theoretical domains predicting Vegetables and Fruits intakes and Milk and Alternatives intakes in adults (ie, knowledge, beliefs about capabilities, beliefs about consequences, intention/goals) and a qualitative study including focus groups to identify female Internet users' perceptions of their use of healthy eating blogs. In Step 2, two behavioral outcomes were selected (ie, increase daily intakes of Vegetables and Fruits and Milk and Alternatives of mothers to reach Canadian dietary recommendations) and subsequently divided into six performance objectives inspired by national and international dietary recommendations such as planning for healthy meals. A matrix of change objectives was then created by crossing performance objectives with theoretical domains predicting Vegetables and Fruits intakes and Milk and Alternatives intakes in adults. Step 3 consisted of selecting theory-based intervention methods (eg, modeling and goal setting) and translating them into practical applications for the context of a dietary intervention delivered through a blog. A 6-month intervention was developed in Step 4 in which we aimed to address one performance objective per month in weekly blog publications written by a registered dietitian. For Step 5, we sought to include engagement-promoting methods (eg, peer and counselor support) to promote mothers' use of the blog and adherence to the intervention. Finally in Step 6, a randomized controlled trial has been launched to evaluate the effects of the blog on dietary behaviors of French-Canadian mothers. RESULTS:The intervention study is expected to be completed in March 2018. CONCLUSIONS:An intervention mapping protocol allowed for effective decision making in the development of a novel knowledge translation tool to increase adherence to dietary recommendations among mothers of preschool and school-aged children.
