Optimizing efficiency of psychopathology assessment through quantitative modeling: development of a brief form of the Externalizing Spectrum Inventory.

ABSTRACT: The Externalizing Spectrum Inventory (ESI; Krueger, Markon, Patrick, Benning, & Kramer, 2007) provides for integrated, hierarchical assessment of a broad range of problem behaviors and traits in the domain of deficient impulse control. The ESI assesses traits and problems in this domain through 23 lower order facet scales organized around 3 higher order dimensions, reflecting general disinhibition, callous aggression, and substance abuse. The full-form ESI contains 415 items, and a shorter form would be useful for questionnaire screening studies or multimethod research protocols. In the current work, we employed item response theory and structural modeling methods to create a 160-item brief form (ESI-BF) that provides for efficient measurement of the ESI's lower order facets and quantification of its higher order dimensions either as scale-based factors or as item-based composites. The ESI-BF is recommended for use in research on psychological or neurobiological correlates of problems such as risk-taking, delinquency, aggression, and substance abuse, and studies of general and specific mechanisms that give rise to problems of these kinds.

Project description:BackgroundThe Externalizing Spectrum Inventory-Brief Form (ESI-BF) [1] is a 160-item self-report instrument designed for the assessment of externalizing psychopathology, yet few studies to date have evaluated its psychometric properties, structural fit, and criterion validity in forensic psychiatric settings.MethodsHere, we investigated these aspects in a sample of forensic psychiatric inpatients (n = 77) from a maximum-security forensic psychiatric hospital in Sweden. We firstly investigated the reliability. Secondly, using confirmatory factor analysis, the structure of the ESI-BF. And thirdly, using a Bayesian approach, assessed how the three ESI-BF subfactors relate to criterion measures of antisocial behaviors, substance use, and lifetime externalizing spectrum diagnoses.ResultsThe ESI-BF demonstrated good to adequate reliability and internal consistency, with all but four facet scales exhibiting α and ω values ≥ 0.80. Average inter-item correlations for the facet scales ranged from 0.31 to 0.74. However, all structural models exhibited poor to mediocre fit, with model fit values for the CFI being 0.66, 0.79 and 0.87 and RMSEA values of 0.14, 0.12 and 0.09. for the unidimensional correlated factors and bifactor model, respectively. Regarding criterion validity, all subscales of the item-based ESI-BF three-factor model exhibited robust correlations with the Life History of Aggression total, aggression and antisocial/consequences subscales, with correlations ranging from 0.29 to 0.55. All ESI-BF subfactors demonstrated robust associations, yet with different externalizing outcomes, lending tentative support to its criterion validity.ConclusionDespite remaining ambiguities regarding its structural fit, the ESI-BF may be promising for assessing externalizing psychopathology in forensic psychiatric populations. However, further investigation of the ESI-BF is needed before any firm conclusions can be drawn about its appropriateness in forensic psychiatric settings.

Project description:BackgroundIndividuals with attention-deficit/hyperactivity disorder and other externalizing psychopathologies tend to display poor behavioral performance on the go/no-go task, which is thought to reflect deficits in inhibitory control. However, clinical neuroimaging studies using this task have yielded conflicting results, raising basic questions about what the task measures and which aspects of the task relate to clinical outcomes. We used computational modeling to provide a clearer understanding of how neural activations from this task relate to the cognitive mechanisms that underlie performance and to probe the implications of these relationships for clinical research.MethodsA total of 143 young adults (8-21 years of age) performed the go/no-go task during functional magnetic resonance imaging scanning. We used the diffusion decision model (DDM), a cognitive modeling approach, to quantify distinct neurocognitive processes that underlie go/no-go performance. We then assessed correlations between DDM parameters and brain activation from standard go/no-go contrasts and assessed relationships of DDM parameters and associated neural measures with clinical ratings.ResultsRight-lateralized prefrontal activations on correct inhibition trials, which are generally assumed to isolate neural processes involved in inhibition, were unrelated to DDM parameters (and other performance indices). However, responses to failed inhibitions in brain regions associated with error monitoring were strongly related to more efficient task performance and correlated with externalizing behavior and attention-deficit/hyperactivity disorder symptoms.ConclusionsOur findings cast doubt on conventional interpretations of go/no-go task-related activations as reflecting the neural basis of inhibitory functioning. We instead found evidence that error-related contrasts provide clinically relevant information about neural systems involved in monitoring and optimizing the efficiency of cognitive performance.

Project description:Little is known on how to best prioritize various tele-ICU specific tasks and workflows to maximize operational efficiency. We set out to: 1) develop an operational model that accurately reflects tele-ICU workflows at baseline, 2) identify workflow changes that optimize operational efficiency through discrete-event simulation and multi-class priority queuing modeling, and 3) implement the predicted favorable workflow changes and validate the simulation model through prospective correlation of actual-to-predicted change in performance measures linked to patient outcomes.SettingTele-ICU of a large healthcare system in New York State covering nine ICUs across the spectrum of adult critical care.PatientsSeven-thousand three-hundred eighty-seven adult critically ill patients admitted to a system ICU (1,155 patients pre-intervention in 2016Q1 and 6,232 patients post-intervention 2016Q3 to 2017Q2).InterventionsChange in tele-ICU workflow process structure and hierarchical process priority based on discrete-event simulation.Measurements and main resultsOur discrete-event simulation model accurately reflected the actual baseline average time to first video assessment by both the tele-ICU intensivist (simulated 132.8 ± 6.7 min vs 132 ± 12.2 min actual) and the tele-ICU nurse (simulated 128.4 ± 7.6 min vs 123 ± 9.8 min actual). For a simultaneous priority and process change, the model simulated a reduction in average TVFA to 51.3 ± 1.6 min (tele-ICU intensivist) and 50.7 ± 2.1 min (tele-ICU nurse), less than the added simulated reductions for each change alone, suggesting correlation of the changes to some degree. Subsequently implementing both changes simultaneously resulted in actual reductions in average time to first video assessment to values within the 95% CIs of the simulations (50 ± 5.5 min for tele-intensivists and 49 ± 3.9 min for tele-nurses).ConclusionsDiscrete-event simulation can accurately predict the effects of contemplated multidisciplinary tele-ICU workflow changes. The value of workflow process and task priority modeling is likely to increase with increasing operational complexities and interdependencies.
