Arthroscopic technique for the treatment of patellar chondral lesions with the patient in the supine position.

ABSTRACT: We describe an arthroscopic approach for the treatment of patellar chondral lesions with the patient in the supine position. This approach can be used to perform certain procedures such as matrix autologous chondrocyte implantation and autologous matrix-induced chondrogenesis. It is possible to perform these arthroscopic techniques working at an angle perpendicular to the patellar joint surface. First, with the patient in the supine position, arthroscopic longitudinal sectioning of the lateral patellar retinaculum is performed, and the patella is reverted with the help of a Codivilla forceps. It is then possible to place the chondral surface perpendicular to the floor, and it can be accessed directly through a lateral parapatellar portal. Short-term follow-up has shown the benignity of opening the patellar retinaculum. This procedure reduces morbidity compared with the traditional open surgery.

Project description:Microfracture of hip chondral lesions has been performed for more than a decade with modified treatment principles and techniques from knee arthroscopy. This note and accompanying video review the pertinent techniques, pearls, and pitfalls of the microfracture procedure in the treatment of hip chondral lesions. After debridement of damaged chondral tissue, the size of the lesion is approximated to determine the number of microfracture holes to create. The working portal may be adjusted based on the site of the lesion. Microfracture picks of different angles are used to ensure perpendicular advancement to a depth of 3 to 4 mm in the subchondral bone. The holes are placed at a gap of approximately 3 to 4 mm. The debris is washed out to obtain open holes connecting the marrow with the articular surface. The microfracture procedure should be performed near the end of the overall procedure to secure the bleeding bone marrow within the joint and prevent washout.

Some specific technical notes are given to provide as much information as possible to help orthopaedic surgeons wishing to perform shoulder cuff repair in this position.

All-Arthroscopic Nanofractured Autologous Matrix-Induced Chondrogenesis (A-NAMIC) Technique for the Treatment of Focal Chondral Lesions of the Knee.

Project description:Nanofractured autologous matrix-induced chondrogenesis (NAMIC©) is a 1-step technique that combines nanofracture needling to induce bone marrow stimulation (BMS) and the use of cell-free collagen matrix to optimize cartilage regeneration. In this Technical Note, we describe a modification of the NAMIC procedure using mosaicplasty trephines to prepare the lesion surface and to shape collagen implants in an all-arthroscopic approach (A-NAMIC). This technique is indicated for the treatment of International Cartilage Repair Society grade III to IV knee chondral lesions of ≤4 cm2. After damaged cartilage is debrided, trephines are used to create a flat, circular lesion surfaces. Subsequently, BMS is performed with nanofracture, eliciting reproducible and stop-controlled subchondral bone perforations of 9-mm depth and 1-mm width. The collagen membrane is then cut to size with the trephine, placed over the prepared defect, and secured with fibrin glue, preventing loss of regenerating cells and growth factors to the joint space. Using trephines allows the rapid and precise creation of smooth defect surfaces with known dimensions, ensuring optimal lesion coverage. Additionally, nanofracture reduces trabecular compaction and allows for a deeper access to subchondral bone in comparison with conventional microfracture, improving lesion filling and production of cartilage with higher hyaline content.

Partial Patellar Lateral Facetectomy: Arthroscopic Technique.

Project description:Lateral patellofemoral osteoarthritis is a relatively common condition in young, active patients. Partial lateral patellar facetectomy is a relatively minimally invasive option to restore knee function and return to sport when conservative treatments are insufficient. We describe the arthroscopic technique of partial lateral patellar facetectomy , which has been historically performed as an open procedure. We describe preoperative planning, arthroscopic portals, landmarks, intra-articular evaluation of bone resection, and surgical difficulties. The goal of the preoperative planning was a target bone resection of 10 to 15 mm including osteophytes and the lateral facet of the patella. Arthroscopic anterolateral and anterior superolateral portals were used to achieve the desired resection. An arthroscopic technique allows a direct view of the osteoarthritic bone for removal and permits intraoperative dynamic evaluation, which allows the surgeon to finetune the bone resection and obtain optimal patellar tracking. Avoiding arthrotomy could reduce morbidity and allows a rapid postoperative rehabilitation.

Hip Chondral Defects: Arthroscopic Treatment With the Needle and Curette Technique and ChondroFiller.

Project description:Management of symptomatic focal cartilage defects of the hip can be challenging. Cells, scaffold therapies, and injectable agents have emerged as an adjunctive modality to improve clinical outcomes. Long and malleable needles that can be bent are used to release these kinds of biological products. Distance between the tip of the needle and the area to be filled should be minimal to ensure full contact with the chondral lesion to avoid losing material inside the hip cavity and to increase the efficiency of the release of the product. Nevertheless in many cases the accessibility is not easy, and the distance between the tip of the needle and the area to be treated is such that the efficiency of the release is difficult, if not impossible. We aim to describe a simple, inexpensive, and reproducible technique to facilitate the implantation of biologic and injectable materials in hip chondral defects during arthroscopy: the use of a combination of a curette and a needle inside the tip of the curette. Additionally we describe the use of ChondroFiller liquid, a liquid cell-free collagen matrix, for the treatment of symptomatic full-thickness chondral defects of the hip in a 1-step arthroscopic procedure.

Valgus Extension Overload: Arthroscopic Decompression in the Supine-Suspended Position.

Project description:Elbow arthroscopy is a useful tool for managing diseases of the elbow, including valgus extension overload, when conservative treatments have failed. Arthroscopic access to the elbow in the supine-suspended position is simple and reproducible with the technique described in this report. Synovial tissue can be cleared, optimizing visualization of the anatomic structures in the elbow including the posterior ulnohumeral joint. This report describes, in detail, arthroscopy of the elbow in the supine-suspended position and basic principles for arthroscopic decompression of the posterior elbow for valgus extension overload.

Surgical Technique: Jumper's Knee-Arthroscopic Treatment of Chronic Tendinosis of the Patellar Tendon.

Project description:Chronic patellar tendinosis (jumper's knee) is a common problem among athletes. Conservative treatment is successful in most of the cases including, among others, the use of nonsteroidal anti-inflammatory drugs, local cryotherapy, eccentric muscle training, limitation of sports activity, and local infiltration. In approximately 10% of conservatively treated patients, conservative treatment fails and surgery is required. Different open and arthroscopic surgical techniques have been described in the literature. The presented all-arthroscopic surgical technique for the treatment of chronic patellar tendinosis includes debridement of soft tissue at the lower patellar pole and resection of the bony lower patellar pole. It leads to excellent clinical results comparable to described open treatment and provides the benefits of a minimally invasive and safe procedure with a faster recovery and return to sporting activities after surgery. An additional bony resection in case of a prominent lower patellar pole does not lead to a significant extension of the operation time and may avoid a relapse or treatment failure in selective cases. Therefore, arthroscopic treatment such as the presented technique may be the preferred method for surgical treatment of chronic patellar tendinosis.

Treatment of Proximal Trochlear Dysplasia in the Setting of Patellar Instability: An Arthroscopic Technique.

Project description:Patellar instability is a complex disorder with multiple etiologies, and treatment must be individualized to the unique pathoanatomy of each patient. Medial patellofemoral ligament reconstruction is one of the most commonly performed procedures for the treatment of patellar instability. Patients with a symptomatic supratrochlear spur, defined by the presence of a "jumping" J sign on examination, also may benefit from an adjunctive proximal trochlear resection. Here, we describe a technique for an arthroscopic proximal trochlear resection, or "bumpectomy," involving resection of the supratrochlear spur. In appropriately indicated patients, we have found this procedure to be a useful adjunct to medial patellofemoral ligament reconstruction without the need for concurrent trochlear sulcus deepening.

A Single-Portal Arthroscopic Technique for Type II Slap Lesions in the Beach Chair Position.

Project description:The options for surgical treatment of an anterior labrum lesion have become extensive. Arthroscopic treatments are widely used as an improved minimally invasive option with a quick recovery. Arthroscopic treatment of the anterior glenoid labrum generally requires the creation of two working portals. However, arthroscopic treatment through a single anterior portal is still successful. Our single-portal technique avoids interference between instruments inserted through the two working portals and minimizes postoperative scarring, pain, and reduction in range of motion. The purpose of this article was to describe our single-portal arthroscopy technique to repair the anterior glenoid labrum.
