Reconstruction of the posterolateral corner of the knee with achilles tendon allograft.

ABSTRACT: Posterolateral corner injuries of the knee are relatively rare; however, they can result in significant long-term disability without appropriate treatment. They often occur in the setting of multiligament knee injuries, and as a result, diagnosis and management can be challenging. Severe injuries often require reconstruction, and both anatomic and nonanatomic techniques exist. We describe our preferred operative technique to reconstruct the fibular collateral ligament and posterolateral corner using a single Achilles tendon allograft.

Knee Posterolateral Corner Reconstruction with a Single Tendon.

Project description:Knee posterolateral corner (PLC) injuries are troublesome conditions and are always involved in complicated knee-ligament injuries. Various surgical techniques have been reported to address these conditions, in either an open or an arthroscopic manner. However, a simple and effective method is still being pursued. We introduce a mini-invasive PLC reconstruction technique in which a single tendon is used to reconstruct the lateral collateral ligament, the popliteofibular ligament and the popliteal tendon simultaneously. The critical points of this technique are proper location and creation of the tibial, fibular and femoral tunnels, proper passing and setting of the tendon graft, as well as protection of the peroneal nerve. Our clinical experience indicates that this technique is easy to perform and effective. We consider that the introduction of this technique will provide more reasonable options when PLC reconstruction is indicated.

Superior Capsular Reconstruction with Achilles Tendon Allograft.

Project description:Superior capsular reconstruction (SCR) has become an increasingly popular choice in the treatment of massive, irreparable rotator cuff tears, pseudoparalysis, and in cases in which revision of previous rotator cuff repair is indicated. The SCR procedure is intended to restore the superior stabilizing forces of a deficient rotator cuff. This technique is accomplished by substituting an autograft or allograft between the superior glenoid and the greater tuberosity of the humerus, thus keeping the humeral head centered in the glenoid during shoulder forward flexion and abduction. Since its advent, numerous techniques have been described for this procedure. A fascia lata autograft was initially described; however, many surgeons in the United States have advocated for the use of a humeral dermal allograft. Yet, biomechanical studies have demonstrated elongation and thinning of this material. Thus, the Achilles tendon allograft may be an attractive choice for SCR, given its previous success with ligamentous and tendinous reconstructions. In this article, we present our technique of SCR using an Achilles tendon allograft.

A Modified Surgical Technique of Patellar Tendon Reconstruction in Total Knee Arthroplasty Using Achilles Tendon Allograft.

Project description:Patellar tendon disruption is one of the most dreaded complications after total knee arthroplasty (TKA) impacting joint function and implant longevity. Management of this condition remains controversial, and no one treatment has provided consistent clinical success. Here we propose a modified technique from the standard Achilles allograft procedure to reconstruct a ruptured patellar tendon after TKA. A fresh-frozen Achilles tendon allograft with attached calcaneal bone graft was used. Our technique offers the advantage of a more stable graft fixation by relying on both quadriceps tendon and transosseous patellar fixation and bone-to-bone allograft fixation to the tibial tuberosity. The patient regained full extension and discontinued walking aids postoperatively. Careful attention to graft preparation, tensioning, stable graft fixation, and postoperative rehabilitation led to encouraging results in this complex problem.

Project description:Arthroscopic assessment of posterior compartments of the knee and the posterior aspect of the proximal tibial and fibula is challenging because of the relative proximity of the neurovascular bundle. This Technical Note describes a reproducible arthroscopic surgical approach in a cadaveric model that aims to identify and expose the popliteus tendon, posterior fibular head, fibular collateral ligament popliteal fibular ligament, biceps femoris tendon, and peroneal nerve.

Project description:Concomitant injuries of the 4 groups of ligaments of the knee are a serious condition and challenge the orthopaedic surgeon when reconstruction of all ligaments is needed. Staged reconstruction can be chosen owing to the complexity of the combined procedures; however, simultaneous reconstruction is favored to facilitate recovery. We describe a simultaneous anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), posteromedial corner (PMC), and posterolateral corner (PLC) reconstruction technique, in which autografts are used and both cruciate ligaments are reconstructed in a single-bundle manner. We believe the introduction of this technique will familiarize surgeons with the principle of simultaneous 4-ligament reconstruction of the knee and the method of cruciate ligament balancing during bicruciate ligament reconstruction.

Proximal Tibiofibular Joint Stabilization With Concurrent Posterolateral Corner Reconstruction in Multiligamentous Knee Injury.

Project description:Proximal tibiofibular joint (PTFJ) instability is a rare knee injury, accounting for less than 1% of knee injuries. It causes significant lateral sided knee pain and functional deficits and can be associated with up to 9% of multiligament knee injuries. Concurrent surgical treatment of posterolateral corner (PLC) and PTFJ instability poses technical challenges due to the limited working space of the fibula head and inherent risk of collision between grafts, bone tunnels, and implants. In this Technical Note, we detail our senior author's technique for PTFJ reconstruction without the use of additional bone tunnels or implants in the fibula head, to reduce the risk of overcrowding and tunnel collision.ClassificationsLevel I: knee; Level II: other, proximal tibiofibular joint.

Anatomic Posterolateral Corner Reconstruction.

Project description:Posterolateral corner injuries represent a complex injury pattern, with damage to important coronal and rotatory stabilizers of the knee. These lesions commonly occur in association with other ligament injuries, making decisions regarding treatment challenging. Grade III posterolateral corner injuries result in significant instability and have poor outcomes when treated nonoperatively. As a result, reconstruction is advocated. A thorough knowledge of the anatomy is essential for surgical treatment of this pathology. The following technical note provides a diagnostic approach, postoperative management, and details of a technique for anatomic reconstruction of the 3 main static stabilizers of the posterolateral corner of the knee.

Medial Patellofemoral Ligament Reconstruction Using Achilles Tendon Allograft With Bone Block.

Project description:Recurrent patellar instability is a common problem and often leads to a tear of the medial patellofemoral ligament. Multiple reconstruction techniques for the medial patellofemoral ligament (MPFL) exist. This Technical Note presents a technique for performing MPFL reconstruction using Achilles tendon allograft with a bone block. The advantages of this technique include (1) bone-to-bone healing in femoral tunnel; (2) docking the bone block in the femoral tunnel obviates the knee for calculating graft and tunnel length; (3) fixing the femoral side before the patellar side allows graft tensioning under direct visualization and avoids over-constraint; (4) the fanned portion of the Achilles tendon allows broad-based MPFL footprint coverage on the medial patellar ridge; and (5) it avoids the need for bone tunnel drilling in the patella, reducing the risk to penetration of the drill holes into the articular cartilage surface as well as the risk for patellar fracture.

Minimally Invasive, Arthroscopic-Assisted, Anatomic Posterolateral Corner Reconstruction.

Project description:As the anatomy and biomechanics of the posterolateral corner (PLC) of the knee have become better understood, the importance of the PLC's proper function has become a more frequently raised subject. Misdiagnosed chronic posterolateral instability may lead to serious consequences, including cruciate ligament reconstruction graft failure. It has been proved that high-grade PLC injuries need to be treated operatively. Surgical approaches vary, and techniques are still developing. Considering avoidance of an extended surgical approach and minimizing the risk of common peroneal nerve or popliteal artery injuries, we developed the minimally invasive, arthroscopic-assisted, anatomic PLC reconstruction.
