Dental caries status of Dai preschool children in Yunnan Province, China.

ABSTRACT:

Background

The Dai people, one of the ethnic minorities in China, have a population of 1,260,000. They have the same origin as one of the main ethnic groups of Laos and Thailand. Most of the Dai live in Yunnan province, which is located in the less-developed southwestern part of China. This study aimed to describe the oral health status of Dai preschool children in China and the factors that influence their oral health status.

Methods

An oral health survey was performed between 2011 and 2012 to select Dai five-year-old children using multi-stage stratified sampling in Yunnan. Their dental caries experience was measured using the "dmft" index, and severe caries was assessed using the "pa" index, which is modified from the "pufa" index. Oral hygiene status was assessed using the visual plaque index (VPI). A questionnaire to study the children's socio-demographic background and oral health-related behaviours was completed by the children's parents.

Results

A total of 833 children were examined. Their caries prevalence was 89% and 49% of the children had carious tooth with pulp involvement. The mean (SD) dmft score was 7.0 (5.3). Higher dmft scores were found among children who were girls, were currently bottle-fed, took daily sweet snacks, had higher VPI scores, and had visited a dentist within the last year.

Conclusions

The caries prevalence and experience of the five-year-old Dai children in Yunnan, China was high, and almost half had severe caries. The caries experience was associated with gender, snack habits, dental visit habits, and oral hygiene status.

Project description:BACKGROUND:This study aims to understand the deciduous dental caries status of preschool children in Xuhui District of Shanghai, China and to analyze the associated risk factors. METHODS:In January of 2016, a cross-sectional investigation was conducted to examine the oral health of all the kindergarten children in Xuihui District of Shanghai, China. Meanwhile, a field questionnaire survey was conducted with the children's guardians to ascertain the potential risk factors associated with deciduous dental caries. RESULTS:Among 11,153 children, the prevalence of deciduous dental caries was 47.02%, and the mean dmft score was 2.21. The first three predilection sites were maxillary central primary incisors, mandible second primary molars, and mandible first primary molars. There were statistically significant differences in caries prevalence and dmft among different age groups and different household registration (Hukou) types (P?<?0.001). Multivariate Logistic regression suggested that the possible risk factors for deciduous caries included: older age, drinking sweetened beverages frequently, often or usually eating sweets before sleep compared to rarely/never eat them at this time, exclusive or predominant breastfeeding compared to exclusive or predominant artificial feeding and latter introduction of toothbrushing. On the other hand, Shanghai Hukou families, high educational level of guardians (high school or college education), regular parental support for children's toothbrushing, guardians' oral health knowledge, and a good perception about children's oral health conditions were shown as potential protective factors for deciduous dental caries. CONCLUSIONS:The deciduous dental caries status of preschool children in Xuhui District of Shanghai was still serious. The caries prevalence in Xuhui, China, is associated with children's age, household registration type, oral health habits, feeding habits, guardians' education level, parental perception about children's oral health and knowledge about oral health.

Project description:Yunnan Province, China is thought to be the original source of biovar Orientalis of Yersinia pestis, the causative agent of the third plague pandemic that has spread globally since the end of the 19th century. Although encompassing a large area of natural plague foci, Y. pestis strains have rarely been found in live rodents during surveillance in Yunnan, and most isolates are from rodent corpses and their fleas. In 2017, 10 Y. pestis strains were isolated from seven live rodents and three fleas in Heqing County (HQ) of Yunnan. These strains were supposed to have low virulence to local rodents Eothenomys miletus and Apodemus chevrieri because the rodents were healthy and no dead animals were found in surrounding areas, as had occurred in previous epizootic disease. We performed microscopic and biochemical examinations of the isolates,and compared their whole-genome sequences and transcriptome with those of 10 high virulence Y. pestis strains that were isolated from the adjacent city (Lijiang). We analyzed the phenotypic, genomic, and transcriptomic characteristics of live rodent isolates. The isolates formed a previously undefined monophyletic branch of Y. pestis that was named 1.IN5. Six SNPs, two indels, and one copy number variation were detected between live rodent isolates and the high virulence neighbors. No obvious functional consequence of these variations was found according to the known annotation information. Among the genes that were differentially expressed between the live rodent isolates and their high virulence neighbors, we detected five iron transfer-related genes that were significantly up-regulated in live rodent isolates compared with high virulence isolates (|log2 (FC) | >1, p.adjust <0.05), indicating these genes may be related to the low-virulence phenotype. The novel genotype of Y. pestis reported here provides further insights into the evolution and spread of plague as well as clues that may help to decipher the virulence mechanism of this notorious pathogen.

Project description:OBJECTIVES:As a vulnerable group, the elders' oral health gained less attention, particularly the relationship between socioeconomic status (SES) and dental caries. This study aimed to assess the associations and to explore the effects of confounders on the associations in elderly people. DESIGN:Cross-sectional study. SETTINGS:3 neighbourhood committees and 3 village committees in Sichuan Province, China. PARTICIPANTS:744 people (362 men and 382 women) aged 65-74 years were included. OUTCOME MEASURES:Oral health outcomes included the decayed, missing and filled teeth (DMFT) index and its components. SES was assigned by educational level, household income and type of household. The bivariate association between the participants' characteristics and DMFT was analysed using non-parametric tests. Four logistic regression models were used to analyse the associations between SES and dental caries by regulating confounders. RESULTS:Poor oral health was observed in these participants. Bivariate analysis showed a significant association between SES and DMFT (p?0.05). Only adjusting gender, high educational level (adjusted (AOR)=0.34, 95%?CI 0.17 to 0.66), high household income (AOR=0.47, 95%?CI 0.41 to 0.77) were protective factors against dental caries, and living in agricultural families (AOR=1.86, 95%?CI 1.32 to 2.63) was risk factor (p?0.05). After adjusting other confounders, SES was partly related to the dental caries. Moreover, an interaction existed among SES indicators. CONCLUSIONS:SES is associated with dental caries, and older people with low SES have poor oral health. The associations were explained partly by diet, behaviour and awareness. Our results provide effective evidence in targeted policy-making and intervention measures and implicate that pertinence measures, economic assistance and medical insurance funds should be provided to older people of low SES. Furthermore, a follow-up design should attempt to confirm the causal relationship between SES and dental caries and evaluate the effect of intervention.

Project description:Background:Vitamin D is crucial for mineralized tissue formation and immunological functions. The purpose of this study was to evaluate the association between vitamin D status and dental status in healthy children with vitamin D supplementation in infancy and at 6 years of age. Method:Eight-year-old children who had participated in a vitamin D intervention project when they were 6 years old were invited to participate in a dental follow-up study. They had fair or darker skin complexion and represented two geographically distant parts of Sweden. 25-hydroxy vitamin D in serum had been measured at 6 years of age and after a 3-month intervention with 25, 10 or 2 (placebo) ?g of vitamin D3 per day. Two years later, caries and enamel defects were scored, self-reported information on e.g., oral behavior, dietary habits and intake of vitamin D supplements was collected, and innate immunity peptide LL37 levels in saliva and cariogenic mutant streptococci in tooth biofilm were analyzed. The outcome variables were caries and tooth enamel defects. Results:Dental status was evaluated in 85 of the 206 children in the basic intervention study. Low vitamin D levels were found in 28% at baseline compared to 11% after the intervention, and 34% reported continued intake of vitamin D supplements. Logistic regression supported a weak inverse association between vitamin D status at 6 years of age and caries 2 years later (odds ratio 0.96; p =?0.024) with minor attenuation after an adjustment for potential confounders. Multivariate projection regression confirmed that insufficient vitamin D levels correlated with caries and higher vitamin D levels correlated with being caries-free. Vitamin D status at 6 years of age was unrelated to enamel defects but was positively associated with saliva LL37 levels. Conclusion:An association between vitamin D status and caries was supported, but it was not completely consistent. Vitamin D status at 6 years of age was unrelated to enamel defects but was positively associated with LL37 expression.
