Left-sided patent ductus arteriosus in a right-sided aortic arch.

ABSTRACT: We present a 31-year-old female with repaired tetralogy of Fallot (TOF) and right-sided aortic arch (RAA) with left-sided patent ductus arteriosus (PDA) originating from the left brachiocephalic artery. This is a rare finding but most common site for a PDA in TOF and a RAA. To the best of our knowledge, this is the first demonstration of this rare finding on MRI in the literature.

Successful treatment of patent ductus arteriosus accompanying right-sided aortic arch and aberrant left subclavian artery: a case report.

Project description:BackgroundThe coexistence of a right-sided aortic arch (RAA), an aberrant left subclavian artery (ALSA), and a patent ductus arteriosus (PDA) is a rarely seen vascular ring anomaly. There is currently no general guideline consensus on the management and follow-up of this congenital defect, posing a challenge to the clinicians. At this point, the heart team plays a critical role in the management of the disease.Case summaryIn the present case, a 25-year-old male patient was presented to the outpatient clinic with dyspnoea and fatigue. A transthoracic echocardiography revealed PDA with a left-to-right shunt. To evaluate the anatomy thoroughly, a thoracic computed tomographic angiography was performed and showed PDA accompanying ALSA and RAA. The patient was evaluated by the Heart Team, and a percutaneous closure of PDA was recommended due to signs of left ventricular volume overload. The closure was successfully performed with Amplatzer vascular plug II. At follow-up, the patient was free of symptoms.DiscussionClinicians should be aware of the potential concomitant lesions during the diagnostic work-up. In selected patients, percutaneous closure of PDA may be the first-line therapy in experienced centres.

In this condition, the aorta extends to the left of the trachea as in normal, while the ductus arteriosus extends to the right of the trachea and joins the descending aorta posterior to the trachea, with a cross-ribbon sign.

Left pneumonectomy for complex aspergilloma after patent ductus arteriosus ligation.

Project description:Surgery for complex pulmonary aspergilloma is known to be technically challenging, often for indurated hilar structures and obliterated pleural space. We report a case of left pneumonectomy for pulmonary aspergilloma with a history of patent ductus arteriosus ligation via anterolateral thoracotomy and aortopexy via median sternotomy and pericardiotomy. Left pneumonectomy was successfully accomplished by devising a surgical approach and procedures for transection of the left main pulmonary artery.

Simultaneous balloon aortic valvotomy and patent ductus arteriosus device closure in preterm newborn.

Project description:Multiple congenital heart disease in the small preterm newborn such as severe narrowing of aortic valve and patent ductus arteriosus (PDA) is a therapeutic challenge. We report successful transcatheter antegrade balloon dilatation of the aortic valve and device closure of the PDA in a 1700-gram preterm newborn. Meticulous planning and team work aids in such transcatheter intervention. <Learning objective: Preterm newborn babies rarely have multiple major structural cardiac lesions complicating the care. Transcatheter intervention for multiple congenital cardiac lesions such as severe valvular aortic stenosis and large patent ductus arteriosus in a small preterm baby is challenging but feasible. Precise anatomical diagnosis and meticulous planning is essential for successful intervention with minimum complications.>.

Left ventricular dysfunction postsurgical patent ductus arteriosus ligation in children: predictor factors analysis.

Project description:OBJECTIVE:To identify the predictor factors of left ventricular (LV) dysfunction following patent ductus arteriosus (PDA) surgical ligation. BACKGROUND:PDA is viewed as a noticeable amongst the most widely recognized congenital heart defects in children and its closure is responsible for many hemodynamic changes that require intervention and care. METHODS:A retrospective study included fifty children with isolated PDA treated by surgical ligation from June 2015 to June 2018. The LV dimensions and systolic function were assessed by two-dimensional echocardiography pre and post PDA ligation. All cases were followed-up on the first-day, 1 month and 6 months post ligation. RESULTS:The mean age of cases was 15.78?±?7.58?months and 72% were females. The mean duct size was 4.08?±?1.25?mm. There was a marked decrease in LVEDd, LA/Ao, EF and FS in the first-day post ligation contrasted with pre ligation values. Moreover, an amazing decline in LVEDd and LA/Ao ratio was observed 1 month post ligation contrasted with the early post ligation status with asynchronous improvement of FS and EF at one and 6 months postoperatively. CONCLUSION:PDA ligation is associated with a noteworthy LV systolic dysfunction within the first day post ligation; that in a significant number of patients may require anti-failure measures, prolong the hospital stay and necessitate a regular follow up and monitoring of LV function. PDA size, age, preoperative LVEDd and FS can be considered as predictor factors for suspicion of acute decrease in the LV systolic function early post PDA ligation.
