Development of the Device-Oriented Subjective Outcome (DOSO) scale.

ABSTRACT: BACKGROUND:The empiric basis for this work is derived from previous research completed in our laboratory and published in 2005 and 2007. The previous work suggested that self-report hearing aid outcomes can be viewed as device oriented or wearer oriented. Furthermore, compared with wearer-oriented outcomes, device-oriented outcomes were more independent of personality variables. PURPOSE:The purpose of this study was to develop a device-oriented questionnaire to measure self-report hearing aid outcomes. RESEARCH DESIGN:Design was a descriptive study in which 140 potential questionnaire items were evaluated and a questionnaire was devised. STUDY SAMPLE:A total of 306 adult hearing aid wearers participated: 189 were clinical patients and 117 were participants in hearing aid field trials. DATA COLLECTION AND ANALYSIS:Some items and some participants were removed because of insufficient responses. The final dataset included 295 participants and 66 items. Response data were subjected to exploratory principal component analysis with orthogonal rotation. Six components, explaining 64% of the variance, were retained. Item statistics were examined. RESULTS:Six subscales were identified. Long and short forms of the questionnaire were developed. There are two equivalent versions of the short form. CONCLUSIONS:The Device-Oriented Subjective Outcome (DOSO) questionnaire is suitable for quantifying subjective hearing aid outcomes in both research and clinical settings. The DOSO is especially suited for comparing outcomes with different hearing aids. Future research is needed to cross-validate the results, determine retest consistency, and to explore the extent to which data from the DOSO is independent of personality.

Project description:The capacity of milli and micro litre bioreactors to accelerate process development has been successfully demonstrated in traditional biotechnology. However, for regenerative medicine present smaller scale culture methods cannot cope with the wide range of processing variables that need to be evaluated. Existing microfabricated culture devices, which could test different culture variables with a minimum amount of resources (e.g. expensive culture medium), are typically not designed with process development in mind. We present a novel, autoclavable, and microfabricated scale-down device designed for regenerative medicine process development. The microfabricated device contains a re-sealable culture chamber that facilitates use of standard culture protocols, creating a link with traditional small-scale culture devices for validation and scale-up studies. Further, the modular design can easily accommodate investigation of different culture substrate/extra-cellular matrix combinations. Inactivated mouse embryonic fibroblasts (iMEF) and human embryonic stem cell (hESC) colonies were successfully seeded on gelatine-coated tissue culture polystyrene (TC-PS) using standard static seeding protocols. The microfluidic chip included in the device offers precise and accurate control over the culture medium flow rate and resulting shear stresses in the device. Cells were cultured for two days with media perfused at 300 µl.h(-1) resulting in a modelled shear stress of 1.1×10(-4) Pa. Following perfusion, hESC colonies stained positively for different pluripotency markers and retained an undifferentiated morphology. An image processing algorithm was developed which permits quantification of co-cultured colony-forming cells from phase contrast microscope images. hESC colony sizes were quantified against the background of the feeder cells (iMEF) in less than 45 seconds for high-resolution images, which will permit real-time monitoring of culture progress in future experiments. The presented device is a first step to harness the advantages of microfluidics for regenerative medicine process development.

Project description:Subjective measures involving clinician ratings or patient self-assessments have become recognised as an important tool for the assessment of health outcome. The value of a health outcome measure is usually assessed by a psychometric evaluation of its reliability, validity and responsiveness. However, psychometric testing involves an accumulation of evidence and has recognised limitations. It has been suggested that an evaluation of how well a measure has been developed would be a useful additional criteria in assessing the value of a measure. This paper explored the theoretical background and methodological development of subjective health status measures commonly used in osteoarthritis research. Fourteen subjective health outcome measures commonly used in osteoarthritis research were examined. Each measure was explored on the basis of their i) theoretical framework (was there a definition of what was being assessed and was it part of a theoretical model?) and ii) methodological development (what was the scaling strategy, how were the items generated and reduced, what was the response format and what was the scoring method?). Only the AIMS, SF-36 and WHOQOL defined what they were assessing (i.e. the construct of interest) and no measure assessed was part of a theoretical model. None of the clinician report measures appeared to have implemented a scaling procedure or described the rationale for the items selected or scoring system. Of the patient self-report measures, the AIMS, MPQ, OXFORD, SF-36, WHOQOL and WOMAC appeared to follow a standard psychometric scaling method. The DRP and EuroQol used alternative scaling methods. The review highlighted the general lack of theoretical framework for both clinician report and patient self-report measures. This review also drew attention to the wide variation in the methodological development of commonly used measures in OA. While, in general the patient self-report measures had good methodological development, the clinician report measures appeared less well developed. It would be of value if new measures defined the construct of interest and, that the construct, be part of theoretical model. By ensuring measures are both theoretically and empirically valid then improvements in subjective health outcome measures should be possible.
