Evidence for distorted mental representation of the hand in osteoarthritis.

ABSTRACT:

Objectives

Some chronic pain states are associated with a distortion of the perceived size or shape of the painful area, and multisensory illusions that disrupt these dimensions can modulate pain in healthy controls and people with painful disorders. Illusory hand resizing has recently been found to relieve pain in hand OA, raising the possibility that the illusion corrects some underlying perceptual disturbance. We evaluated this possibility by measuring perceived hand size in healthy controls and those with painful hand OA before and after illusory hand resizing. The aim was to investigate whether people with painful hand OA have distorted representations of hand size and whether these representations are malleable. We hypothesized that hand OA is associated with a distorted mental representation of the painful hand and that perceived hand size can be modulated via multisensory illusion.

Methods

Twelve volunteers with painful hand OA and 12 healthy age-matched controls performed three tasks (hand stretch, hand shrink and no illusion) in a randomized order then estimated the size of their hand using an adjustable photographic image.

Results

Our hypotheses were supported: under normal conditions, perceived hand size was smaller for the OA group than for healthy controls, consistent with a distorted mental representation of the painful hand. Furthermore, illusory stretching increased perceived hand size in both groups, while illusory shrinking decreased perceived hand size in healthy controls but not in the OA group.

Conclusion

These results suggest that hand OA is associated with a distorted mental representation of the painful hand and are consistent with the idea that the pain relief offered by multisensory illusions may work via normalization of this distortion.

Project description:In French, regardless of stem regularity, inflectional verbal suffixes are extremely regular and paradigmatic. Considering the complexity of the French verbal system, we argue that all French verbs are polymorphemic forms that are decomposed during visual recognition independently of their stem regularity. We conducted a behavioral experiment in which we manipulated the surface and cumulative frequencies of verbal inflected forms and asked participants to perform a visual lexical decision task. We tested four types of verbs with respect to their stem variants: a. fully regular (parler "to speak," [parl-]); b. phonological change e/E verbs with orthographic markers (répéter "to repeat," [répét-] and [répèt-]); c. phonological change o/O verbs without orthographic markers (adorer "to adore," [ador-] and [adOr-]); and d. idiosyncratic (boire "to drink," [boi-] and [buv-]). For each type of verb, we contrasted four conditions, forms with high and low surface frequencies and forms with high and low cumulative frequencies. Our results showed a significant cumulative frequency effect for the fully regular and idiosyncratic verbs, indicating that different stems within idiosyncratic verbs (such as [boi-] and [buv-]) have distinct representations in the mental lexicon as different fully regular verbs. For the phonological change verbs, we found a significant cumulative frequency effect only when considering the two forms of the stem together ([répét-] and [répèt-]), suggesting that they share a single abstract and under specified phonological representation. Our results also revealed a significant surface frequency effect for all types of verbs, which may reflect the recombination of the stem lexical representation with the functional information of the suffixes. Overall, these results indicate that all inflected verbal forms in French are decomposed during visual recognition and that this process could be due to the regularities of the French inflectional verbal suffixes.

Project description:Experimental studies on polysemy have come to contradictory conclusions on whether words with multiple senses are stored as separate or shared mental representations. The present study examined the semantic relatedness and semantic similarity of literal and non-literal (metonymic and metaphorical) senses of three word classes: nouns, verbs, and adjectives. Two methods were used: a psycholinguistic experiment and a distributional analysis of corpus data. In the experiment, participants were presented with 6-12 short phrases containing a polysemous word in literal, metonymic, or metaphorical senses and were asked to classify them so that phrases with the same perceived sense were grouped together. To investigate the impact of professional background on their decisions, participants were controlled for linguistic vs. non-linguistic education. For nouns and verbs, all participants preferred to group together phrases with literal and metonymic senses, but not any other pairs of senses. For adjectives, two pairs of senses were often grouped together: literal with metonymic, and metonymic with metaphorical. Participants with a linguistic background were more accurate than participants with non-linguistic backgrounds, although both groups shared principal patterns of sense classification. For the distributional analysis of corpus data, we used a semantic vector approach to quantify the similarity of phrases with literal, metonymic, and metaphorical senses in the corpora. We found that phrases with literal and metonymic senses had the highest degree of similarity for the three word classes, and that metonymic and metaphorical senses of adjectives had the highest degree of similarity among all word classes. These findings are in line with the experimental results. Overall, the results suggest that the mental representation of a polysemous word depends on its word class. In nouns and verbs, literal and metonymic senses are stored together, while metaphorical senses are stored separately; in adjectives, metonymic senses significantly overlap with both literal and metaphorical senses.

Project description:Data from focal brain injury and functional neuroimaging studies implicate a distributed network of parieto-fronto-temporal areas in the human left cerebral hemisphere as playing distinct roles in the representation of meaningful actions (praxis). Because these data come primarily from right-handed individuals, the relationship between left cerebral specialization for praxis representation and hand dominance remains unclear. We used functional magnetic resonance imaging (fMRI) to evaluate the hypothesis that strongly left-handed (right hemisphere motor dominant) adults also exhibit this left cerebral specialization. Participants planned familiar actions for subsequent performance with the left or right hand in response to transitive (e.g., "pounding") or intransitive (e.g. "waving") action words. In linguistic control trials, cues denoted non-physical actions (e.g., "believing"). Action planning was associated with significant, exclusively left-lateralized and extensive increases of activity in the supramarginal gyrus (SMg), and more focal modulations in the left caudal middle temporal gyrus (cMTg). This activity was hand- and gesture-independent, i.e., unaffected by the hand involved in subsequent action performance, and the type of gesture (i.e., transitive or intransitive). Compared directly with right-handers, left-handers exhibited greater involvement of the right angular gyrus (ANg) and dorsal premotor cortex (dPMC), which is indicative of a less asymmetric functional architecture for praxis representation. We therefore conclude that the organization of mechanisms involved in planning familiar actions is influenced by one's motor dominance. However, independent of hand dominance, the left SMg and cMTg are specialized for ideomotor transformations-the integration of conceptual knowledge and motor representations into meaningful actions. These findings support the view that higher-order praxis representation and lower-level motor dominance rely on dissociable mechanisms.

Project description:ObjectiveSystemic inflammatory factors have been implicated in symptomatic hand osteoarthritis (OA). Gut microbiome dysbiosis promotes systemic inflammation. The aim of this study was to examine the association between the gut microbiome and the presence of symptomatic hand OA in a population-based study.MethodsStudy participants were subjects of the Xiangya Osteoarthritis Study, a community-based observational study conducted in the Hunan Province of China. Symptomatic hand OA was defined as the presence of both symptoms and radiographic OA in the same hand. The gut microbiome was analyzed using 16S ribosomal RNA gene sequencing in stool samples. We examined the relation of α-diversity, β-diversity, relative abundance of taxa, and potential bacterial functional pathways to symptomatic hand OA.ResultsA total of 1,388 participants (mean age 61.3 years, 57.4% women) were included in the study, of whom 72 had symptomatic hand OA (prevalence of symptomatic hand OA 5.2%). Beta-diversity of the gut microbiome, but not α-diversity, was significantly associated with the presence of symptomatic hand OA (P = 0.003). Higher relative abundance of the genera Bilophila and Desulfovibrio as well as lower relative abundance of the genus Roseburia was associated with symptomatic hand OA. Most functional pathways (i.e., those annotated in the KEGG Ortholog hierarchy) that were observed to be altered in participants with symptomatic hand OA belonged to the amino acid, carbohydrate, and lipid metabolic pathways.ConclusionThis large, population-based study provides the first evidence that alterations in the composition of the gut microbiome were observed among study participants who had symptomatic hand OA, and a low relative abundance of Roseburia but high relative abundance of Bilophila and Desulfovibrio at the genus level were associated with prevalent symptomatic hand OA. These findings may help investigators understand the role of the microbiome in the development of symptomatic hand OA and could contribute to potential translational opportunities.
