Occupational and genetic risk factors for osteoarthritis: a review.

ABSTRACT:

Background

Osteoarthritis (OA) is a multifactorial disease with strong genetic and occupational components. Although published studies have described several risk factors for OA, very few studies have investigated the occupational and genetic factors that contribute to this debilitating condition.

Objective

To describe occupational and genetic factors that may contribute to the risk of developing (OA).

Methods

A literature search was conducted in PubMed using the search terms osteoarthritis, occupation, work, and genetics.

Results

Heavy physical work load was the most common occupational risk factor for OA in several anatomical locations. Other factors include kneeling and regular stair climbing, crawling, bending and whole body vibration, and repetitive movements. Numerous studies have also shown the influence of genetic variability in the pathogenesis of OA. Genetic variants of several groups of genes e.g., cartilage extracellular matrix structural genes and the genes related to bone density have been implicated in disease pathogenesis.

Conclusion

This review shows that occupational factors were extensively studied in knee OA unlike OA of other anatomical regions. Although genetic association studies performed to date identified a number of risk variants, some of these associations have not been consistently replicated across different studies and populations. Therefore, more research is needed.

Project description:BACKGROUND:Meniscal lesions are common and are associated with the development of knee osteoarthritis. Knee-straining activities at work such as kneeling or squatting cause high biomechanical stresses on the knee joints that can lead to acute or chronic injuries. The objective of this systematic review is to update the evidence on the potential relationship between occupational risk factors and meniscal lesions. METHODS:We searched the Medline, Embase and Web of Science databases until August 2021 to identify epidemiological observational studies on the association between occupational risk factors and meniscal lesions. Study selection, data extraction and risk of bias assessment were performed independently by two reviewers. Effect measures were extracted from individual studies and pooled with random effects meta-analysis. Heterogeneity analyses were conducted. We used GRADE (Grades of Recommendations, Assessment, Development and Evaluation) to assess the overall quality of evidence. RESULTS:The database search resulted in 11,006 references, and 46 additional studies were identified through hand search. Twenty-two studies (represented in 25 publications) met the predefined eligibility criteria and nine records were included in the meta-analysis. There was only one study with an overall low risk of bias. Significant associations between occupational risk factors and the development of meniscal lesions were found for kneeling (effect size (ES) 2.15, 95% CI 1.67-2.76), squatting (ES 2.01, 95% CI 1.34-3.03), climbing stairs (ES 2.28, 95% CI 1.58-3.30), lifting and carrying weights ≥ 10 kg (ES 1.63, 95% CI 1.35-1.96), lifting and carrying weights ≥ 25 kg (ES 1.56, 95% CI 1.08-2.24), playing football on a professional level (ES 5.22, 95% CI 3.24-8.41), working as a hard coal miner (ES 5.23, 95% CI 2.16-12.69) and floor layers (ES 1.99, 95% CI 1.43-2.78). The overall quality of evidence according GRADE was moderate to low. CONCLUSION:We found consistent evidence of an increased risk of meniscal lesions by occupational knee-straining exposures. Our findings are important for the development of preventive strategies to reduce work-related knee disorders and work absence. TRIAL REGISTRATION:PROSPERO (registration no. CRD42020196279 ).

Project description:IntroductionGlobally, osteoarthritis (OA) is the most prevalent musculoskeletal disease, affecting approximately 600 million people. It is characterised by progressive degeneration of the articular cartilage, osteophyte formation and asymmetric joint space narrowing, leading to pain, stiffness and functional impairment. The current focus of disease management is on symptom relief and functional improvement. However, these interventions frequently do not provide adequate outcomes. This may be attributed to a lack of consideration for contextual factors, including the presence of comorbidities such as depression. Several studies indicate that depression is highly prevalent, affecting up to 20% of patients with OA. As depression is known to be a multifactorial disorder, there are various risk factors that may increase the likelihood of comorbid depression in patients with OA. Consequently, the scoping review's objective is to map the existing literature on risk factors for comorbid depression in individuals with OA. Furthermore, the review is expected to provide important information for further in-depth investigation and the development of predictive tools to support the early identification of patients with OA at risk of comorbid depression.Methods and analysisThe intended review will target studies reporting risk factors for comorbid depression in individuals with OA. All available primary and grey literature will be considered for inclusion. Only articles published in English or German will be included in the review. The review will follow the format specified by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews and the Joanna Briggs Institute Manual for Evidence Synthesis. Original research in published and unpublished literature from inception until August 2024 will be included in the review. The databases to be searched will include PubMed, EMBASE, PsycInfo and Web of Science. In addition, further literature will be identified by searching the reference lists of the included studies. Three reviewers will independently screen the identified studies for final inclusion. The data will be extracted and presented in tabular form and in a narrative summary that aligns with the review's objective. Furthermore, a quality assessment of the included studies will be conducted using appropriate tools, and the results will be incorporated into the synthesis. Any disagreements that arise between the reviewers will be resolved through discussion or with an additional reviewer.Ethics and disseminationEthics committee approval will not be required because only published and publicly available data will be examined to answer the research question of the scoping review. We will not collect any personal information or any information that requires ethical approval. Following the conclusion of the review, the information will be disseminated to clinicians, health administrators and researchers via publications in peer-reviewed journals and conference presentations.
