Accelerating Improvement and Narrowing Gaps: Trends in Patients' Experiences with Hospital Care Reflected in HCAHPS Public Reporting.

ABSTRACT: Measure HCAHPS improvement in hospitals participating in the second and fifth years of HCAHPS public reporting; determine whether change is greater for some hospital types.Surveys from 4,822,960 adult inpatients discharged July 2007-June 2008 or July 2010-June 2011 from 3,541 U.S. hospitals.Linear mixed-effect regression models with fixed effects for time, patient mix, and hospital characteristics (bedsize, ownership, Census division, teaching status, Critical Access status); random effects for hospitals and hospital-time interactions; fixed-effect interactions of hospital characteristics and patient characteristics (gender, health, education) with time predicted HCAHPS measures correcting for regression-to-the-mean biases.National probability sample of adult inpatients in any of four approved survey modes.HCAHPS scores increased by 2.8 percentage points from 2008 to 2011 in the most positive response category. Among the middle 95 percent of hospitals, changes ranged from a 5.1 percent decrease to a 10.2 percent gain overall. The greatest improvement was in for-profit and larger (200 or more beds) hospitals.Five years after HCAHPS public reporting began, meaningful improvement of patients' hospital care experiences continues, especially among initially low-scoring hospitals, reducing some gaps among hospitals.

Project description:This study presents the experiences of current science, technology, engineering and mathematics (STEM) Ph.D. students and alumni with respect to professional development opportunities in their Ph.D. training. Specifically, it investigates if and how the Ph.D. training supports graduates to pursue non-academic and non-R&D roles, which have become increasingly common career paths post-graduation. A mixed-methods questionnaire was developed to obtain quantitative and qualitative data regarding the graduate school experiences of current Ph.D. students and recent Ph.D. graduates pursuing diverse career paths. The study investigates the values, needs, and conceptions of professional development from the student perspective, as well as the contributions of peers and mentors in graduate school towards their professional development. Experiences of Ph.D. alumni are used to identify the barriers for transitioning to the first job post-graduation and to provide an assessment of the current professional development opportunities in Ph.D. programs. It is reported that although Ph.D. training allowed alumni to develop a robust skillset that includes research, teaching, and scientific writing; some common barriers associated with obtaining a job post-graduation were lack of awareness about career options, limited or no professional networks outside academia, and a lack of preparation and support for non-academic job transitions. Through analyzing the student perspective on various aspects of professional development, the study identifies gaps and avenues for improvement for professional development in Ph.D. training, including increased awareness of diverse career paths for STEM PhDs, increased networking opportunities for PhD students with sectors outside academia, embedding professional development in the PhD curriculum, and others; so that programs can support students in entering the labor market in a variety of careers that extend beyond academia and traditional R&D jobs, using interventions that resonate with the students and meet their needs.

Project description:Purpose:Adjuvant radiation therapy has historically been underused by black patients with breast cancer compared with white patients. We prospectively investigated factors, including sociocultural, psychosocial, and health care factors, that may be associated with the use or omission of adjuvant radiation therapy by both racial groups. Methods and Materials:Women with primary invasive, nonmetastatic breast cancer were recruited from hospitals and through community outreach efforts in the Washington, DC, and Detroit, Michigan, areas between July 2006 and April 2011. Data were collected via telephone interviews regarding psychosocial (eg, self-efficacy) and health care factors (eg, communication) at the time they received a diagnosis. Clinical data were extracted from their medical charts after the completion of treatment. We examined the association among multiple demographic, socio-cultural, healthcare process factors and the use of radiotherapy. Logistic multivariable regression models identified associations with radiotherapy receipt. Results:Among 395 eligible and consenting women, 315 had complete baseline data, and 217 were in the final analytical sample, having met criteria for adjuvant breast or chest wall radiation therapy after breast conservation surgery or mastectomy. Among women eligible for radiation, all were insured, 59% were black, the mean age was 55.4 years, and the majority had stage I or II disease. Overall, approximately 70% percent of women received adjuvant radiation therapy. On multivariable analyses, the likelihood of receiving adjuvant radiation therapy was higher for those who were black with any level of indication for radiation therapy (odds ratio 2.21; P < .01), those for whom comorbidities were present, and those who demonstrated positive sociocultural factors such as self-efficacy and high reported rates of provider communication about radiation therapy (odds ratio 1.20; P < .05). Among women with strong indications for radiation therapy, there was no significant association with race on multivariable analysis. Conclusions:Our findings suggest that among women with any indication for radiation therapy, black patients were more likely to receive radiation therapy compared with white patients. Furthermore, data suggest improved provider communication and self-efficacy are important predictors of receipt of radiation therapy. Further studies exploring the effects of provider communication and sociocultural factors to diverse patient populations may be warranted.
