Situation Change: Stability and Change of Situation Variables between and within Persons.

ABSTRACT: When, how, and why situations flow into one another is important for understanding dynamic personality processes, but the topic of situation change has traditionally been a thorny issue in personality/social psychology. We explore conceptual and methodological issues in research on situation change: (1) What is situation change, which variables could we measure, and how can situation change be methodologically captured and analyzed (at between- and within-person levels)? (2) Which person-situation transaction mechanisms (situation management strategies) could entail stability and change of situations in daily life? (3) How do single or repeated instances of situation change impact short-, middle-, and long-term outcomes (e.g., intra- and interpersonal adjustment)? Besides laying out a research program for situation change, we present preliminary data from participants who wore mini-video cameras recording their situations so that they could be rated later in the lab. We demonstrate rater consensus on when situations change, mean-level changes of situation characteristics across situations, similarity of situation characteristics across adjacent situations, and inter-individual differences in intra-individual situation change in change networks.

Project description:BackgroundIn recent years, shared decision making (SDM) has been promoted as a model to guide interactions between persons with MS and their neurologists to reach mutually satisfying decisions about disease management - generally about deciding treatment courses of prevailing disease modifying therapies. In 2009, Dr. Paolo Zamboni introduced the world to his hypothesis of Chronic Cerebrospinal Venous Insufficiency (CCSVI) as a cause of MS and proposed venous angioplasty ('liberation therapy') as a potential therapy. This study explores the discussions that took place between persons with MS (PwMS) and their neurologists about CCSVI against the backdrop of the recent calls for the use of SDM to guide clinical conversations.MethodsIn 2012, study researchers conducted focus groups with PwMS (n = 69) in Winnipeg, Canada. Interviews with key informants were also carried out with 15 participants across Canada who were stakeholders in the MS community: advocacy organizations, MS clinicians (i.e. neurologists, nurses), clinical researchers, and government health policy makers.ResultsPwMS reported a variety of experiences when attempting to discuss CCSVI with their neurologist. Some found that there was little effort to engage in desired discussions or were dissatisfied with critical or cautious stances of their neurologist. This led to communication breakdowns, broken relationships, and decisions to autonomously access alternative opinions or liberation therapy. Other participants were appreciative when clinicians engaged them in discussions and were more receptive to more critical appraisals of the evidence. Key informants reported that they too had heard of neurologists who refused to discuss CCSVI with patients and that neurology as a whole had been particularly vilified for their response to the hypothesis. Clinicians indicated that they had shared information as best they could but recommended against seeking liberation therapy. They noted that being respectful of patient emotions, values, and hope were also key to maintaining good relationships.ConclusionsWhile CCSVI proved a challenging context to carry out patient-physician discussions and brought numerous tensions to the surface, following the approach of SDM can minimize the potential for unfortunate outcomes as much as possible because it is based on principles of respect and more two-way communication.

Project description:Malaria eradication involves eliminating malaria from every country where transmission occurs. Current theory suggests that the post-elimination challenges of remaining malaria-free by stopping transmission from imported malaria will have onerous operational and financial requirements. Although resurgent malaria has occurred in a majority of countries that tried but failed to eliminate malaria, a review of resurgence in countries that successfully eliminated finds only four such failures out of 50 successful programmes. Data documenting malaria importation and onwards transmission in these countries suggests malaria transmission potential has declined by more than 50-fold (i.e. more than 98%) since before elimination. These outcomes suggest that elimination is a surprisingly stable state. Elimination's 'stickiness' must be explained either by eliminating countries starting off qualitatively different from non-eliminating countries or becoming different once elimination was achieved. Countries that successfully eliminated were wealthier and had lower baseline endemicity than those that were unsuccessful, but our analysis shows that those same variables were at best incomplete predictors of the patterns of resurgence. Stability is reinforced by the loss of immunity to disease and by the health system's increasing capacity to control malaria transmission after elimination through routine treatment of cases with antimalarial drugs supplemented by malaria outbreak control. Human travel patterns reinforce these patterns; as malaria recedes, fewer people carry malaria from remote endemic areas to remote areas where transmission potential remains high. Establishment of an international resource with backup capacity to control large outbreaks can make elimination stickier, increase the incentives for countries to eliminate, and ensure steady progress towards global eradication. Although available evidence supports malaria elimination's stickiness at moderate-to-low transmission in areas with well-developed health systems, it is not yet clear if such patterns will hold in all areas. The sticky endpoint changes the projected costs of maintaining elimination and makes it substantially more attractive for countries acting alone, and it makes spatially progressive elimination a sensible strategy for a malaria eradication endgame.

Project description:This study aims to determine the approximate number of hospitalizations of persons without family and the medical challenges they encounter in hospitals across Japan. Self-administered questionnaires were mailed to 4,000 randomly selected hospitals nationwide to investigate the actual conditions and problems, decision-making processes, and use of the government-recommended Guidelines for the hospitalization of, and decision-making support for, persons without family. To identify the characteristics of each region and role of hospitals, chi-square tests were used to make separate group comparisons by hospital location and type. Responses were received from 1,271 hospitals (31.2% response rate), of which 952 hospitals provided information regarding the number of admissions of persons without family. The mean (SD) and median number of hospitalizations (approximate number per year) of patients without family was 16 (79) and 5, respectively. Approximately 70% of the target hospitals had experienced the hospitalization of a person without family, and 30% of the hospitals did not. The most common difficulties encountered during the hospitalization were collecting emergency contact information, decision-making related to medical care, and discharge support. In the absence of family members and surrogates, the medical team undertook the decision-making process, which was commonly performed according to manuals and guidelines and by consulting an ethics committee. Regarding the use of the government-recommended Guidelines, approximately 70% of the hospitals that were aware of these Guidelines responded that they had never taken any action based on these Guidelines, with significant differences by region and hospital type. To solve the problems related to the hospitalization of persons without family, the public should be made aware of these Guidelines, and measures should be undertaken to make clinical ethics consultation a sustainable activity within hospitals.

Project description:ObjectivesPrevious studies found a general increase in prejudice against Chinese people during the first months of the pandemic. The present study aims to consider inter-individual heterogeneity in stability and change regarding prejudice involving Chinese people during the pandemic. The first objective is to identify and describe different trajectories of prejudice over a seven-month period during the pandemic. The second and third objectives are to test the association between trajectory group membership and antecedent variables such as: socio-demographic factors (i.e., age, gender, political affiliation) and two psychological mechanisms, namely economic threat and global citizenship identification.MethodsA representative Canadian sample (N = 3,617) according to age, gender and province of residence, was recruited for a 10-wave survey starting from April 2020 to December 2020. First, a group-based modeling approach was used to identify trajectories of prejudice. Second, a multinomial logistic regression model was used to test associations between membership in trajectories and antecedents.ResultsFour trajectories were identified. The first three trajectories have a low (71.4% of the sample), high (18.5%) or very high (5.3%) level of prejudice against Chinese people which is relatively stable over time. The fourth trajectory (4.9%) reports low levels of prejudice in favor of Chinese people which become more positive throughout 2020. Regarding socio-demographic factors: gender is not associated with trajectory group membership, younger people are more likely to follow the trajectory in favor of Chinese people and conservatives are more likely to follow the highest trajectories against Chinese people. Regarding some psychological mechanisms: personal but not collective economic threat is associated with the trajectory in favor of Chinese people. Finally, the highest levels of prejudice are found when the strategy of identification is more local rather than global.ConclusionThe present study shows that Canadians differ in terms of both their level and change in prejudice against Chinese people throughout the pandemic with some socio-demographic groups being more likely than others to be associated with prejudice. The results also suggest that a promising way to tackle the major social issue of prejudice is to highlight a vision of the world where individuals are all "global citizens" facing the same challenge.
