ABSTRACT: We provide a nonparametric estimate of ?-restricted mean survival using follow-up information beyond ?when appropriate to improve precision. The variance accounts for correlation between follow-up windows. Both asymptotic calculations and simulation studies recommend follow-up intervals spaced approximately ?/2 apart.

Ultra-long-term follow-up of pediatric spinal deformity problems: 23 patients with a mean follow-up of 51 years.

Project description:BACKGROUND: The aim of this study was to analyze the true outcomes of a unique cohort of patients with spinal deformities who were treated as children and followed for 40 or more years. METHODS: Altogether, 23 patients were reviewed who had been originally treated in our community, whose original charts and radiographs were still available, and who had undergone recent evaluation. RESULTS: The diagnoses were congenital deformity in eight, adolescent idiopathic scoliosis in four, poliomyelitis in three, infantile idiopathic scoliosis in two, spondylolisthesis in two, and one each of tuberculosis and dwarfism. Sixteen had undergone fusion surgery. CONCLUSIONS: Early spine fusion for deformity produced far better results than delayed fusion. A solid fusion at the end of growth remained unchanged. Degenerative changes outside the fusion area were rare and seldom required further surgery. In summary, 23 patients with a mean follow-up of 51 years after treatment are presented. Early fusion was far superior to delayed or nonsurgical treatment.

Bias in Mean Survival From Fitting Cure Models With Limited Follow-Up.

Project description:ObjectivesWhen populations contain mixtures of cured and uncured patients, the use of traditional parametric approaches to estimate overall survival (OS) can be biased. Mixture cure models may reduce bias compared with traditional parametric models, but their accuracy is subject to certain conditions. Importantly, mixture cure models assume that that there is enough follow-up to identify individuals censored at the end of the follow-up period as cured. The purpose of this article is to describe biases that can occur when mixture cure models are used to estimate mean survival from data with limited follow-up.MethodsWe analyzed 6 trials conducted by the SWOG Cancer Research Network Leukemia Committee. For each trial, we analyzed 2 data sets: the data released to the committee when the results of the trial were unblinded and a second data set with additional follow-up. We estimated mean OS using parametric survival models with and without a cure fraction.ResultsWhen using mixture cure models, in 4 trials, estimates of mean OS were higher with the first analysis (with limited follow-up) compared with estimates from data with longer follow-up. In 1 trial, the reverse pattern was observed. In 1 trial, the cure estimate changed little with additional follow-up.ConclusionsCaution should be taken when using mixture cure models in scenarios with limited follow-up. The biases resulting from fitting these models may be exacerbated when the models are being used to extrapolate OS and estimate mean OS.

Analysis of restricted mean survival time for length-biased data.

Project description:In clinical studies with time-to-event outcomes, the restricted mean survival time (RMST) has attracted substantial attention as a summary measurement for its straightforward clinical interpretation. When the data are subject to length-biased sampling, which is frequently encountered in observational cohort studies, existing methods to estimate the RMST are not applicable. In this article, we consider nonparametric and semiparametric regression methods to estimate the RMST under the setting of length-biased sampling. To assess the covariate effects on the RMST, a semiparametric regression model that directly relates the covariates and the RMST is assumed. Based on the model, we develop unbiased estimating equations to obtain consistent estimators of covariate effects by properly adjusting for informative censoring and length bias. Stochastic process theories are used to establish the asymptotic properties of the proposed estimators. We investigate the finite sample performance through simulations and illustrate the methods by analyzing a prevalent cohort study of dementia in Canada.

On the restricted mean survival time curve in survival analysis.

Project description:For a study with an event time as the endpoint, its survival function contains all the information regarding the temporal, stochastic profile of this outcome variable. The survival probability at a specific time point, say t, however, does not transparently capture the temporal profile of this endpoint up to t. An alternative is to use the restricted mean survival time (RMST) at time t to summarize the profile. The RMST is the mean survival time of all subjects in the study population followed up to t, and is simply the area under the survival curve up to t. The advantages of using such a quantification over the survival rate have been discussed in the setting of a fixed-time analysis. In this article, we generalize this approach by considering a curve based on the RMST over time as an alternative summary to the survival function. Inference, for instance, based on simultaneous confidence bands for a single RMST curve and also the difference between two RMST curves are proposed. The latter is informative for evaluating two groups under an equivalence or noninferiority setting, and quantifies the difference of two groups in a time scale. The proposal is illustrated with the data from two clinical trials, one from oncology and the other from cardiology.

A High-Dimensional Nonparametric Multivariate Test for Mean Vector.

Project description:This work is concerned with testing the population mean vector of nonnormal high-dimensional multivariate data. Several tests for high-dimensional mean vector, based on modifying the classical Hotelling T2 test, have been proposed in the literature. Despite their usefulness, they tend to have unsatisfactory power performance for heavy-tailed multivariate data, which frequently arise in genomics and quantitative finance. This paper proposes a novel high-dimensional nonparametric test for the population mean vector for a general class of multivariate distributions. With the aid of new tools in modern probability theory, we proved that the limiting null distribution of the proposed test is normal under mild conditions when p is substantially larger than n. We further study the local power of the proposed test and compare its relative efficiency with a modified Hotelling T2 test for high-dimensional data. An interesting finding is that the newly proposed test can have even more substantial power gain with large p than the traditional nonparametric multivariate test does with finite fixed p. We study the finite sample performance of the proposed test via Monte Carlo simulations. We further illustrate its application by an empirical analysis of a genomics data set.

What Does Depression Mean for Korean American Elderly?: A Qualitative Follow-Up Study.

Project description:ObjectiveKorean American Elderly (KAE) have high rates of depression but underuse mental health services. The purpose of this study was to assess the meaning of depression and help seeking among KAE residing in the United States who have clinically significant depressive symptoms.MethodsAs a follow up to the Memory and Aging Study of Koreans (MASK; n=1,118), a descriptive epidemiological study which showed that only one in four of KAE with clinically significant depressive symptoms (Patient Health Questionnaire-9?10) used mental health services, we conducted a qualitative study using semi-structured interviews with participants with clinically significant depressive symptoms regarding the meaning of depression and beliefs about help seeking. Ten participants with clinically significant depressive symptoms were approached and 8 were recruited for semi-structured interviews.ResultsKAE did not identify themselves as depressed though experiencing clinically significant depressive symptoms. They associated depression with social discrimination, social isolation, and suicide in the extreme circumstance. They attributed depression to not achieving social and material success in America and strained relationships with their children. Participants attempted to self-manage distress without telling others in their social network. However, KAE were willing to consult with mental health professionals if the services were bilingual, affordable, and confidential.ConclusionKAE with clinically significant depressive symptoms are a vulnerable group with need and desire for linguistically and culturally relevant mental health services who are isolated due to a complex array of psychological and social factors.
