In-depth performance analysis of an EEG based neonatal seizure detection algorithm.

ABSTRACT: To describe a novel neurophysiology based performance analysis of automated seizure detection algorithms for neonatal EEG to characterize features of detected and non-detected seizures and causes of false detections to identify areas for algorithmic improvement.EEGs of 20 term neonates were recorded (10 seizure, 10 non-seizure). Seizures were annotated by an expert and characterized using a novel set of 10 criteria. ANSeR seizure detection algorithm (SDA) seizure annotations were compared to the expert to derive detected and non-detected seizures at three SDA sensitivity thresholds. Differences in seizure characteristics between groups were compared using univariate and multivariate analysis. False detections were characterized.The expert detected 421 seizures. The SDA at thresholds 0.4, 0.5, 0.6 detected 60%, 54% and 45% of seizures. At all thresholds, multivariate analyses demonstrated that the odds of detecting seizure increased with 4 criteria: seizure amplitude, duration, rhythmicity and number of EEG channels involved at seizure peak. Major causes of false detections included respiration and sweat artefacts or a highly rhythmic background, often during intermediate sleep.This rigorous analysis allows estimation of how key seizure features are exploited by SDAs.This study resulted in a beta version of ANSeR with significantly improved performance.

Project description:OBJECTIVE:The purpose of this study was to evaluate and validate an offline, automated scalp EEG-based seizure detection system and to compare its performance to commercially available seizure detection software. METHODS:The test seizure detection system, IdentEvent™, was developed to enhance the efficiency of post-hoc long-term EEG review in epilepsy monitoring units. It translates multi-channel scalp EEG signals into multiple EEG descriptors and recognizes ictal EEG patterns. Detection criteria and thresholds were optimized in 47 long-term scalp EEG recordings selected for training (47 subjects, ?3653h with 141 seizures). The detection performance of IdentEvent was evaluated using a separate test dataset consisting of 436 EEG segments obtained from 55 subjects (?1200h with 146 seizures). Each of the test EEG segments was reviewed by three independent epileptologists and the presence or absence of seizures in each epoch was determined by majority rule. Seizure detection sensitivity and false detection rate were calculated for IdentEvent as well as for the comparable detection software (Persyst's Reveal®, version 2008.03.13, with three parameter settings). Bootstrap re-sampling was applied to establish the 95% confidence intervals of the estimates and for the performance comparison between two detection algorithms. RESULTS:The overall detection sensitivity of IdentEvent was 79.5% with a false detection rate (FDR) of 2 per 24h, whereas the comparison system had 80.8%, 76%, and 74% sensitivity using its three detection thresholds (perception score) with FDRs of 13, 8, and 6 per 24h, respectively. Bootstrap 95% confidence intervals of the performance difference revealed that the two detection systems had comparable detection sensitivity, but IdentEvent generated a significantly (p<0.05) smaller FDR. CONCLUSIONS:The study validates the performance of the IdentEvent™ seizure detection system. SIGNIFICANCE:With comparable detection sensitivity, an improved false detection rate makes the automated seizure detection software more useful in clinical practice.

Project description:Seizure prediction might be the solution to tackle the apparent unpredictability of seizures in patients with drug-resistant epilepsy, which comprise about a third of all patients with epilepsy. Designing seizure prediction models involves defining the pre-ictal period, a transition stage between inter-ictal brain activity and the seizure discharge. This period is typically a fixed interval, with some recent studies reporting the evaluation of different patient-specific pre-ictal intervals. Recently, researchers have aimed to determine the pre-ictal period, a transition stage between regular brain activity and a seizure. Authors have been using deep learning models given the ability of such models to automatically perform pre-processing, feature extraction, classification, and handling temporal and spatial dependencies. As these approaches create black-box models, clinicians may not have sufficient trust to use them in high-stake decisions. By considering these problems, we developed an evolutionary seizure prediction model that identifies the best set of features while automatically searching for the pre-ictal period and accounting for patient comfort. This methodology provides patient-specific interpretable insights, which might contribute to a better understanding of seizure generation processes and explain the algorithm's decisions. We tested our methodology on 238 seizures and 3687 h of continuous data, recorded on scalp recordings from 93 patients with several types of focal and generalised epilepsies. We compared the results with a seizure surrogate predictor and obtained a performance above chance for 32% patients. We also compared our results with a control method based on the standard machine learning pipeline (pre-processing, feature extraction, classifier training, and post-processing), where the control marginally outperformed our approach by validating 35% of the patients. In total, 54 patients performed above chance for at least one method: our methodology or the control one. Of these 54 patients, 21 ([Formula: see text]38%) were solely validated by our methodology, while 24 ([Formula: see text]44%) were only validated by the control method. These findings may evidence the need for different methodologies concerning different patients.
