Plague in Arab Maghreb, 1940-2015: A Review.

ABSTRACT: We reviewed the epidemiology of 49 plague outbreaks that resulted in about 7,612 cases in 30 localities in the Arabic Maghreb (Mauritania, Morocco, Algeria, Tunisia, Libya, and Egypt) over 75?years. Between 1940 and 1950, most cases recorded in Morocco (75%) and Egypt (20%), resulted from plague imported to Mediterranean harbors and transmitted by rat ectoparasites. By contrast, the re-emergence of plague in the southern part of Western Sahara in 1953 and in northeast Libya in 1976 was traced to direct contact between nomadic populations and infected goats and camels in natural foci, including the consumption of contaminated meat, illustrating this neglected oral route of contamination. Further familial outbreaks were traced to human ectoparasite transmission. Efforts to identify the factors contributing to natural foci may guide where to focus the surveillance of sentinel animals in order to eradicate human plague, if not Yersinia pestis from the Arab Maghreb.

Coccidioidomycosis Outbreaks, United States and Worldwide, 1940-2015.

Project description:Coccidioidomycosis causes substantial illness and death in the United States each year. Although most cases are sporadic, outbreaks provide insight into the clinical and environmental features of coccidioidomycosis, high-risk activities, and the geographic range of Coccidioides fungi. We identified reports published in English of 47 coccidioidomycosis outbreaks worldwide that resulted in 1,464 cases during 1940-2015. Most (85%) outbreaks were associated with environmental exposures; the 2 largest outbreaks resulted from an earthquake and a large dust storm. More than one third of outbreaks occurred in areas where the fungus was not previously known to be endemic, and more than half of outbreaks involved occupational exposures. Coccidioidomycosis outbreaks can be difficult to detect and challenging to prevent given the unknown effectiveness of environmental control methods and personal protective equipment; therefore, increased awareness of coccidioidomycosis outbreaks is needed among public health professionals, healthcare providers, and the public.

Pneumonic Plague Transmission, Moramanga, Madagascar, 2015.

Project description:During a pneumonic plague outbreak in Moramanga, Madagascar, we identified 4 confirmed, 1 presumptive, and 9 suspected plague case-patients. Human-to-human transmission among close contacts was high (reproductive number 1.44) and the case fatality rate was 71%. Phylogenetic analysis showed that the Yersinia pestis isolates belonged to group q3, different from the previous outbreak.

Gridded land use data for the conterminous United States 1940-2015.

Project description:Multiple aspects of our society are reflected in how we have transformed land through time. However, limited availability of historical-spatial data at fine granularity have hindered our ability to advance our understanding of the ways in which land was developed over the long-term. Using a proprietary, national housing and property database, which is a result of large-scale, industry-fuelled data harmonization efforts, we created publicly available sequences of gridded surfaces that describe built land use progression in the conterminous United States at fine spatial (i.e., 250 m × 250 m) and temporal resolution (i.e., 1 year - 5 years) between the years 1940 and 2015. There are six land use classes represented in the data product: agricultural, commercial, industrial, residential-owned, residential-income, and recreational facilities, as well as complimentary uncertainty layers informing the users about quantifiable components of data uncertainty. The datasets are part of the Historical Settlement Data Compilation for the U.S. (HISDAC-US) and enable the creation of new knowledge of long-term land use dynamics, opening novel avenues of inquiry across multiple fields of study.

Human response to live plague vaccine EV, Almaty region, Kazakhstan, 2014-2015.

Project description:BackgroundIn Kazakhstan, a live plague vaccine EV 76 NIIEG has been used for plague prophylaxis since the mid-1930s. Vaccination is administered yearly among people living in plague-enzootic areas. Similar practices are used in other former Soviet Union countries. Yet, to this day, the effectiveness period of the vaccine is unknown. It is also not clear how different factors can affect the effectiveness of the vaccine over time.MethodsWe surveyed changes in antibody levels specific for F1 antigens of Yersinia pestis among vaccinated people 4, 8, and 12 months post- vaccination. Blood samples were taken from the participants of the study for producing sera, which was later analyzed using indirect hemagglutination reaction with antigenic erythrocyte assay (micromethod) for identifying antibodies to F1 Y.pestis.ResultsIn first-time-receivers of the plague vaccine, antibody titer reached the highest level of antibody that represents a conditionally protective titer after 4 months, dropped drastically after 8 months, and dropped again after 12 months. Similar results were obtained among those who have been vaccinated previously. However, in that group, the percentage of people with a level of antibody that represents a conditionally protective titer remained statistically significant even after 8 and 12 months.ConclusionBased on the results of this study, we recommend initiating vaccination campaigns for the medical and veterinary staff, as well as the general population four months prior to the springtime epizootics of plague among wild rodents.

Plague Epizootic Dynamics in Chipmunk Fleas, Sierra Nevada Mountains, California, USA, 2013-2015.

Project description:We describe Yersinia pestis minimum infection prevalence in fleas collected from Tamias spp. chipmunks in the Sierra Nevadas (California, USA) during 2013-2015. Y. pestis-positive fleas were detected only in 2015 (year of plague epizootic), mostly in T. speciosus chipmunks at high-elevation sites. Plague surveillance should include testing vectors for Y. pestis.

Soil-transmitted Helminthiasis in the United States: a systematic review--1940-2010.

Project description:The epidemiology of soil-transmitted helminth infections (hookworm, Trichuris trichiura, Ascaris lumbricoides, and Strongyloides stercoralis) in the United States is poorly understood. To gain understanding of the status of disease, a systematic review was performed to assess the prevalence of soil-transmitted helminth infections in the United States. Of all studies reviewed, 14 were designated as high-quality. High-quality studies were published from 1942 to 1982 and showed that infection was prevalent throughout the southern United States and Appalachia as recently as 1982, finding that hookworm (19.6%), T. trichiura (55.2%), A. lumbricoides (49.4%), and S. stercoralis (3.8%) affected significant percentages of the population. However, because the most recent high-quality studies were published over 25 years ago, the literature does not provide sufficient data to assess current endemic transmission. Because the status of disease remains unclear, there is a need for additional studies to determine if soil-transmitted helminths remain endemic in the United States.

The epidemiology of hepatitis C virus in the Maghreb region: systematic review and meta-analyses.

Project description:ObjectiveTo systematically review and synthesize available epidemiological data on hepatitis C virus (HCV) prevalence and incidence in the Maghreb region and to estimate the country-specific population-level HCV prevalence.MethodsWe conducted a systematic review of HCV antibody prevalence and incidence in the Maghreb countries as outlined by the PRISMA guidelines. Meta-analyses were conducted using DerSimonian-Laird random-effect models with inverse variance weighting to pool HCV prevalence estimates among general population groups.ResultsWe identified 133 HCV prevalence measures and two HCV incidence measures. Among high risk groups, HCV prevalence ranged between 22% and 94% among people who inject drugs, 20% and 76% among dialysis patients, and 2% and 51% among hemophiliacs. Among intermediate-risk groups, considerable but widely variable HCV prevalence was found. Most common risk factors cited across studies were the duration of dialysis, number of transfusions, and having a history of surgery or dental work. The national HCV prevalence in Algeria was estimated at 0.3% (95%CI: 0.1-0.5), Libya 1.2% (95%CI: 1.1-1.3), Mauritania 1.1% (95%CI: 0-2.3), Morocco 0.8% (95%CI: 0.5-1.2), and Tunisia 0.6% (95%CI: 0.5-0.8).ConclusionsHCV prevalence in the Maghreb region of the Middle East and North Africa is comparable to that in developed countries of about 1%. HCV exposures appear often to be linked to medical care and are suggestive of ongoing transmission in such settings. Injecting drug use appears also to be a major, though not dominant, contributor to HCV transmission. Further research is needed to draw a more thorough understanding of HCV epidemiology, especially in the countries with limited number of studies. HCV prevention policy and programming in these countries should focus on the settings of exposure.

Project description:Berlin neurologist and neurohistologist Max Bielschowsky counts among the most innovative microanatomical researchers at the beginning of the twentieth century. Although being quite underrated in the history of neurology today, Bielschowsky contributed substantially to the understanding of neurohereditary pathologies, such as Alzheimer's disease, Parkinsonism, and Huntington's chorea, as well as the assessment of structural changes in several movement disorders. Working with other leading research neurologists, such as Oskar and Cecile Vogt or Korbinian Brodmann at the newly founded Kaiser Wilhelm Institute for Brain Research in Berlin-Buch, he also pioneered neurohistological work on de- and regeneration processes in the Central Nervous System along with new morphological definitions of "nervous trauma."
