Financial strain, trajectories of marital processes, and African American newlyweds' marital instability.

ABSTRACT: The present study examined the longitudinal associations among financial strain, trajectories of marital processes, and increases in marital instability concerns among a sample of 280 African American newlywed couples followed over the first 3 years of marriage. Results from dyadic structural equation modeling revealed that financial strain experienced during the early years of marriage was associated with increased marital instability concerns for both husbands and wives. Latent growth curves of marital processes revealed mean declines in appraisals of spousal warmth and increases in appraisals of spousal hostility, with variability between individuals in rates of decline in warmth; further, wives' appraisals of spousal warmth covaried with levels of financial strain, such that high levels of financial strain were associated with steeper declines in spousal warmth appraisals. For both husbands and wives, rates of change in spousal warmth appraisals had a greater influence on increases in marital instability concerns than either starting levels of spousal warmth appraisals or financial strain. Project description:BACKGROUND:Compared with white persons, African Americans have a greater incidence of diabetes, decreased control, and higher rates of microvascular complications. A peer mentorship model could be a scalable approach to improving control in this population and reducing disparities in diabetic outcomes. OBJECTIVE:To determine whether peer mentors or financial incentives are superior to usual care in helping African American veterans decrease their hemoglobin A(1c) (HbA(1c)) levels. DESIGN:A 6-month randomized, controlled trial. (ClinicalTrials.gov registration number: NCT01125956) SETTING:Philadelphia Veterans Affairs Medical Center. PATIENTS:African American veterans aged 50 to 70 years with persistently poor diabetes control. INTERVENTION:118 patients were randomly assigned to 1 of 3 groups: usual care, a peer mentoring group, and a financial incentives group. Usual care patients were notified of their starting HbA(1c) level and recommended goals for HbA(1c). Those in the peer mentoring group were assigned a mentor who formerly had poor glycemic control but now had good control (HbA(1c) level ?7.5%). The mentor was asked to talk with the patient at least once per week. Peer mentors were matched by race, sex, and age. Patients in the financial incentive group could earn $100 by decreasing their HbA(1c) level by 1% and $200 by decreasing it by 2% or to an HbA(1c) level of 6.5%. MEASUREMENTS:Change in HbA(1c) level at 6 months. RESULTS:Mentors and mentees talked the most in the first month (mean calls, 4; range, 0 to 30), but calls decreased to a mean of 2 calls (range, 0 to 10) by the sixth month. Levels of HbA(1c) decreased from 9.9% to 9.8% in the control group, from 9.8% to 8.7% in the peer mentor group, and from 9.5% to 9.1% in the financial incentive group. Mean change in HbA(1c) level from baseline to 6 months relative to control was -1.07% (95% CI, -1.84% to -0.31%) in the peer mentor group and -0.45% (CI, -1.23% to 0.32%) in the financial incentive group. LIMITATION:The study included only veterans and lasted only 6 months. CONCLUSION:Peer mentorship improved glucose control in a cohort of African American veterans with diabetes. Project description:Genomic tools and analyses are now being widely used to understand genome-wide patterns and processes associated with speciation and adaptation. In this article, we apply a genomics approach to the model organism Drosophila melanogaster. This species originated in Africa and subsequently spread and adapted to temperate environments of Eurasia and the New World, leading some populations to evolve reproductive isolation, especially between cosmopolitan and Zimbabwean populations. We used tiling arrays to identify highly differentiated regions within and between North America (the United States and Caribbean) and Africa (Cameroon and Zimbabwe) across 63% of the D. melanogaster genome and then sequenced representative fragments to study their genetic divergence. Consistent with previous findings, our results showed that most differentiation was between populations living in Africa vs. outside of Africa (i.e., "out-of-Africa" divergence), with all other geographic differences being less substantial (e.g., between cosmopolitan and Zimbabwean races). The X chromosome was much more strongly differentiated than the autosomes between North American and African populations (i.e., greater X divergence). Overall differentiation was positively associated with recombination rates across chromosomes, with a sharp reduction in regions near centromeres. Fragments surrounding these high F(ST) sites showed reduced haplotype diversity and increased frequency of rare and derived alleles in North American populations compared to African populations. Nevertheless, despite sharp deviation from neutrality in North American strains, a small set of bottleneck/expansion demographic models was consistent with patterns of variation at the majority of our high F(ST) fragments. Although North American populations were more genetically variable compared to Europe, our simulation results were generally consistent with those previously based on European samples. These findings support the hypothesis that most differentiation between North America and Africa was likely driven by the sorting of African standing genetic variation into the New World via Europe. Finally, a few exceptional loci were identified, highlighting the need to use an appropriate demographic null model to identify possible cases of selective sweeps in species with complex demographic histories.
