The Meaning of Weight: A Big Way of Life for Rural Mexican-American Adolescent Men and Caregivers.

ABSTRACT: To describe the meaning of body weight among rural Mexican-American adolescents (RMAA) and an influential person (IP) of their social network.This is qualitative descriptive study. Convenience sampling was used to recruit RMAA seeking care at a rural primary care-based clinic. Content analysis was used to identify categories and subcategories. Fifteen male adolescents and 14 IPs were interviewed (n = 29).Individual interviews were conducted using a semi-structured questionnaire.Participants described body weight through family norms, body size, and history of heart disease or diabetes. Cultural scripts were used to describe weight gain, judgment of size, and resource limitations.Health care providers must evaluate cultural scripts associated with body size to develop interventions that are derived from the meaning ascribed to weight status rather than simply based on evidence-based standards. Collective Care: Multiple Caregivers and Multiple Care Recipients in Mexican American Families.

Project description:Specific stressors associated with caregiving in Mexican American (MA) families are not well documented, yet caregiving issues are paramount because informal care for parents is central to their culture. Although MA families who band together to provide care for one member are not unique, the literature does not describe the phenomenon of collective caregiving, which may be widespread but unrecognized. This article describes these understudied families who are poorly served by contemporary health systems because their characteristics are unknown.Descriptive, multisite, longitudinal mixed-methods study of MA caregiving families.We identified three types of collective caregivers: those providing care for multiple family members simultaneously, those providing care successively to several family members, and/or those needing care themselves during their caregiving of others.Collective caregiving of MA elders warrants further investigation.Exploration of collective caregiving may provide a foundation for tailored family interventions.

The meaning of numbers in health: exploring health numeracy in a Mexican-American population.

Project description:BackgroundHealth numeracy can be defined as the ability to use numeric information in the context of health. The interpretation and application of numbers in health may vary across cultural groups.ObjectiveTo explore the construct of health numeracy among persons who identify as Mexican American.DesignQualitative focus group study. Groups were stratified by preferred language and level of education. Audio-recordings were transcribed and Spanish groups (n = 3) translated to English. An analysis was conducted using principles of grounded theory.ParticipantsA purposeful sample of participants from clinical and community sites in the Milwaukee and Chicago metropolitan areas.Main measuresA theoretical framework of health numeracy was developed based upon categories and major themes that emerged from the analysis.Key resultsSix focus groups were conducted with 50 participants. Initial agreement in coding was 59-67% with 100% reached after reconciliation by the coding team. Three major themes emerged: 1) numeracy skills are applied to a broad range of communication and decision making tasks in health, 2) affective and cognitive responses to numeric information influence use of numbers in the health setting, and 3) there exists a strong desire to understand the meaning behind numbers used in health. The findings informed a theoretical framework of health numeracy.ConclusionsNumbers are important across a range of skills and applications in health in a sample of an urban Mexican-American population. This study expands previous work that strives to understand the application of numeric skills to medical decision making and health behaviors.

Plasma MicroRNA signature predicting weight gain among Mexican-American women.

Project description:ObjectiveThere is growing evidence that circulating microRNAs (miRNAs) play an important role in obesity. However, whether they can contribute to adult weight gain is still unclear.MethodsIn the training set with 40 nonsmoking, healthy women identified from the Mano-A-Mano Mexican American Cohort study, global circulating miRNA profiles in plasma samples were assessed. Cox proportional hazard regression was used to assess the effects of plasma miRNAs on significant weight gain during a 5-year follow-up. Plasma miRNAs associated with significant weight gain were further validated in two testing sets (N?=?160 and 100, respectively).ResultsA total of 23 significant plasma miRNAs were identified in the training set. Among them, eight were validated in two testing sets. They were miR-142, miR-122, miR-125b, miR-15b, miR-130b, miR222, miR-519d, and miR-31. Using those eight miRNAs, a risk score for significant weight gain was created. Study participants with a high risk score had 3.01-fold increased risk of having significant weight gain in the whole study population (hazard ratio: 3.01, 95% confidence interval: 1.70-5.47).ConclusionsThe findings provide evidence that circulating miRNAs play important roles in obesity and weight gain and suggest new targets for understanding the mechanisms of weight gain and developing weight loss intervention strategies.

Very low levels of energy expenditure among pre-adolescent Mexican-American girls.

Project description:We assessed activity energy expenditure (AEE) in Mexican-American (MA) and European-American (EA) children. Total energy expenditure (TEE) was measured using the doubly-labeled water method; AEE was calculated as the difference between TEE and resting EE (REE), and physical activity level (PAL) was calculated as TEE/REE. Groups were comparable for age, sex and body mass index (BMI). REE did not differ between groups. The boys did not differ in TEE, AEE, or PAL (MA vs. EA, respectively: TEE, 7.9+/-1.5 vs. 7.5+/-0.9 MJ x d(-1); AEE: 64.9+/-24.7 vs. 65.3+/-22.3 kJ x kg(-1) x d(-1); PAL: 1.57+/-0.18 vs. 1.58+/-0.19 kJ x kg(-1) x d(-1)). MA girls had lower TEE, AEE, and PAL than EA girls (TEE: 6.8+/-0.9 vs. 8.1+/-0.8 MJ x d(-1); AEE, 37.3+/-15.9 vs. 64.9+/-24.7 kJ x kg(-1) x d(-1); PAL, 1.40+/-0.12 vs. 1.57+/-0.18; P <0.005). Results suggest that these MA girls were expending less energy than EA children of comparable body size due to a reduced activity energy expenditure.

Predictors of hypertension awareness, treatment, and control among Mexican American women and men.

Project description:The burden of hypertension and related health care needs among Mexican Americans will likely increase substantially in the near future.In a nationally representative sample of U.S. Mexican American adults we examined: 1) the full range of blood pressure categories, from normal to severe; 2) predictors of hypertension awareness, treatment and control and; 3) prevalence of comorbidities among those with hypertension.Cross-sectional analysis of pooled data from the National Health and Nutrition Examination Surveys (NHANES), 1999-2004.The group of participants encompassed 1,359 Mexican American women and 1,421 Mexican American men, aged 25-84 years, who underwent a standardized physical examination.Physiologic measures of blood pressure, body mass index, and diabetes. Questionnaire assessment of blood pressure awareness and treatment.Prevalence of Stage 1 hypertension was low and similar between women and men ( approximately 10%). Among hypertensives, awareness and treatment were suboptimal, particularly among younger adults (65% unaware, 71% untreated) and those without health insurance (51% unaware, 62% untreated). Among treated hypertensives, control was suboptimal for 56%; of these, 23% had stage >/=2 hypertension. Clustering of CVD risk factors was common; among hypertensive adults, 51% of women and 55% of men were also overweight or obese; 24% of women and 23% of men had all three chronic conditions-hypertension, overweight/obesity and diabetes.Management of hypertension in Mexican American adults fails at multiple critical points along an optimal treatment pathway. Tailored strategies to improve hypertension awareness, treatment and control rates must be a public health priority.

"Big men" in the office: The gender-specific influence of weight upon persuasiveness.

Project description:Height has been closely studied as a factor that influences myriad measures of leadership; however, the potential influence of weight on socially beneficial traits has been neglected. Using the anthropological concept of "big men" who relied on influence to lead their communities, we examine the role of weight upon persuasiveness. We present the results of six studies that suggest a tendency for raters to expect larger body mass to correspond with more persuasiveness among men. In the sixth, pre-registered study, we find evidence that fits the hypothesis that weight among men is positively associated with perceived persuasiveness. While the "big man" leadership concept is based on studies of pre-industrial societies where weight embodied status, our findings suggest an evolved bias to favor moderately big men-with respect to perceived persuasiveness-even in environments where there is no reason to interpret over-consumption of food and conservation of energy as a signal of wealth. Our studies contribute novel perspectives on the relevance of weight as an understudied dimension of "big" and offer an important qualification informed by evolutionary perspectives for the stigmatizing effects of relatively large body mass.

Polygenic risk, family cohesion, and adolescent aggression in Mexican American and European American families: Developmental pathways to alcohol use.

Project description:Poor family cohesion and elevated adolescent aggression are associated with greater alcohol use in adolescence and early adulthood. In addition, evocative gene-environment correlations (rGEs) can underlie the interplay between offspring characteristics and negative family functioning, contributing to substance use. Gene-environment interplay has rarely been examined in racial/ethnic minority populations. The current study examined adolescents' polygenic risk scores for aggression in evocative rGEs underlying aggression and family cohesion during adolescence, their contributions to alcohol use in early adulthood (n = 479), and differences between Mexican American and European American subsamples. Results suggest an evocative rGE between polygenic risk scores, aggression, and low family cohesion, with aggression contributing to low family cohesion over time. Greater family cohesion was associated with lower levels of alcohol use in early adulthood and this association was stronger for Mexican American adolescents compared to European American adolescents. Results are discussed with respect to integration of culture and racial/ethnic minority samples into genetic research and implications for alcohol use.

Mexican American Immigrants Demonstrate Better Functional Stroke Outcomes Compared With Mexican American Nonimmigrants.

Project description:Background and purposeWe analyzed differences in 90-day poststroke outcomes between Mexican Americans born in the United States (nonimmigrant) compared with those born outside the United States (immigrant).MethodsWe performed a retrospective analysis of prospective data from the population-based Brain Attack Surveillance in Corpus Christi project. We identified stroke cases from 2008 to 2016 and quantified functional, cognitive, and neurological outcomes. Associations between outcome scores and immigration status were analyzed using weighted linear regression models.ResultsEighty-three Mexican American stroke cases (n=935) were immigrants, and 852 stroke cases were nonimmigrants. Average length of stay in the United States for immigrants was 47 years. Immigrants were older (69 versus 66 years), more likely men (60% versus 49%), had less education on average, and were more likely to have atrial fibrillation compared with nonimmigrants. No differences in other comorbidities existed between groups. After adjustment for confounders, immigrants had better functional outcomes (activities of daily living/instrumental activities of daily living; mean difference, -0.22; P=0.02; 1-4, higher scores worse) and no difference in neurological outcomes (log-National Institutes of Health Stroke Scale; mean difference, -0.15; P=0.15; 0-44, higher scores worse) or cognitive outcomes (3 Mini-Mental State Examination; mean difference, -0.79; P=0.64; 0-100, lower scores worse).ConclusionsLong-term Mexican American immigrants in this community display better stroke functional outcomes than nonimmigrant Mexican Americans and comparable neurological and cognitive outcomes.

CHRNA5 and CHRNA3 variants and level of neuroticism in young adult Mexican American men and women.

Project description:A lifetime history of alcohol dependence has been associated with elevations in neuroticism in Mexican American young adults. The identification of genetic markers associated with neuroticism and their influence on the development of alcohol use disorders (AUD) may contribute to our understanding of the relationship between personality traits and the increased risk of AUD in Mexican Americans. The purpose of this study was to investigate associations between neuroticism and 13 single nucleotide polymorphisms (SNPs) in the nicotinic acetylcholine (nAChR) ?5-subunit (CHRNA5) and ?3-subunit (CHRNA3) genes in young adult Mexican American men and women. Participants were 465 young adult Mexican American men and women who are literate in English and are residing legally in San Diego County. Each participant gave a blood sample and completed a structured diagnostic interview. Neuroticism was assessed using the Maudsley Personality Inventory. The minor alleles of four CHRNA5 polymorphisms (rs588765, rs601079, rs680244 and rs555018) and three CHRNA3 polymorphisms (rs578776, rs6495307 and rs3743078) showed associations with neuroticism. Several of these SNPs also displayed nominal associations with DSM-IV alcohol and nicotine dependence, but tests of mediation suggested that these relations could be partially explained by the presence of co-occurring neuroticism. These findings suggest that genetic variations in nicotinic receptor genes may influence the development of neuroticism, which in turn is involved in the development of AUDs and nicotine dependence in Mexican American young adults.

Project description:This is a pilot study regarding the use of mobile technology and short message service (SMS) for diet and exercise among rural Mexican American adolescents (RMAA). Authors used convenience sampling to recruit RMAA seeking care at a rural healthcare clinic and conducted three focus groups (n=12). Content analysis was used to identify categories and subcategories. Participants applied diet and exercise information in their lives based on an interaction with community and through the use of use mobile devices. Culturally sensitive use of mobile devices and SMS may be a tool to provide rural adolescent populations with resources.
