Relationships of Community and Individual Level Social Capital with Activities of Daily Living and Death by Gender.

ABSTRACT: This study determined whether there is an association between social capital and a composite outcome of decline in Activities of Daily Living (ADL) and death by gender. A prospective 3.5 year cohort study was conducted in a rural town in Japan. The study participants were 984 individuals aged 65 years and older with not impaired on ADL at 2010 baseline survey. Social participation and generalized trust were measured as social capital. The individual level responses were dichotomized and aggregated into the community level (eight areas). Multilevel logistic regression adjusting for covariates revealed that social participation at the individual level was significantly associated with higher odds of composite outcome (OR of "not participate" = 1.97, 95% CI = 1.38-2.81). Regarding generalized trust, only in men, there was an inverse association at the community level (OR of "low" = 0.55, 95% CI = 0.32-0.96), and a positive association at the individual level (OR of "tend to be careful" = 2.22, 95% CI = 1.27-3.90). These results suggest that social capital were associated with a decline in ADL and death and that the association may differ by gender.

Project description:BackgroundAlthough many studies have suggested social participation (SP) has beneficial effects on elderly people's health, most of them failed to deal with paid work. Additionally, few studies have focused on the age effect between SP and older people's health. To investigate whether the association between SP, including paid work, and instrumental activities of daily living (IADL), exhibits not only in gender, but also in age among community-dwelling older adults.MethodsIn 2014, we distributed self-administered questionnaires to all community-dwelling elderly aged ≥65 in two medium-sized cities in Nara Prefecture, Japan (n = 32,825). 22,845 residents submitted the questionnaire (response rate, 69.6%). Analyzed subjects were limited to 17,680 persons who had neither dependency in basic ADL nor missing data for required items. SP was assessed based on participation frequency in seven types of social activities: volunteer groups, sports groups, hobby groups, cultural groups, senior citizens' clubs, neighborhood community associations, and paid work. Using Poisson regression models, prevalence ratio for poor IADL was calculated. To examine age and gender differences in the association between SP and IADL, we performed stratified analyses by age and gender group; male young-old (aged 65-74), male old-old (aged ≥75), female young-old, and female old-old.ResultsPrevalence of those with poor IADL was 17.1% in males and 4.5% in females, showing a significant gender difference. After adjustment for relevant covariates, volunteer groups were inversely associated with poor IADL only in males and the relationship was stronger in the old-old group than in the young-old group. Conversely, only females had a significant inverse association between paid work and poor IADL, and the association was not reliant on their ages but only those who participated infrequently had a favorable effect. Influence of age in the beneficial association between SP and IADL was generally larger in the old-old group than in the young-old group, but hobby groups were inversely associated with poor IADL, regardless of age, gender, and frequency.ConclusionsOur results suggest that SP in older age is positively associated with IADL, however, the association seems to differ depending on the type of activities participated in, the participants' gender, and their age.

Project description:ObjectivesWe aimed to examine the associations of both individual-level and county-level social capital with individual health in China during a period of rapid economic growth.Design and settingA serial cross-sectional study in China.Participants and methodsThe participants were 42 829 Chinese adults (aged ≥18 years) from the 2010, 2012, 2013 and 2015 Chinese General Social Survey. The outcomes were self-rated physical and mental health in all time points. We assessed social capital by the individual-level and county-level indicators, including frequency of socialising, civic participation and trust. We conducted multilevel binary logistic regression models to examine the associations of individual-level and county-level social capital with self-rated physical and mental health.ResultsAt the individual level, high frequency of socialising (2010-OR: 1.49, 95% CI: 1.33 to 1.66; 2012-OR: 1.39, 95% CI: 1.26 to 1.54; 2013-OR: 1.28, 95% CI: 1.15 to 1.42; 2015-OR: 1.36, 95% CI: 1.23 to 1.50) and high trust (2010-OR: 1.34, 95% CI: 1.22 to 1.47; 2012-OR: 1.30, 95% CI: 1.18 to 1.42; 2013-OR: 1.21, 95% CI: 1.10 to 1.33; 2015-OR: 1.41, 95% CI: 1.28 to 1.55) was significantly associated with good physical health in all years. At the individual level, high frequency of socialising (2010-OR: 1.27, 95% CI: 1.14 to 1.42; 2012-OR: 1.21, 95% CI: 1.09 to 1.34; 2013-OR: 1.30, 95% CI: 1.17 to 1.45; 2015-OR: 1.35, 95% CI: 1.22 to 1.50) and high trust (2010-OR: 1.47, 95% CI: 1.34 to 1.61; 2012-OR: 1.42, 95% CI: 1.30 to 1.56; 2013-OR: 1.36, 95% CI: 1.24 to 1.49; 2015-OR: 1.43, 95% CI: 1.30 to 1.57) was also significantly associated with good mental health in all years. No evidence showed that the associations of individual-level frequency of socialising and trust with physical and mental health changed over time. There were no consistent associations of individual-level civic participation or any county-level social capital indicators with physical or mental health.ConclusionThe positive associations of individual-level social capital in terms of socialising and trust with physical and mental health were robust during a period of rapid economic growth. Improving individual-level socialising and trust for health promotion could be a long-term strategy even within a rapidly developing society.

Project description:BackgroundNumerous studies have clarified that family socioeconomic status (SES) is positively associated with health. However, the mechanism of family SES on health needs to be further investigated from a social epidemiological perspective. This study aims to analyze the relationships among family SES, family social capital, and adult general health and tests whether gender-based differences exist in the relationship between family social capital and general health.MethodsA cross-sectional survey was used to collect data from 4187 representative households in six Chinese provinces. Family SES was conceptualized based on household income, family education, and family occupational status. Family social capital was measured by using family cohesion and health-related family support. General health was assessed by using five general health perception items of the Health Survey Short Form. Structural equation modeling (SEM) was applied to examine the relationships among family SES, family social capital, and general health, and a linear regression model was used to test gender-based differences.ResultsThe SEM showed that the direct effects of family SES, family cohesion, and health-related family support on health were 0.08 (P?<?0.001), 0.17 (P?<?0.001), and 0.10 (P?<?0.001), respectively. Family SES had indirect effect (??=?0.05, P?<?0.01) on general health via health-related family support. The total effect of family social capital (??=?0.27, P?<?0.001) on general health was greater than that of family SES (??=?0.13, P?<?0.001). Besides, the regression showed that the effect of health-related family support on general health was greater for women (??=?0.13, P?<?0.001) than men (??=?0.04, P?>?0.05).ConclusionsThe results provide strong support for the positive association between family SES, family social capital, and adult health. Family intervention programs should focus on establishing a harmonious family relationship to mobilize family support, particularly for the families with low cohesion and low SES.

Project description:IntroductionSome Alzheimer's disease (AD) patients die without ever developing cognitively impaired basic activities of daily living (basic ADL), which may reflect slower disease progression or better compensatory mechanisms. Although impaired basic ADL is related to disease severity, it may exert an independent risk for death. This study examined the association between impaired basic ADL and survival of AD patients, and proposed a multistate approach for modeling the time to death for patients who demonstrate different patterns of progression of AD that do or do not include basic ADL impairment.Methods1029 patients with probable AD at the Baylor College of Medicine Alzheimer's Disease and Memory Disorders Center met the criteria for this study. Two complementary definitions were used to define development of basic ADL impairment using the Physical Self-Maintenance Scale score. A weighted Cox regression model, including a time-dependent covariate (development of basic ADL impairment), and a multistate survival model were applied to examine the effect of basic ADL impairment on survival.ResultsAs expected decreased ability to perform basic ADL at baseline, age at initial visit, years of education, and sex were all associated with significantly higher mortality risk. In those unimpaired at baseline, the development of basic ADL impairment was also associated with a much greater risk of death (hazard ratios 1.77-4.06) over and above the risk conferred by loss of MMSE points. A multi-state Cox model, controlling for those other variables quantified the substantive increase in hazard ratios for death conferred by the development of basic ADL impairment by two definitions and can be applied to calculate the short term risk of mortality in individual patients.ConclusionsThe current study demonstrates that the presence of basic ADL impairment or the development of such impairments are important predictors of death in AD patients, regardless of severity.
