Ultrasound-Guided Portal Placement for Hip Arthroscopy.

ABSTRACT: Over the past 2 decades, hip arthroscopy has developed as a surgical technique, with more orthopaedic surgeons attempting to become proficient in performing this surgical procedure as indications and surgical skills evolve. The hip joint presents unique arthroscopic challenges because of its anatomic location, and it can be challenging to safely and confidently establish portal sites. Ultrasound-guided hip arthroscopy portal placement is not yet common practice for orthopaedic surgeons. The potential advantages of ultrasound-guided hip arthroscopy portal placement are the lack of radiation exposure to the patient and operating room personnel, direct visualization of the hip joint and surrounding soft-tissue structures, and lack of bulky fluoroscopic equipment in the operative field. We describe a detailed technique incorporating video examples using ultrasound guidance to establish appropriate portal sites for hip arthroscopy with the aim to improve safety and overall operator competence.

Modified anterior portal for hip arthroscopy.

Project description:Routine portal placement for the central compartment should be based on minimizing iatrogenic injury and maximizing access. Anchor placement for labral repair/refixation requires a more distal entry position to accomplish sufficient divergence to avoid perforating the articular surface of the acetabulum. If a standard portal for joint access is moved more distal, this can compromise its utility for addressing intra-articular pathology. In addition, it can be difficult to position this portal sufficiently distally to ensure adequate divergence. Moving the anchor away from the rim to avoid perforation, due to suboptimal portal placement, can result in nonanatomic labral repair. Thus a percutaneous anchor delivery system is advantageous in ensuring adequate divergence without compromising routine portal placement for the central compartment.

Ultrasound-assisted hip arthroscopy.

Project description:We describe the use of intraoperative ultrasound for the safe development of arthroscopic portals during hip arthroscopy without the requirement for fluoroscopy. We find this technique consistently accurate, allowing the safe introduction of arthroscopic instruments into the hip with a very low rate of iatrogenic injury. We have further developed the technique for application to both central- and peripheral-compartment procedures. We now have a total experience of more than 700 procedures to date. With the described technique of ultrasound guidance for portal placement, fluoroscopy is required in fewer than 2% of hip arthroscopy procedures at our institution.

Intra-articular Fluid Distension for Initial Portal Placement During Hip Arthroscopy: The "Femoral Head Drop" Technique.

Project description:Iatrogenic injury is a known complication of initial portal placement during hip arthroscopy. The rate of labral puncture or damage to the articular surfaces with arthroscopic instruments is variable and may be associated with operator inexperience or complex anatomy. In addition, the amount of traction applied to achieve joint distraction may unnecessarily place patients at risk of neurapraxia. The purpose of this article is to describe the "femoral head drop" technique as a method to increase safe access to the central compartment and minimize the amount of traction needed to do so, especially in patients with challenging bony anatomy. This technique uses the application of intra-articular saline solution to cause inferior migration of the femoral head. Intra-articular fluid distension, or the femoral head drop technique, is simple, safe, and reproducible, making it appropriate for hip arthroscopists at any level of experience.

Project description:The modified mid-anterior portal is a utilitarian hip arthroscopy working portal that permits dual-portal comprehensive surgery for femoroacetabular impingement and related chondrolabral procedures without the need for interportal exchange. Its distal location facilitates labral reparative and reconstructive procedures while minimizing iatrogenic acetabular chondral damage. The relatively lateral location permits instrument navigation not only along the anterosuperior acetabular rim and anterolateral proximal femur typically required for acetabuloplasty and femoroplasty but even to the posterior regions of the hip in cases of global pincer femoroacetabular impingement and posterior extensions of cam morphology and the anteromedial proximal femur while avoiding direct injury to the lateral femoral cutaneous nerve.

Single-Portal Arthroscopy of the Central Compartment of the Hip.

Project description:Since hip arthroscopy has become a standard of orthopaedic practice, the indications have continued to expand as it has proved to be a helpful diagnostic, as well as therapeutic, tool. Access to the hip joint, however, remains challenging for the orthopaedic surgeon who does not routinely perform hip arthroscopy. We present a single-portal arthroscopic technique, showing the feasibility of single-portal arthroscopic access to the hip joint, as well as describing basic indications and instrumentation for single-portal hip arthroscopy. Single-portal hip arthroscopy is ideal for the patient who needs to undergo diagnostic hip arthroscopy or for treatment in patients with simple hip pathology (e.g., removal of loose bodies or debridement).

Hip capsulolabral spacer placement for the treatment of severe capsulolabral adhesions after hip arthroscopy.

Project description:Recently, there has been a rapid increase in the number of hip arthroscopies performed. The increase in the number of primary surgeries has been associated with a similar increase in the number of revision procedures. The most frequent indications for revision hip arthroscopy are residual bony deformity (impingement), persistent labral pathology, and intra-articular adhesions. Our current understanding of capsulolabral adhesions is limited. Although adhesions between the capsule and labrum are common after hip arthroscopy, generally, they are mild and asymptomatic. However, in severe cases they may cause persistent synovitis and pain, and they may tether the labrum away from the femoral head, causing loss of the suction-seal effect. Such patients present with nonspecific symptoms such as persistent pain, giving way, catching, and pain in hip flexion. Magnetic resonance imaging can aid in the diagnosis by showing the absence of liquid in the capsulolabral recess, although the definitive diagnosis is based on dynamic arthroscopic evaluation. We present our approach to the lysis of capsulolabral adhesions with preservation of labral tissue and describe a technique that uses an iliotibial band allograft to prevent recurrence of such adhesions by maintaining space between the capsule and labrum.

Ultrasound Arthroscopy of Hip in Treatment of Osteochondritis Dissecans.

Project description:An ultrasound arthroscopy (UA) technique is a promising tool for the evaluation of the articular cartilage during arthroscopic examination. However, the applicability of UA for the evaluation of the hip joint is unknown. We describe a UA assessment of a patient with osteochondritis dissecans at the femoral head. An ultrasound catheter designed for intravascular imaging was inserted into the hip joint by use of conventional arthroscopic portals, and the cartilage surfaces of the femoral head and acetabulum were investigated with ultrasound. UA provided essential quantitative information on the integrity of the articular cartilage and the condition of the subchondral plate not assessable with conventional arthroscopy. Furthermore, the UA technique provided the possibility to monitor arthroscopy-assisted retrograde drilling and bone transplantation in the hip joint.

Hip arthroscopy and the anterolateral portal: avoiding labral penetration and femoral articular injuries.

Project description:Establishing the introductory viewing portal is a key step in hip arthroscopy. Most hip arthroscopists initially gain access to the central compartment of the hip through the anterolateral portal. Iatrogenic injury to the labrum or the femoral head chondral surface is a common yet under-reported complication of the procedure. Instead of directing the needle toward the clear space of the distracted joint, labral penetration can be minimized by directing the needle slightly anteroinferior to the clear space, overlapping the superior femoral head. Femoral head scuffing can be minimized by adjusting the position of the beveled needle and confirming a straight guidewire trajectory before trocar placement. This technical note addresses surgical pearls to minimize iatrogenic injury during initial central compartment access.
