Surgical immune interventions for solid malignancies.

ABSTRACT: BACKGROUND:The purpose of this study was to systematically review clinically translatable immunotherapeutic agents that are delivered regionally for solid malignancies. DATA SOURCES:PubMed and ClinicalTrials.gov were searched for published and registered clinical trials, respectively. The search yielded 334 relevant publications, of which 116 articles were included for review after exclusion criteria were applied. CONCLUSIONS:There has been an increase in the regional administration of cell-based and viral vector-based clinical trials over the last 5 years. Surgical interventions have been developed for intrapleural, intracranial, intraperitoneal, and intratumoral routes of access to enhance the local delivery of these therapies. Multimodality therapies that combine regional immunotherapy with other local and systemic therapies are demonstrating continued growth as the field of immunotherapy continues to expand.

Project description:Microbes are an integral component of the tumor microenvironment (TME). However, mechanisms that direct microbial recruitment into tumors and the spatial relationship between intratumoral microbes and host cells remain poorly understood. Here, we show that microbes and immune cells have parallel spatial distribution and that the presence of intratumoral microbes is dependent on T cells. Analysis of human pancreatic ductal adenocarcinomas (PDAC) and lung adenocarcinomas (LUAD) revealed a spatially heterogeneous distribution of lipopolysaccharide (LPS) that is associated with T cell infiltration. Using mouse models of PDAC, we found that microbes were more abundant and diverse in tumors that were enriched in T cells compared to tumors that lacked T cells, despite no significant differences in the fecal microbiome. Consistent with these findings, we detected elevated levels of microbial genes in T cell-enriched tumor nests in human PDAC. Compared to microbe-poor tumor nests, microbe-enriched tumor nests displayed a higher number of myeloid cells, B cells, and plasma cells. Microbe-enriched tumor nests also showed upregulation of immune-related processes, including responses to bacteria, and receptors that mediate mucosal immune responses to microbes. Administration of antibiotics to tumor-bearing mice altered the phenotype and presence of intratumoral myeloid cells and B cells but did not alter T cell infiltration. In contrast, depletion of T cells reduced the presence of intratumoral microbes. Our results identify a novel coupling between microbes and the intratumoral immune landscape, with T cells shaping microbial presence and subsequent microbial-host interactions.

Project description:Solid tumors are composed of cancer cells and host immune cells that are distributed in a non-uniform pattern. Although this spatial heterogeneity may reflect mutational variations in cancer cells, mechanisms that direct the recruitment of immune cells to distinct regions within a tumor remain poorly understood. Here, we show that microbial-host interactions define tumor nests enriched in immune cells, and the distribution of microbes within tumors parallels the spatial heterogeneity of intratumoral lymphoid and myeloid cell populations. Analysis of human solid tumors revealed that the spatial distribution of immune cells, particularly CD8+ T cells, is markedly heterogeneous. Compared to T cell-poor (“cold”) tumor nests, T cell-rich (“hot”) tumor nests displayed a significantly higher number of myeloid cells, B cells, and plasma cells. We performed laser capture microdissection (LCM) followed by RNA sequencing to identify unique gene signatures that define tumor epithelium and stroma of cold and hot tumor nests. Cold tumor nests expressed genes that promote tumor proliferation and fibrosis, whereas hot tumor stroma and epithelium showed upregulation of immune-related processes, including responses to bacteria, and receptors that mediate mucosal immune responses to microbes, respectively. Consistent with these findings, we detected elevated levels of microbes within hot tumor nests in human pancreatic and lung cancers. Our data implicate host immune responses to microbes in defining intratumoral immune heterogeneity and highlight a potential role for crosstalks between microbes, cancer cells, and the host immune system in regulating cancer immunogenicity.

Here, we show that intratumoral infiltration of immune cells and microbes are heterogeneous, and the distribution of microbes within tumors are orchestrated by the spatial heterogeneity of intratumoral lymphoid populations. Fatigue and Functional Ability in Patients Undergoing Upfront Surgical Treatment for Solid Malignancies.

Project description:Fatigue is an underassessed and underreported aspect of cancer patients undergoing treatment. In patients being treated with surgery, its extent and manifestations may be varied but it affects their functional quality of life. This study was designed to evaluate the level of fatigue in pre- and post-surgery period and its relation with the functional disability in patients undergoing upfront surgery for solid malignancies. A prospective observational study was conducted between 2016 and 2017. A total of 71 patients with malignant solid tumors (up to stage III) undergoing upfront surgery were included. The fatigue and functional disability were assessed in pre- and post-surgery period using Multidimensional Fatigue Inventory-20 (MFI-20) and Functional Assessment of Chronic Illness Therapy (FACIT-F) questionnaires respectively. The mean age was 42.4 years. The post-operative fatigue levels were significantly higher compared with the pre-operative levels (p?=?0.001). The maximum levels of fatigue and loss of functional ability were seen at the time of discharge that recovered up to some extent after 30 days of surgery. Operative duration >?8 h, hospital stay >?9 days, and blood loss of >?200 ml were associated with increased fatigue level. Mental fatigue and limitation of physical activity were the most significant domains in pre- and post-surgery period respectively. This study concludes that cancer-related fatigue is present in both pre- and post-surgery period and it correlates with functional disability. Assessment of different dimensions of fatigue is important and patients need to be made aware about them for planning any specific intervention including life style modification to help them cope up with these practical issues.

The Rationale and Emerging Use of Neoadjuvant Immune Checkpoint Blockade for Solid Malignancies.

Project description:Unprecedented advances in the treatment of cancer have occurred through the use of immunotherapy, with several agents currently approved by the Food and Drug Administration (FDA) for the treatment of widespread metastatic disease across cancer types. Immune checkpoint blockade represents a particularly promising class of agents that block inhibitory molecules on the surface of T cells, resulting in their activation and propagation of an immune response. Treatment with these agents may re-invigorate anti-tumor immunity, resulting in therapeutic responses, and use of these agents currently is being studied in the adjuvant setting. Additionally, a strong rationale exists for their use in the neoadjuvant setting for high-risk resectable disease (e.g., regional nodal disease in the case of melanoma). This rationale is based on the relatively high risk of relapse for these patients, as well as on scientific evidence suggesting that long-term immunologic memory and tumor control may be superior in the setting of treatment for an intact tumor (i.e., neoadjuvant therapy) as opposed to treatment in the setting of micrometastatic disease (e.g., adjuvant treatment). The potential advantages of this approach and the current landscape for neoadjuvant immune checkpoint blockade is discussed in this report, as well as caveats that should be considered by clinicians contemplating this strategy.

Cerebrovasculature in brain malignancies from 13 surgical resected samples

Project description:Dysfunction in human cerebrovasculature contribute to pathogenesis of multiple neurological diseases, and the blood brain barrier (BBB), an organotypic specialization of the cerebrovasculature, impedes the delivery of systemic therapies for nearly all brain disorders.Here we developed a novel methodology to isolate fresh cells from the human cerebrovasculature with high capture efficiency and cell viability, and performed single-cell mRNA sequencing (scRNAseq) for human cerebrovascular cells from 13 surgical resected samples, including normal human brain tissue and gliomas, the most common brain malignancy. We profiled the transcriptome of 103,230 single cells, including 57,324 endothelial cells and 27,703 mural cells.We molecular defined the principle vascular cell types in the BBB, and uncovered molecular underpinning of heterogeneity in endothelial cells, mural cells and perivascular fibroblasts along the arteriovenous axis.
