Remplissage of the Femoral Head-Neck Junction in Revision Hip Arthroscopy: A Technique to Correct Excessive Cam Resection.

ABSTRACT: Femoroacetabular impingement (FAI) is an increasingly recognized cause of hip pain. This pathology often involves abnormal femoral neck shape (cam-type FAI), acetabular over coverage (pincer-type FAI), or mixed pathology. Surgical treatment of this entity includes labral repair and femoral head-neck osteochondroplasty. A mindful arthroscopic technique is of paramount importance, because maintaining the integrity of the labrum and a corresponding neck volume has been reported to be vital in maintaining the hip suction seal. Arthroscopic resection of the cam deformity must be performed with care to ensure appropriate femoral offset and maintenance of appropriate femoroacetabular contact. Although the most common cause of failure after hip arthroscopy is incomplete resection of a cam lesion, a previously unrecognized complication is excessive cam resection that can also lead to excessive femoral offset and loss of the suction seal. The purpose of this technical note is to describe the technique for arthroscopic recognition of excessive cam decompression leading to loss of the suction seal and a surgical treatment approach using the "remplissage" technique.

Project description:Hip arthroscopy is one of the most rapidly growing fields in orthopaedic surgery. One of the most frequent pathologies treated with hip arthroscopy remains femoroacetabular impingement, which is addressed by labral repair and femoral osteoplasty. The most commonly cited reason for failure of arthroscopic treatment of femoroacetabular impingement is under-resection of the cam lesion. Surgeons frequently use evaluations of preoperative images, intraoperative fluoroscopy, and dynamic range of motion to ensure adequate resection. In this article, we describe a reproducible and standardized technique to assist in appropriate resection. This is achieved by a 2-tiered resection technique: Tier 1 aims to set the depth of resection and restore the head-neck offset. Tier 2 then matches the depth of the resection set by tier 1 and allows for retention of appropriate transition of the proximal convexity to the distal concavity seen in more ideally shaped femoral heads. With this technique, we offer a tool to avoid under-resection in the area of maximal conflict while simultaneously minimizing the risk of proximal over-resection and thus compromising the fluid seal dynamics of the joint in deeper flexion angles. Technique Video Video 1 Visualization of femoral osteoplasty from distal anterolateral portal. The resection begins with visual inspection and localization of important anatomy including the lateral retinacular vessels. The osteoplasty ensues in a 2-tiered manner. The tier 1 resection is begun 1.5 burr widths distal to the articular surface margin with the goal to create the depth of resection required to re-create the native head-neck offset. This is also confirmed with fluoroscopic imaging from the Dunn lateral view to identify the physeal scar as the proximal extent of the osteoplasty. During the tier 1 resection, the proximal head-neck junction is contoured to achieve sphericity. The tier 2 resection is then begun distal to tier 1 with the goal of matching the depth and resecting the remainder of the cam. Finally, attention is turned back to the head-neck junction with the goal of creating a smooth, spherical head via gentle contouring. During the resection, multiple views through the distal anterolateral portal are used to gather a greater perspective of the resection. At the completion of the procedure, evaluation of dynamic hip range of motion under direct visualization, along with fluoroscopic imaging, is used to ensure adequate resection of the cam lesion.

Project description:PurposeTo identify clinical and radiographic factors associated with failure of revision hip arthroscopy (RHA).MethodsA database was used to identify patients who underwent primary hip arthroscopy and revision hip arthroscopy (RHA) from January 2007 to December 2017 for the indication of femoroacetabular impingement and failure of the index procedure, respectively. The primary outcome was defined as the change, or difference, in the preoperative to postoperative alpha angle between patients with successful RHA and those with failed RHA. Failure was defined as reoperation on the operative hip for any indication or a modified Harris Hip Score (mHHS) of less than 70 at the 1-year postoperative time point. All patients had a minimum of 2 years' follow-up from the date of revision hip surgery. Patients with a history of revision were divided into those with failed revisions and those with successful revisions. The inclusion criteria for failed revision included a history of subsequent revision surgery (or arthroplasty) or an mHHS of less than 70 at final follow-up.ResultsThe study included 26 patients, comprising 8 (31%) with failed RHA and 18 (69%) with successful revision. The failure group showed a significantly smaller decrease in the alpha angle with surgery, measured on the Dunn view, compared with the success group. When the preoperative alpha angle was held constant, each 1° increase in the difference between the preoperative and postoperative alpha angles achieved during surgery was associated with a 17% decrease in the odds of failure. Patients included in the success group had both a higher preoperative mHHS (44.2 ± 8.6 vs 34.7 ± 9.6) and a higher postoperative mHHS (83.2 ± 8.3 vs 62.3 ± 14.2) than patients with failed RHA. There was a statistically significant difference in the frequency of patients who achieved the patient acceptable symptomatic state of +74.0 between the failure (25%) and success (83%) groups; 88% of patients in the failure group met the minimal clinically important difference, whereas 100% of patients in the success group (n = 18) met it.ConclusionsComplete resection of cam lesions as determined by changes in the alpha angle, anterior offset, and head-neck ratio when measured on the Dunn 45° view correlates with positive clinical outcomes after RHA.Level of evidenceIII, Retrospective Comparative Study.
