Instrumental Variable Analyses and Selection Bias.

ABSTRACT: Instrumental variables (IV) are used to draw causal conclusions about the effect of exposure E on outcome Y in the presence of unmeasured confounders. IV assumptions have been well described: (1) IV affects E; (2) IV affects Y only through E; (3) IV shares no common cause with Y. Even when these assumptions are met, biased effect estimates can result if selection bias allows a noncausal path from E to Y. We demonstrate the presence of bias in IV analyses on a sample from a simulated dataset, where selection into the sample was a collider on a noncausal path from E to Y. By applying inverse probability of selection weights, we were able to eliminate the selection bias. IV approaches may protect against unmeasured confounding but are not immune from selection bias. Inverse probability of selection weights used with IV approaches can minimize bias.

Selecting on treatment: a pervasive form of bias in instrumental variable analyses.

Project description:Instrumental variable (IV) methods are increasingly being used in comparative effectiveness research. Studies using these methods often compare 2 particular treatments, and the researchers perform their IV analyses conditional on patients' receiving this subset of treatments (while ignoring the third option of "neither treatment"). The ensuing selection bias that occurs due to this restriction has gone relatively unnoticed in interpretations and discussions of these studies' results. In this paper we describe the structure of this selection bias with examples drawn from commonly proposed instruments such as calendar time and preference, illustrate the bias with causal diagrams, and estimate the magnitude and direction of possible bias using simulations. A noncausal association between the proposed instrument and the outcome can occur in analyses restricted to patients receiving a subset of the possible treatments. This results in bias in the numerator for the standard IV estimator; the bias is amplified in the treatment effect estimate. The direction and magnitude of the bias in the treatment effect estimate are functions of the distribution of and relationships between the proposed instrument, treatment values, unmeasured confounders, and outcome. IV methods used to compare a subset of treatment options are prone to substantial biases, even when the proposed instrument appears relatively strong.

Selection Bias When Estimating Average Treatment Effects Using One-sample Instrumental Variable Analysis.

Project description:Participants in epidemiologic and genetic studies are rarely true random samples of the populations they are intended to represent, and both known and unknown factors can influence participation in a study (known as selection into a study). The circumstances in which selection causes bias in an instrumental variable (IV) analysis are not widely understood by practitioners of IV analyses. We use directed acyclic graphs (DAGs) to depict assumptions about the selection mechanism (factors affecting selection) and show how DAGs can be used to determine when a two-stage least squares IV analysis is biased by different selection mechanisms. Through simulations, we show that selection can result in a biased IV estimate with substantial confidence interval (CI) undercoverage, and the level of bias can differ between instrument strengths, a linear and nonlinear exposure-instrument association, and a causal and noncausal exposure effect. We present an application from the UK Biobank study, which is known to be a selected sample of the general population. Of interest was the causal effect of staying in school at least 1 extra year on the decision to smoke. Based on 22,138 participants, the two-stage least squares exposure estimates were very different between the IV analysis ignoring selection and the IV analysis which adjusted for selection (e.g., risk differences, 1.8% [95% CI, -1.5%, 5.0%] and -4.5% [95% CI, -6.6%, -2.4%], respectively). We conclude that selection bias can have a major effect on an IV analysis, and further research is needed on how to conduct sensitivity analyses when selection depends on unmeasured data.

Toward a clearer portrayal of confounding bias in instrumental variable applications.

Project description:Recommendations for reporting instrumental variable analyses often include presenting the balance of covariates across levels of the proposed instrument and levels of the treatment. However, such presentation can be misleading as relatively small imbalances among covariates across levels of the instrument can result in greater bias because of bias amplification. We introduce bias plots and bias component plots as alternative tools for understanding biases in instrumental variable analyses. Using previously published data on proposed preference-based, geography-based, and distance-based instruments, we demonstrate why presenting covariate balance alone can be problematic, and how bias component plots can provide more accurate context for bias from omitting a covariate from an instrumental variable versus non-instrumental variable analysis. These plots can also provide relevant comparisons of different proposed instruments considered in the same data. Adaptable code is provided for creating the plots.

Combating unmeasured confounding in cross-sectional studies: evaluating instrumental-variable and Heckman selection models.

Project description:Unmeasured confounding is the principal threat to unbiased estimation of treatment "effects" (i.e., regression parameters for binary regressors) in nonexperimental research. It refers to unmeasured characteristics of individuals that lead them both to be in a particular "treatment" category and to register higher or lower values than others on a response variable. In this article, I introduce readers to 2 econometric techniques designed to control the problem, with a particular emphasis on the Heckman selection model (HSM). Both techniques can be used with only cross-sectional data. Using a Monte Carlo experiment, I compare the performance of instrumental-variable regression (IVR) and HSM to that of ordinary least squares (OLS) under conditions with treatment and unmeasured confounding both present and absent. I find HSM generally to outperform IVR with respect to mean-square-error of treatment estimates, as well as power for detecting either a treatment effect or unobserved confounding. However, both HSM and IVR require a large sample to be fully effective. The use of HSM and IVR in tandem with OLS to untangle unobserved confounding bias in cross-sectional data is further demonstrated with an empirical application. Using data from the 2006-2010 General Social Survey (National Opinion Research Center, 2014), I examine the association between being married and subjective well-being.

Birth weight is not causally associated with adult asthma: results from instrumental variable analyses.

Project description:The association between lower birth weight and childhood asthma is well established. However, it remains unclear whether the influence of lower birth weight on asthma can persist into adulthood. We conducted a Mendelian randomization analysis to assess the causal relationship of birth weight (~140,000 individuals) on the risk of adult asthma (~62,000 individuals). We estimated the causal effect of birth weight to be 1.00 (95% CI 0.98~1.03, p = 0.737) using the genetic risk score method. We did not observe nonlinear relationship or gender difference for the estimated causal effect. With the inverse-variance weighted method, the causal effect of birth weight on adult asthma was estimated to be 1.02 (95% CI 0.84~1.24, p = 0.813). Additionally, the iMAP method provides no additional genome-wide evidence supporting the causal effects of birth weight on adult asthma. Our results were robust against various sensitivity analyses, and MR-PRESSO and MR-Egger regression showed that no instrument outliers and no horizontal pleiotropy were likely to bias the results. Overall, our study provides no evidence for the fetal origins of diseases hypothesis for adult asthma, implying that the impact of birth weight on asthma in years of children and adolescents does not persist into adult and previous findings may be biased by confounders.

Comparison of intergenerational instrumental variable analyses of body mass index and mortality in UK Biobank.

Project description:BackgroundAn increasing proportion of people have a body mass index (BMI) classified as overweight or obese and published studies disagree whether this will be beneficial or detrimental to health. We applied and evaluated two intergenerational instrumental variable methods to estimate the average causal effect of BMI on mortality in a cohort with many deaths: the parents of UK Biobank participants.MethodsIn Cox regression models, parental BMI was instrumented by offspring BMI using an 'offspring as instrument' (OAI) estimation and by offspring BMI-related genetic variants in a 'proxy-genotype Mendelian randomization' (PGMR) estimation.ResultsComplete-case analyses were performed in parents of 233 361 UK Biobank participants with full phenotypic, genotypic and covariate data. The PGMR method suggested that higher BMI increased mortality with hazard ratios per kg/m2 of 1.02 (95% CI: 1.01, 1.04) for mothers and 1.04 (95% CI: 1.02, 1.05) for fathers. The OAI method gave considerably higher estimates, which varied according to the parent-offspring pairing between 1.08 (95% CI: 1.06, 1.10; mother-son) and 1.23 (95% CI: 1.16, 1.29; father-daughter).ConclusionBoth methods supported a causal role of higher BMI increasing mortality, although caution is required regarding the immediate causal interpretation of these exact values. Evidence of instrument invalidity from measured covariates was limited for the OAI method and minimal for the PGMR method. The methods are complementary for interrogating the average putative causal effects because the biases are expected to differ between them.

Instrumental variable with competing risk model.

Project description:In this paper, we discuss causal inference on the efficacy of a treatment or medication on a time-to-event outcome with competing risks. Although the treatment group can be randomized, there can be confoundings between the compliance and the outcome. Unmeasured confoundings may exist even after adjustment for measured covariates. Instrumental variable methods are commonly used to yield consistent estimations of causal parameters in the presence of unmeasured confoundings. On the basis of a semiparametric additive hazard model for the subdistribution hazard, we propose an instrumental variable estimator to yield consistent estimation of efficacy in the presence of unmeasured confoundings for competing risk settings. We derived the asymptotic properties for the proposed estimator. The estimator is shown to be well performed under finite sample size according to simulation results. We applied our method to a real transplant data example and showed that the unmeasured confoundings lead to significant bias in the estimation of the effect (about 50% attenuated). Analysis of the impact of early surgery on in-hospital mortality of native valve endocarditis: use of propensity score and instrumental variable methods to adjust for treatment-selection bias.

Project description:The impact of early surgery on mortality in patients with native valve endocarditis (NVE) is unresolved. This study sought to evaluate valve surgery compared with medical therapy for NVE and to identify characteristics of patients who are most likely to benefit from early surgery.Using a prospective, multinational cohort of patients with definite NVE, the effect of early surgery on in-hospital mortality was assessed by propensity-based matching adjustment for survivor bias and by instrumental variable analysis. Patients were stratified by propensity quintile, paravalvular complications, valve perforation, systemic embolization, stroke, Staphylococcus aureus infection, and congestive heart failure. Of the 1552 patients with NVE, 720 (46%) underwent early surgery and 832 (54%) were treated with medical therapy. Compared with medical therapy, early surgery was associated with a significant reduction in mortality in the overall cohort (12.1% [87/720] versus 20.7% [172/832]) and after propensity-based matching and adjustment for survivor bias (absolute risk reduction [ARR] -5.9%, P<0.001). With a combined instrument, the instrumental-variable-adjusted ARR in mortality associated with early surgery was -11.2% (P<0.001). In subgroup analysis, surgery was found to confer a survival benefit compared with medical therapy among patients with a higher propensity for surgery (ARR -10.9% for quintiles 4 and 5, P=0.002) and those with paravalvular complications (ARR -17.3%, P<0.001), systemic embolization (ARR -12.9%, P=0.002), S aureus NVE (ARR -20.1%, P<0.001), and stroke (ARR -13%, P=0.02) but not those with valve perforation or congestive heart failure.Early surgery for NVE is associated with an in-hospital mortality benefit compared with medical therapy alone.

Visualization tool of variable selection in bias-variance tradeoff for inverse probability weights.

Project description:PurposeInversed probability weighted (IPW) estimators are commonly used to adjust for time-fixed or time-varying confounders. However, in high-dimensional settings, including all identified confounders may result in unstable weights leading to higher variance. We aimed to develop a visualization tool demonstrating the impact of each confounder on the bias and variance of IPW estimates, as well as the propensity score overlap.MethodsA SAS macro was developed for this visualization tool and we demonstrate how this tool can be used to identify potentially problematic confounders of the association of statin use after myocardial infarction on one-year mortality in a plasmode simulation study using a cohort of 39,792 patients from the UK (1998-2012).ResultsThrough the tool's output, we can identify problematic confounders (two instrumental variables) and important confounders by comparing the estimated psuedo MSE with that from the fully adjusted model and propensity score overlap plot.ConclusionOur results suggest that the analytic impact of all confounders should be considered carefully when fitting IPW estimators.

Instrumental variable and colocalization analyses identify endotrophin and HTRA1 as potential therapeutic targets for coronary artery disease.

Project description:Coronary artery disease (CAD) remains a leading cause of disease burden globally, and there is a persistent need for new therapeutic targets. Instrumental variable (IV) and genetic colocalization analyses can help identify novel therapeutic targets for human disease by nominating causal genes in genome-wide association study (GWAS) loci. We conducted cis-IV analyses for 20,125 genes and 1,746 plasma proteins with CAD using molecular trait quantitative trait loci variant (QTLs) data from three different studies. 19 proteins and 119 genes were significantly associated with CAD risk by IV analyses and demonstrated evidence of genetic colocalization. Notably, our analyses validated well-established targets such as PCSK9 and ANGPTL4 while also identifying HTRA1 and endotrophin (a cleavage product of COL6A3) as proteins whose levels are causally associated with CAD risk. Further experimental studies are needed to confirm the causal role of the genes and proteins identified through our multiomic cis-IV analyses on human disease.
