Project description:BackgroundIn the poorly understood relationship between orthostatic hypotension and falls, next to blood pressure (BP), baroreflex sensitivity (BRS) and cerebral autoregulation (CAR) may be key measures. The posture- and movement dependency of orthostatic hypotension requires continuous and unobtrusive monitoring. This may be possible using simultaneous photoplethysmography (PPG), electrocardiography (ECG), and near-infrared spectroscopy (NIRS) signal recordings, from which pulse wave velocity (PWV; potentially useful for BP estimation), BRS and CAR can be derived. The PPG, NIRS and PWV signal correlation with BP and BRS/CAR reliability and validity need to be addressed.MethodsIn 34 healthy adults (mean age 25 years, inter quartile range 22-45; 10 female), wrist and finger PPG, ECG, bifrontal NIRS (oxygenated and deoxygenated hemoglobin) and continuous BP were recorded during sit to stand and supine to stand movements. Sixteen participants performed slow and rapid supine to stand movements; eighteen other participants performed a 1-min squat movement. Pulse wave velocity (PWV) was defined as the inverse of the ECG R-peak to PPG pulse delay; PPG, NIRS and PWV signal correlation with BP as their Pearson correlations with mean arterial pressure (MAP) within 30 s after the postural changes; BRS as inter beat interval drop divided by systolic BP (SBP) drop during the postural changes; CAR as oxygenated hemoglobin drop divided by MAP drop. BRS and CAR were separately computed using measured and estimated (linear regression) BP. BRS/CAR reliability was defined by the intra class correlation between repeats of the same postural change; validity as the Pearson correlation between BRS/CAR values based on measured and estimated BP.ResultsThe highest correlation with MAP was found for finger PPG and oxygenated hemoglobin, ranging from 0.75-0.79 (sit to stand), 0.66-0.88 (supine to stand), and 0.82-0.94 (1-min squat). BRS and CAR reliability was highest during the different supine to stand movements, ranging from 0.17 - 0.49 (BRS) and 0.42-0.75 (CAR); validity was highest during rapid supine to stand movements, 0.54 and 0.79 respectively.ConclusionPPG-ECG-NIRS recordings showed high correlation with BP and enabled computation of reliable and valid BRS and CAR estimates, suggesting their potential for continuous unobtrusive monitoring of orthostatic hypotension key measures.

Project description:Positron emission tomography (PET) is increasingly used for the detection, characterization, and follow-up of tumors located in the thorax. However, patient respiratory motion presents a unique limitation that hinders the application of high-resolution PET technology for this type of imaging. Efforts to transcend this limitation have been underway for more than a decade, yet PET remains for practical considerations a modality vulnerable to motion-induced image degradation. Respiratory motion control is not employed in routine clinical operations. In this article, we take an opportunity to highlight some of the recent advancements in data-driven motion control strategies and how they may form an underpinning for what we are presenting as a fully automated data-driven motion control framework. This framework represents an alternative direction for future endeavors in motion control and can conceptually connect individual focused studies with a strategy for addressing big picture challenges and goals.

Project description:PurposeRespiratory rate can provide auxiliary information on the physiological changes within the human body, such as physical and emotional stress. In a clinical setup, the abnormal respiratory rate can be indicative of the deterioration of the patient's condition. Most of the existing algorithms for the estimation of respiratory rate using photoplethysmography (PPG) are sensitive to external noise and may require the selection of certain algorithm-specific parameters, through the trial-and-error method.MethodsThis paper proposes a new algorithm to estimate the respiratory rate using a photoplethysmography sensor signal for health monitoring. The algorithm is resistant to signal loss and can handle low-quality signals from the sensor. It combines selective windowing, preprocessing and signal conditioning, modified Welch filtering and postprocessing to achieve high accuracy and robustness to noise.ResultsThe Mean Absolute Error and the Root Mean Square Error of the proposed algorithm, with the optimal signal window size, are determined to be 2.05 breaths count per minute and 2.47 breaths count per minute, respectively, when tested on a publicly available dataset. These results present a significant improvement in accuracy over previously reported methods. The proposed algorithm achieved comparable results to the existing algorithms in the literature on the BIDMC dataset (containing data of 53 subjects, each recorded for 8 min) for other signal window sizes.ConclusionThe results endorse that integration of the proposed algorithm to a commercially available pulse oximetry device would expand its functionality from the measurement of oxygen saturation level and heart rate to the continuous measurement of the respiratory rate with good efficiency at home and in a clinical setting.Supplementary informationThe online version contains supplementary material available at 10.1007/s40846-022-00700-z.

Project description:PurposeTo develop a patient-specific respiratory motion correction technique with true 100% acquisition efficiency.MethodsA short training scan consisting of a series of single heartbeat images, each acquired with a preceding diaphragmatic navigator, was performed to fit a model relating the patient-specific 3D respiratory motion of the heart-to-diaphragm position. The resulting motion model was then used to update the imaging plane in real-time to correct for translational motion based on respiratory position provided by the navigator. The method was tested in a group of 11 volunteers with 5 separate free-breathing acquisitions: FB, no motion correction; FB-TF, free breathing with a linear tracking factor; Nav Gate, navigator gating; Nav Gate-TF, navigator gating with a tracking factor; and PROCO, prospective motion correction (proposed). Each acquisition lasted for 50 accepted heartbeats, where non-gated scans had a 100% acceptance rate, and gated scans accepted data only within a ±4 mm navigator window. Retrospective image registration was used to measure residual motion and determine the effectiveness of each method.ResultsPROCO reduced the range/RMSE of residual motion to 4.08 ± 1.4/0.90 ± 0.3 mm, compared to 10.78 ± 6.9/2.97 ± 2.2 mm for FB, 5.32 ± 2.92/1.24 ± 0.8 mm for FB-TF, 4.08 ± 1.6/0.93 ± 0.4 mm for Nav Gate, and 2.90 ± 1.0/0.63 ± 0.2 mm for Nav Gate-TF. Nav Gate and Nav Gate-TF reduced scan efficiency to 48.84 ± 9.31% and 54.54 ± 10.12%, respectively.ConclusionPROCO successfully limited the residual motion in single-shot imaging to the level of traditional navigator gating while maintaining 100% acquisition efficiency.

Project description:Respiratory motion modeling of both tumor and surrounding tissues is a key element in minimizing errors and uncertainties in radiation therapy. Different continuous motion models have been previously developed. However, most of these models are based on the use of parameters such as amplitude and phase extracted from 1D external respiratory signal. A potentially reduced correlation between the internal structures (tumor and healthy organs) and the corresponding external surrogates obtained from such 1D respiratory signal is a limitation of these models. The objective of this work is to describe a continuous patient specific respiratory motion model, accounting for the irregular nature of respiratory signals, using patient external surface information as surrogate measures rather than a 1D respiratory signal.Ten patients were used in this study having each one 4D CT series, a synchronized RPM signal and patient surfaces extracted from the 4D CT volumes using a threshold based segmentation algorithm. A patient specific model based on the use of principal component analysis was subsequently constructed. This model relates the internal motion described by deformation matrices and the external motion characterized by the amplitude and the phase of the respiratory signal in the case of the RPM or using specific regions of interest (ROI) in the case of the patients' external surface utilization. The capability of the different models considered to handle the irregular nature of respiration was assessed using two repeated 4D CT acquisitions (in two patients) and static CT images acquired at extreme respiration conditions (end of inspiration and expiration) for one patient.Both quantitative and qualitative parameters covering local and global measures, including an expert observer study, were used to assess and compare the performance of the different motion estimation models considered. Results indicate that using surface information [correlation coefficient (CC): 0.998 ± 0.0006 and model error (ME): 1.35 ± 0.21 mm] is superior to the use of both motion phase and amplitude extracted from a 1D respiratory signal (CC and ME of 0.971 ± 0.02 and 1.64 ± 0.28 mm). The difference in performance was more substantial compared to the use of only one parameter (phase or amplitude) used in the motion model construction. Similarly, the patient surface based model was better in estimating the motion in the repeated 4D CT acquisitions and those CT images acquired at the full inspiration (FI) and the full expiration (FE). Once more, within this context the use of both amplitude and phase in the model building was substantially more robust than the use of phase or amplitude only.The present study demonstrates the potential of using external patient surfaces for the construction of patient specific respiratory motion models. Such information can be obtained using different devices currently available. The use of external surface information led to the best performance in estimating internal structure motion. On the other hand, the use of both amplitude and phase parameters derived from an 1D respiration signal led to largely superior model performance relative to the use of only one of these two parameters in the model building process.

Project description:BackgroundIn the current era of personalized medicine, there is increasing interest in understanding the heterogeneity in disease populations. Cluster analysis is a method commonly used to identify subtypes in heterogeneous disease populations. The clinical data used in such applications are typically multimodal, which can make the application of traditional cluster analysis methods challenging.ObjectiveThis study aimed to review the research literature on the application of clustering multimodal clinical data to identify asthma subtypes. We assessed common problems and shortcomings in the application of cluster analysis methods in determining asthma subtypes, such that they can be brought to the attention of the research community and avoided in future studies.MethodsWe searched PubMed and Scopus bibliographic databases with terms related to cluster analysis and asthma to identify studies that applied dissimilarity-based cluster analysis methods. We recorded the analytic methods used in each study at each step of the cluster analysis process.ResultsOur literature search identified 63 studies that applied cluster analysis to multimodal clinical data to identify asthma subtypes. The features fed into the cluster algorithms were of a mixed type in 47 (75%) studies and continuous in 12 (19%), and the feature type was unclear in the remaining 4 (6%) studies. A total of 23 (37%) studies used hierarchical clustering with Ward linkage, and 22 (35%) studies used k-means clustering. Of these 45 studies, 39 had mixed-type features, but only 5 specified dissimilarity measures that could handle mixed-type features. A further 9 (14%) studies used a preclustering step to create small clusters to feed on a hierarchical method. The original sample sizes in these 9 studies ranged from 84 to 349. The remaining studies used hierarchical clustering with other linkages (n=3), medoid-based methods (n=3), spectral clustering (n=1), and multiple kernel k-means clustering (n=1), and in 1 study, the methods were unclear. Of 63 studies, 54 (86%) explained the methods used to determine the number of clusters, 24 (38%) studies tested the quality of their cluster solution, and 11 (17%) studies tested the stability of their solution. Reporting of the cluster analysis was generally poor in terms of the methods employed and their justification.ConclusionsThis review highlights common issues in the application of cluster analysis to multimodal clinical data to identify asthma subtypes. Some of these issues were related to the multimodal nature of the data, but many were more general issues in the application of cluster analysis. Although cluster analysis may be a useful tool for investigating disease subtypes, we recommend that future studies carefully consider the implications of clustering multimodal data, the cluster analysis process itself, and the reporting of methods to facilitate replication and interpretation of findings.

Project description:PurposeTo explore the benefit of using 4D multimodal visualization and interaction techniques for defined radiotherapy planning tasks over a treatment planning system used in clinical routine (C-TPS) without dedicated 4D visualization.MethodsWe developed a 4D visualization system (4D-VS) with dedicated rendering and fusion of 4D multimodal imaging data based on a list of requirements developed in collaboration with radiation oncologists. We conducted a user evaluation in which the benefits of our approach were evaluated in comparison to C-TPS for three specific tasks: assessment of internal target volume (ITV) delineation, classification of tumor location in peripheral or central, and assessment of dose distribution. For all three tasks, we presented test cases for which we measured correctness, certainty, consistency followed by an additional survey regarding specific visualization features.ResultsLower quality of the test ITVs (ground truth quality was available) was more likely to be detected using 4D-VS. ITV ratings were more consistent in 4D-VS and the classification of tumor location had a higher accuracy. Overall evaluation of the survey indicates 4D-VS provides better spatial comprehensibility and simplifies the tasks which were performed during testing.ConclusionsThe use of 4D-VS has improved the assessment of ITV delineations and classification of tumor location. The visualization features of 4D-VS have been identified as helpful for the assessment of dose distribution during user testing.

Project description:Imaging the infant brain with MRI has improved our understanding of early neurodevelopment. However, head motion during MRI acquisition is detrimental to both functional and structural MRI scan quality. Though infants are typically scanned while asleep, they commonly exhibit motion during scanning causing data loss. Our group has shown that providing MRI technicians with real-time motion estimates via Framewise Integrated Real-Time MRI Monitoring (FIRMM) software helps obtain high-quality, low motion fMRI data. By estimating head motion in real time and displaying motion metrics to the MR technician during an fMRI scan, FIRMM can improve scanning efficiency. Here, we compared average framewise displacement (FD), a proxy for head motion, and the amount of usable fMRI data (FD ≤ 0.2 mm) in infants scanned with (n = 407) and without FIRMM (n = 295). Using a mixed-effects model, we found that the addition of FIRMM to current state-of-the-art infant scanning protocols significantly increased the amount of usable fMRI data acquired per infant, demonstrating its value for research and clinical infant neuroimaging.

Project description:Duchenne muscular dystrophy (DMD) is the most common x-chromosomal inherited dystrophinopathy which leads to progressive muscle weakness and a premature death due to cardiorespiratory dysfunction. The mdx mouse lacks functional dystrophin protein and has a comparatively human-like diaphragm phenotype. To date, diaphragm function can only be inadequately mapped in preclinical studies and a simple reliable translatable method of tracking the severity of the disease still lacks. We aimed to establish a sensitive, reliable, harmless and easy way to assess the effects of respiratory muscle weakness and subsequent irregularity in breathing pattern. Optical respiratory dynamics tracking (ORDT) was developed utilising a camera to track the movement of paper markers placed on the thoracic-abdominal region of the mouse. ORDT successfully distinguished diseased mdx phenotype from healthy controls by measuring significantly higher expiration constants (k) in mdx mice compared to wildtype (wt), which were also observed in the established X-ray based lung function (XLF). In contrast to XLF, with ORDT we were able to distinguish distinct fast and slow expiratory phases. In mdx mice, a larger part of the expiratory marker displacement was achieved in this initial fast phase as compared to wt mice. This phenomenon could not be observed in the XLF measurements. We further validated the simplicity and reliability of our approach by demonstrating that it can be performed using free-hand smartphone acquisition. We conclude that ORDT has a great preclinical potential to monitor DMD and other neuromuscular diseases based on changes in the breathing patterns with the future possibility to track therapy response.

Project description:Respiratory motion degrades the detection and quantification capabilities of PET/CT imaging. Moreover, mismatch between a fast helical CT image and a time-averaged PET image due to respiratory motion results in additional attenuation correction artifacts and inaccurate localization. Current motion compensation approaches typically have 3 limitations: the mismatch among respiration-gated PET images and the CT attenuation correction (CTAC) map can introduce artifacts in the gated PET reconstructions that can subsequently affect the accuracy of the motion estimation; sinogram-based correction approaches do not correct for intragate motion due to intracycle and intercycle breathing variations; and the mismatch between the PET motion compensation reference gate and the CT image can cause an additional CT-mismatch artifact. In this study, we established a motion correction framework to address these limitations. Methods: In the proposed framework, the combined emission-transmission reconstruction algorithm was used for phase-matched gated PET reconstructions to facilitate the motion model building. An event-by-event nonrigid respiratory motion compensation method with correlations between internal organ motion and external respiratory signals was used to correct both intracycle and intercycle breathing variations. The PET reference gate was automatically determined by a newly proposed CT-matching algorithm. We applied the new framework to 13 human datasets with 3 different radiotracers and 323 lesions and compared its performance with CTAC and non-attenuation correction (NAC) approaches. Validation using 4-dimensional CT was performed for one lung cancer dataset. Results: For the 10 18F-FDG studies, the proposed method outperformed (P < 0.006) both the CTAC and the NAC methods in terms of region-of-interest-based SUVmean, SUVmax, and SUV ratio improvements over no motion correction (SUVmean: 19.9% vs. 14.0% vs. 13.2%; SUVmax: 15.5% vs. 10.8% vs. 10.6%; SUV ratio: 24.1% vs. 17.6% vs. 16.2%, for the proposed, CTAC, and NAC methods, respectively). The proposed method increased SUV ratios over no motion correction for 94.4% of lesions, compared with 84.8% and 86.4% using the CTAC and NAC methods, respectively. For the 2 18F-fluoropropyl-(+)-dihydrotetrabenazine studies, the proposed method reduced the CT-mismatch artifacts in the lower lung where the CTAC approach failed and maintained the quantification accuracy of bone marrow where the NAC approach failed. For the 18F-FMISO study, the proposed method outperformed both the CTAC and the NAC methods in terms of motion estimation accuracy at 2 lung lesion locations. Conclusion: The proposed PET/CT respiratory event-by-event motion-correction framework with motion information derived from matched attenuation-corrected PET data provides image quality superior to that of the CTAC and NAC methods for multiple tracers.
