Arthroscopic Decompression of a Type III Subspine Impingement.

ABSTRACT: Extra-articular hip impingement refers to a variety of hip disorders causing pain and limited function in young, non-arthritic patients. Recently, there has been an increased focus on analyzing the degree of anterior inferior iliac spine (AIIS) dysmorphism and its correlation with subspine impingement (SSI), defined as abutment between a prominent distal aspect of the AIIS and the anterior aspect of the femoral head-neck junction. Arthroscopic decompression of the AIIS is recognized as an effective treatment for SSI. However, there may be some inherent risks of performing this procedure arthroscopically that require further investigation.

Arthroscopic Focal Subspinal Decompression and Management of Pincer-Type Femoroacetabular Impingement.

Project description:Femoroacetabular impingement syndrome is a common hip pathology significantly affecting not only the intra- and extra-articular structures but also the biomechanical function of the joint. Cam and pincer bony lesions have been extensively studied. However, during recent years, other types of extra-articular impingement between the pelvic and femoral bone have been investigated. When a prominent or morphologically abnormal anterior-inferior iliac spine (AIIS) impinges repetitively on the femoral side during motion, the subspinal acetabular region becomes prominent and extends toward the intra-articular part of the joint. This results in restriction of the range of motion of the hip and pain, especially with flexion. Therefore, during hip arthroscopy, it is necessary to evaluate the subspinal region (triangular area located at 1:30 to 2:30 o'clock using the acetabular clock face system). For the correction of the acetabular bone pathology to be complete, the surgeon should focus both on the pincer and subspinal impingement lesions. This article describes our preferred technique to successfully address subspinal and pincer acetabular impingement during hip arthroscopy. The pearls and pitfalls of this technique are discussed.

Arthroscopic decompression of central acetabular impingement with notchplasty.

Project description:Acetabular notch osteophytes are often encountered during routine diagnostic arthroscopy of the hip. It has been our observation that when notch osteophytes are present, there is often corresponding chondral damage to the anterosuperior femoral head and ligamentum teres degeneration. We propose that removal of the notch osteophyte and decompression of the articulating surface offer an effective method of delaying the progression of arthritis. This article describes in detail the technique used to perform arthroscopic acetabular notchplasty, and a companion video, demonstrating the procedure, is included. Our experience suggests that decompression of the acetabular notch can remove offending structural abnormalities that can potentially cause further chondral damage and may hasten the progression of arthritis.

Arthroscopic Decompression for Medial Ankle Impingement After Total Ankle Arthroplasty.

Project description:Total ankle arthroplasty (TAA) is gaining in popularity, with the main objective to restore a pain-free mobile and stable ankle and is hoped to solve the long-term problems associated with ankle arthrodesis. Residual pain is not uncommon after TAA, and most is located at the medial gutter with bony impingement as the frequent cause. In this Technical Note, the technical details of arthroscopic decompression for medial ankle impingement after total ankle arthroplasty is described. It has the advantage of earlier weightbearing, faster recovery and less risk of periprosthetic infection.

Femoroacetabular impingement in 45 professional athletes: associated pathologies and return to sport following arthroscopic decompression.

Project description:Femoroacetabular impingement (FAI) occurs when an osseous abnormality of the proximal femur (cam) or acetabulum (pincer) triggers damage to the acetabular labrum and articular cartilage in the hip. Although the precise etiology of FAI is not well understood, both types of FAI are common in athletes presenting with hip pain, loss of range-of-motion, and disability in athletics. An open surgical approach to decompressing FAI has shown good clinical outcomes; however, this highly invasive approach inherently may delay or preclude a high level athlete's return to play. The purpose of this study was to define associated pathologies and determine if an arthroscopic approach to treating FAI can allow professional athletes to return to high-level sport. Hip arthroscopy for the treatment of FAI allows professional athletes to return to professional sport. Between October 2000 and September 2005, 45 professional athletes underwent hip arthroscopy for the decompression of FAI. Operative and return-to-play data were obtained from patient records. Average time to follow-up was 1.6 years (range: 6 months to 5.5 years). Forty two (93%) athletes returned to professional competition following arthroscopic decompression of FAI. Three athletes did not return to play; however, all had diffuse osteoarthritis at the time of arthroscopy. Thirty-five athletes (78%) remain active in professional sport at an average follow-up of 1.6 years. Arthroscopic treatment of FAI allows professional athletes to return to professional sport.

Arthroscopic Femoral Osteochondroplasty for Cam-Type Femoroacetabular Impingement: The Trough Technique.

Project description:Arthroscopic osteochondroplasty has become the most common treatment for cam-type femoroacetabular impingement. However, gauging the appropriate depth and location of the femoral osteochondroplasty remains challenging, given the parallax observed from using a 70° arthroscope across multiple viewing perspectives. Consequently, reliable techniques must use a combination of arthroscopic and fluoroscopic checks and balances to assess the femoral head-neck junction to help guide bony resection. We have developed a technique for osteochondroplasty that has made the process more efficient and reliable in our hands. It involves creating a trough at the apex of the osteochondroplasty and then contouring the proximal and distal regions to re-create normal proximal femoral geometry. This article details our technique for femoral osteochondroplasty, which can be performed alone for isolated cam impingement or in concert with other intra- and extra-articular procedures to address associated hip pathology.

Arthroscopic Management of Posteromedial Ankle Impingement.

Project description:Posteromedial ankle impingement is a rare clinical entity. It usually follows an inversion injury, with compression of the posterior tibiotalar ligament between the medial malleolus and talus. This can be treated by posterior ankle endoscopy through the posteromedial and posterolateral portals. The flexor hallucis longus tendon can be examined for any tenosynovitis or tendinopathy. The posteromedial corner of the ankle joint is reached with the instruments staying on the lateral side of the flexor hallucis longus tendon. The inflamed synovium, scar tissue, and fibrillated cartilage are debrided.

Arthroscopic technique for treatment of femoroacetabular impingement.

Project description:With an increasing understanding of femoroacetabular impingement (FAI) and advancement of the surgical treatment in patients with FAI, various techniques have been published. Successful outcome after arthroscopic hip surgery depends on appropriately reshaping the bony architecture to allow for improved range of motion before impingement symptoms occur, with special attention to preserve the labrum and restore its function. We present our surgical technique for the arthroscopic treatment of FAI.

Arthroscopic Scapulothoracic Decompression for Snapping Scapula Syndrome.

Project description:Snapping scapula syndrome at the superomedial corner of the scapula can lead to significant shoulder dysfunction. Bursectomy with or without partial scapulectomy is currently the most beneficial primary method of treatment in patients in whom nonoperative therapy fails. Arthroscopic access to the scapulothoracic space is simple and reproducible with the technique described in this report. The bursal tissue can be cleared, optimizing visualization of the scapulothoracic space and the anatomic structures. Arthroscopic decompression of the scapulothoracic bursa and resection of the superomedial corner of the scapula are highlighted in a video example.

Arthroscopic Femoral Osteochondroplasty for Cam-type Femoroacetabular Impingement: Cortical-Cancellous Sclerotic Boundary Guides Resection Depth.

Project description:Hip arthroscopy and femoral osteochondroplasty are commonly used in the treatment of femoroacetabular impingement (FAI). Determining the correct resection depth of the femoral head-neck cam lesion intraoperatively can be challenging. Both inadequate resection and over-resection may result in complications, underlying the importance of using a consistent and accurate technique when resecting and reshaping the proximal femur. Osseous resection to a depth of the subchondral cortical-cancellous bone margin in individuals with FAI has been shown to restore proximal femoral anatomy to within submillimeter differences when compared with control subjects without FAI. This bony boundary may be used as an intraoperative guide to consistently achieve appropriate resection depth. The sclerotic margin indicating the extent of the cam-type deformity can be evaluated with preoperative radiographs and recreated fluoroscopically, giving the surgeon a reliable intraoperative template. In addition, changes in clinical appearance during arthroscopy, particularly identification of the underlying trabecular structure at the cortical-cancellous border during resection, provides a visible, reliable intraoperative guide to resection depth.
