Single Portal Subscapular Repair by a Cross Shuttle Loop Technique.

ABSTRACT: With the rise in subscapular tendon tears, most of them being partial that eventually end up in complete tears, we present our experience in the management of such tears using a cross shuttle loop technique. This technique incorporates the advantages of a single portal for anchor placement and repair, easy to use cross loops in limited working space, and familiar viewing.

Project description:Successful techniques for arthroscopic repair of subscapularis tendon tears have been previously described in the literature. Recommendations regarding portal placement, tissue mobilization, and suture passage have been published. We present a novel technique that uses a shuttle suture passed with the Viper suture passer (Arthrex, Naples, FL) through a standard anterior arthroscopy portal. The described technique easily passes a suture through the subscapularis tendon while the surgeon visualizes suture placement from the posterior portal.

Single Portal Technique for Subscapularis Tendon Repair.

Project description:The fast and steady development of arthroscopy techniques in the last couple of decades led to a drastic increase of arthroscopic rotator cuff repairs over the open procedure. Supraspinatus tears are the most common of all, but the subscapularis tendon tear is a more common injury than expected. Most of the time it presents as a partial tear or is associated with a subsequent rotator cuff tendon injury, especially the supraspinatus. Nowadays, the standard procedure to repair the subscapularis tendon is performed arthroscopically, even though a real superior result over the open repair it is yet to be reported. Ideally less operative time, less scarring, and postoperative pain would be the expected benefits, but no study has compared the long-term outcome of these 2 procedures yet. To maximize possible improvements, we would like to present an arthroscopic technique: a subscapularis tendon repair performed with the aid of an angled suture passer and using a single anterior working portal.

Aquarium Portal Technique for PASTA Lesion Repair.

Project description:The simultaneous arthroscopic exposure of the subacromial and intra-articular space of the shoulder is challenging in the presence of only partial-thickness rotator cuff tears. We present our experience and method of entering the joint through the opened rotator cuff interval from an anterosuperior portal between the coracoid process and anterior acromion. With moderate (approximately 30°) abduction and external rotation, the rotator interval opens readily, offering a view with the camera toward the anterior edge of the supraspinatus tendon. An anterior view on the anterior leading edge of the supraspinatus tendon is obtained, showing the subacromial space above and the glenohumeral space below the tendon, similar to the view in an aquarium. The rotator cuff can be elevated using a rod inserted intra-articularly from posterior, whereas anchors and other instruments may be inserted from lateral. This approach offers the advantages of full exposure of the posterior undersurface of the rotator cuff insertion; a convenient approximately 90° angle between the camera and instruments; and no need to change portals for anchor placement, tendon stitching, or suture handling. The objective of this Technical Note is to describe our arthroscopic repair approach (aquarium technique) to PASTA (partial articular supraspinatus tendon avulsion) lesions.

Some complications occur related to portal incisions, such as infection, bleeding, increased surgical time, and surgical scar. By using 1 lateral working portal with decreased suture tangling, surgical complications can be reduced and rehabilitation and patient satisfaction can be enhanced.

Single Portal Knee Arthroscopy: 2015 Technique Update.

Project description:A technique of single portal knee arthroscopy was reported in 2013. Using a parallel working cannula assembled to the arthroscope cannula, the instruments are passed into the joint in line with the arthroscope. The original technique video in Arthroscopy Techniques shows the use of a 25 mm tube assembly with a parallel portal through which biters and proximally bent cutter instruments are used to perform procedures in the knee. This 2015 technical update shows the current preferred and most versatile method of single portal knee arthroscopy using a parallel locking and rotating hub for passage of biters and double-bend cutters. The video shows adequate completion of a partial meniscectomy using only 1 portal.

Outside-In Single-Lasso Loop Technique for Meniscal Repair: Fast, Economic, and Reproducible.

Project description:The current understanding of the biomechanical role of the meniscus, in conjunction with the increasing efforts to achieve its preservation within the orthopaedic community during treatment of meniscal lesions, has prompted the development of different meniscal repair techniques. The outside-in technique is recommended for anterior horn and middle-segment meniscal tears and has been recognized as a low-cost procedure with a low incidence of complications. Diverse modifications of this technique have been published over the past decade. On the basis of these previous outside-in technique modifications and aiming to simplify and reduce the number of surgical steps, as well as simplify suture and/or instrument manipulation, during this technique, we describe the single-lasso loop outside-in technique for meniscal repair. We believe this modified technique represents a simplified, economic, and highly reproducible procedure option whenever an outside-in technique for meniscal repair is considered.

Lateral Meniscus Repair Using Posterolateral Portal: Suture Hook Technique.

Project description:Lateral meniscus lesions result in loss of meniscus hoop stresses and can lead to lateral compartment overload and early degenerative changes. Arthroscopic suture repair provides good long-term results. However, posterior vertical tears in the peripheral area of the meniscus can be technically challenging to resolve. This Technical Note describes the suture hook technique using an accessory posterolateral portal. We believe it is a safe, effective method for repairing full vertical tears of the lateral meniscus.

Arthroscopic Single-Portal Subscapularis Tendon Repair.

Project description:Tears of the subscapularis tendon can be challenging to diagnose and treat. Because the subscapularis plays an important role in shoulder function, careful arthroscopic evaluation and treatment are necessary to restore function. Previous surgical techniques have ranged from full open repairs to complex arthroscopic procedures needing suture passer and/or retriever devices. We describe an arthroscopic surgical technique of subscapularis repair through a single anterior portal using only penetrating graspers. This approach can be used for partial upper-border subscapularis tears, as well as complete and retracted subscapularis tendon tears.

Outside-In Repair of Longitudinal Tear of Medial Meniscus: Suture Shuttle Technique.

Project description:Meniscal tears are commonly encountered conditions of the knee. In the past, torn menisci were treated by excision of the loose flap. A better understanding of the meniscus anatomy and its biomechanical characteristics has led to the concept of meniscus preservation in eligible cases. Several suture-based repair techniques have been described in literature, including the outside-in technique. Although the outside-in technique of meniscus repair is commonly indicated for the anterior two-thirds of the meniscus, it can be used to repair the posterior part of the meniscus as well. Several modifications of this technique have been described in the literature. We hereby describe our modification of the outside-in technique of meniscus repair with the help of an epidural needle and high-strength sutures that is readily available in operating rooms. The advantages of our technique are that no large incision is required around knee joint, it's inexpensive, it can be performed with basic instruments, and even the tear of meniscus extending up to the posterior horn can be repaired. A supplemental video demonstration of the technique is included with this article.
