Emptying and filling a tunnel bronze.

ABSTRACT: The classical orthorhombic layered phase of V2O5 has long been regarded as the thermodynamic sink for binary vanadium oxides and has found great practical utility as a result of its open framework and easily accessible redox states. Herein, we exploit a cation-exchange mechanism to synthesize a new stable tunnel-structured polymorph of V2O5 (?-V2O5) and demonstrate the subsequent ability of this framework to accommodate Li and Mg ions. The facile extraction and insertion of cations and stabilization of the novel tunnel framework is facilitated by the nanometer-sized dimensions of the materials, which leads to accommodation of strain without amorphization. The topotactic approach demonstrated here indicates not just novel intercalation chemistry accessible at nanoscale dimensions but also suggests a facile synthetic route to ternary vanadium oxide bronzes (M x V2O5) exhibiting intriguing physical properties that range from electronic phase transitions to charge ordering and superconductivity.

Biomechanics of urinary bladder: slow-filling and slow-emptying cystometry and accommodation.

Project description:ObjectiveIn a concept of accommodation of detrusor pressure to volume as an autonomous potency of the bladder, a crucial physiological biomechanical role has been attributed to spontaneous contraction activity. This concept is experimentally investigated on pig bladder in vitro.MethodsSlowly emptying of not-stimulated pig bladders via a flow resistor has been recorded and the effect of spontaneous contractions on the tonic pressure during emptying by expulsion has been studied.ResultsThe expulsed volume can be separated in a reduction of elastic volume and of rest volume. Tonic pressure is determined by the elastic volume in combination with elastic compliance. In an accommodated state completely transient superimposed pressure waves affect rest volume not elastic volume. Accommodation of tonic detrusor pressure to bladder volume is based on equilibration between passive elongations and active transient contractions distributed in bladder wall.ConclusionMaintenance of a tonic accommodated detrusor pressure to a constant or slowly varying volume, obtained by a process of equilibration between passive elongations and active contractions, can be understood as an autonomous potency of a bladder. The earlier presented concept of active accommodation has been validated by the experiments. The pressure-volume relation of the bladder is fundamentally revised. Total volume V can be virtually separated in an elastic volume VE and a plastic or rest volume VR. Both parts change with V and in changing ratio. Tonic pressure marks a border between VE and VR.

Charge filling factors in clean and disordered arrays of tunnel junctions.

Project description:We simulate one-dimensional arrays of tunnel junctions using the kinetic Monte Carlo method to study charge filling behaviour in the large charging energy limit. By applying a small fixed voltage bias and varying the offset voltage, we investigate this behaviour in clean and disordered arrays (both weak and strong disorder effects). The offset voltage dependent modulation of the current is highly sensitive to background charge disorder and exhibits substantial variation depending on the strength of the disorder. We show that while small fractional charge filling factors are likely to be washed out in experimental devices due to strong background charge disorder, larger factors may be observable.

Assessment of symptoms during gastric emptying scintigraphy to correlate symptoms to delayed gastric emptying.

Project description:BackgroundSymptoms of gastroparesis based on patient recall correlate poorly with gastric emptying. The aim of this study is to determine if symptoms recorded during gastric emptying scintigraphy (GES) correlate with gastric emptying and with symptoms based on patient recall.MethodsPatients undergoing GES completed the Patient Assessment of GI Symptoms (PAGI-SYM) assessing symptoms over the prior 2 weeks and a questionnaire for which patients graded six symptoms during GES. A Symptom Severity Index (SSI) represented the mean of six symptoms at each time point.Key resultsA total of 560 patients underwent GES for clinical evaluation of symptoms. Of 388 patients included in the study: 232 patients had normal GES (NGES), 156 delayed GES (DGES), and 11 rapid GES (RGES). Symptom severity index increased pre to postprandial for each group: NGES: 0.51 +/- 0.07 to 0.92 +/- 0.03, DGES: 0.60 +/- 0.09 to 1.13 +/- 0.05, and RGES: 0.56 +/- 0.12 to 0.79 +/- 0.13. Delayed gastric emptying scintigraphy patients had a higher postprandial SSI than NGES patients (1.13 +/- 0.05 vs 0.92 +/- 0.03, P < 0.05). Postprandial symptoms of stomach fullness (1.9 +/- 0.12 vs 1.5 +/- 0.09; P = 0.011), bloating (1.4 +/- 0.11 vs 1.1 +/- 0.09; P = 0.033), and abdominal pain (1.1 +/- 0.08 vs 0.7 +/- 0.12; P = 0.012) were higher in DGES than NGES. Symptom severity based on PAGI-SYM for 2 weeks prior to GES correlated with symptoms during the test for nausea (NGES, r = 0.61; DGES, r = 0.70), stomach fullness (NGES, r = 0.47; DGES, r = 0.60), and bloating (NGES, r = 0.62, DGES, r = 0.66).Conclusions & inferencesStomach fullness, bloating, and abdominal pain recorded during GES were higher in patients with delayed gastric emptying than in patients with normal gastric emptying. Symptoms recorded during GES correlated with those during daily life by patient recall.

Mechanism-Based Modeling of Gastric Emptying Rate and Gallbladder Emptying in Response to Caloric Intake.

Project description:Bile acids released postprandially modify the rate and extent of absorption of lipophilic compounds. The present study aimed to predict gastric emptying (GE) rate and gallbladder emptying (GBE) patterns in response to caloric intake. A mechanism-based model for GE, cholecystokinin plasma concentrations, and GBE was developed on data from 33 patients with type 2 diabetes and 33 matched nondiabetic individuals who were administered various test drinks. A feedback action of the caloric content entering the proximal small intestine was identified for the rate of GE. The cholecystokinin concentrations were not predictive of GBE, and an alternative model linking the nutrients amount in the upper intestine to GBE was preferred. Relative to fats, the potency on GBE was 68% for proteins and 2.3% for carbohydrates. The model predictions were robust across a broad range of nutritional content and may potentially be used to predict postprandial changes in drug absorption.

Ultrasonography Findings of the Carpal Tunnel after Endoscopic Carpal Tunnel Release for Carpal Tunnel Syndrome.

Project description:ObjectiveTo investigate changes in the median nerve, retinaculum, and carpal tunnel on ultrasound after successful endoscopic carpal tunnel release (ECTR).Materials and methodsThis prospective study involved 37 wrists in 35 patients (5 male, 30 female; mean age ± standard deviation [SD], 56.9 ± 6.7 years) with primary carpal tunnel syndrome (CTS). An in-house developed scoring system (0-3) was used to gauge the clinical improvement after ECTR. Ultrasound was performed before ECTR, and at 1, 3, and 12 months post-ECTR. Changes in the median nerve, flexor retinaculum, and carpal tunnel morphology on ultrasound after ECTR were analyzed. Ultrasound parameters for different clinical improvement groups were compared.ResultsAll patients improved clinically after ECTR. The average clinical improvement score ± SD at 12 months post-ECTR was 2.2 ± 0.7. The median nerve cross-sectional area proximal and distal to the tunnel decreased at all time intervals post-ECTR but remained swollen compared to normal values. Serial changes in the median nerve caliber and retinacular bowing after ECTR were more pronounced at the tunnel outlet than at the tunnel inlet. The flexor retinaculum had reformed in 25 (68%) of 37 wrists after 12 months.ConclusionPostoperative changes in median nerve and retinaculum parameters were most pronounced at the tunnel outlet. Even in patients with clinical improvement after ECTR, nearly all ultrasound parameters remain abnormal at one year post-ECTR. These ultrasound parameters should not necessarily be relied upon to diagnose persistent CTS after ECTR.
