Temporal trend of hepatitis B surface mutations in the post-immunization period: 9 years of surveillance (2005-2013) in eastern China.

ABSTRACT: Limited information is available about the temporal trend in the prevalence and evolution of hepatitis B virus (HBV) S-gene mutations in the post-immunization era in China. From 2005 to 2013, 1077 hepatitis B cases under 15 years of age reported through Chinese National Notifiable Disease Reporting System (NNDRS) were successfully sequenced of S-gene in Shandong province, China. A total of 97 (9.01%) cases had amino acid (aa) substitution in the "?" determinant of HBsAg. The yearly prevalence from 2005 to 2013 maintained at a relatively stable level, and showed no significant change (P?>?0.05). Multivariate logistic regression analysis demonstrated that the prevalence of "?" mutations was independently associated with the maternal HBsAg status (P??0.05). The hottest mutation position was aa126 (I126S/N and T126A, 29.63%), and aa 145 (G145R/A, 25.93%). Mutated residue 126 tended to occur less frequent, while that of residue 145 was more frequent with increasing year. Our data showed that there was no increase in the frequency of HBV "?" mutations over time during the post-immunization period. However, long-term vaccination might enhance the change of HBV mutational pattern, and G145 mutation was becoming dominant.

Project description:Eleven polybrominated diphenyl ether (tri- to deca-BDE) congeners and 2,2',4,4',5,5'-hexabromobiphenyl (BB153) have been measured in pooled serum samples from the National Health and Nutrition Examination Survey (NHANES) for one decade (from survey years 2005/06 through 2013/14). The pools, which are representative of the general noninstitutionalized population of the United States, encompassed thirty-two demographic groups defined by sex, race/ethnicity (Mexican American, non-Hispanic black, non-Hispanic white, and all other race/ethnicities), and age (12-19, >20-39, >40-59, and ?60 years). The adjusted geometric means were determined in a multiple linear regression model for the six congeners (BDE28, BDE47, BDE99, BDE100, BDE153, and BB153) with detectable concentrations in at least 60% of pools in each of the thirty-two demographic groups; the level of significance for all statistical comparisons thereof were determined. BDE154 and BDE209 were detected in 60% of the NHANES 2011/12 and 2013/14 pools; only these two survey periods were evaluated for these congeners. The percent change in concentration by a 2-year survey period was calculated. All examined PBDEs reported in five survey periods decreased in concentration, except BDE153, for which concentrations increased by 12.0% (95% CI 7.1-16.4) and 8.4% (95% CI 2.9-14.1) for the age groups 40-59 and ?60 years, respectively; no significant change was observed in younger age groups. Excluding BDE153, we observed larger percentage decreases by a 2-year survey period for the age groups 12-19, 20-39, and ?60 years compared with the age group 40-59 years. The percentage decrease by a two-year survey period ranged between -19.6% (BDE99, 20-39 years old) and -4.5% (BDE100, 40-59 years old). Although five polybrominated diphenyl ether (PDBE) congeners and BB153 are still frequently detected in the U.S. general population, PBDE concentrations have decreased since 2005-2006, likely, because of changes in manufacturing practices that started in the mid-2000s.

Project description:PurposeFirst, to investigate the utilization rate and effect of proven myopic interventions. Second, to predict the prevalence of myopia and high myopia, as well as Years Lived with Disability (YLD) caused by an uncorrected refractive error in children and teens in Eastern China from 2010 to 2050 under different interventions.Methods(1) The surveillance of common diseases among children and adolescents in Jiangsu Province from 2010 to 2021 provides the database for myopia screening and intervention utilization surveys. (2) The National Bureau of Statistics and the Global Burden of Disease Study 2016 (GBD2016) are the foundation for the estimated myopes and YLD. (3) A systematic review provides the strong or weak impact of intervention in the prediction model. (4) The trend of screening myopia from 2010 to 2050 under various treatments is predicted using a GM (1,1) model.ResultsBy the year 2050, myopia is expected to affect 8,568,305 (7-12 years old) and 15,766,863 (13-18 years old) children and adolescents, respectively (95% CI: 8,398,977-8,737,633). The utilization prevalence of myopia-proven interventions for myopic children included outdoor activities, orthokeratology lenses, atropine treatment, contact lenses, frame glasses, and eye exercises, with respective rates of 31.9-33.1, 2.1-2.3, 6.0-7.5, 2.2-2.7, 60.4-62.2, and 64.7-72.5%. All interventions have substantial effects on myopia after parental myopia and behavior pattern adjustment, including physical activity, near work, dietary pattern, and sleep. Under strong intervention, the estimated reduced myopia prevalence by the year 2050 is 1,259,086 (95% CI: 1,089,758-1,428,414) for children aged 7-12, and 584,785 (95% CI: 562,748-606,823) for children aged 13-18, respectively.ConclusionAmong myopic Chinese children and adolescents, the use rates and effects of proven myopia interventions vary. Under the present intervention strategy, the prevalence of myopia and high myopia will increase from 2010 to 2050. The overall number of myopic people can be greatly decreased by implementing timely, steady, comprehensive interventions.
