Tension pneumomediastinum: A literal form of chest tightness.

ABSTRACT: Tension pneumomediastinum is a rare and life-threatening complication of mediastinal emphysema which can occur with mechanical ventilation. We present a case of tension mediastinum associated with mechanical ventilation in a patient with Acute Respiratory Distress Syndrome due to Pneumocystis jirovecii pneumonia. We discuss the mechanism and pathophysiology of tension pneumomediastinum, the potential association with Pneumocystis jirovecii pneumonia and recruitment manouvres, and its definitive emergency treatment.

Spontaneous pneumomediastinum (Hamman's syndrome): a rare cause of postpartum chest pain.

Project description:We present a case of a 28-year-old primiparous woman with facial swelling followed by acute chest pain immediately after delivery. Chest radiograph revealed pneumomediastinum and surgical emphysema. She recovered well within 24 h of observation and conservative management. Postpartum spontaneous pneumomediastinum should be considered in the differential diagnosis of sudden-onset postpartum chest pain immediately or a few hours after delivery. It is a rare benign condition and usually resolves spontaneously without serious consequences. Chest X-ray is the single most important diagnostic test. It is important to rule out other serious and life-threatening conditions. Prolonged pushing, difficult labour and use of inhalational drugs place young primiparous women at higher risk. Recurrence is uncommon in subsequent pregnancy and management is unclear, although expectant management with epidural analgaesia to prevent recurrence in subsequent pregnancy is suggested.

Response of patients with chest tightness variant asthma with routine asthma treatment regimen: A 1-year multicenter, prospective, real-world study.

Project description:BackgroundAsthmatic patients with chest tightness as their only presenting symptom (chest tightness variant asthma [CTVA]) have clinical characteristics of eosinophilic airway inflammation similar to those of classic asthma (CA); however, whether CTVA has similar response to antiasthma treatment as compared with CA remains unclear.ObjectiveThe response of 76 CTVA patients to standard asthma treatments with inhaled corticosteroids with long-acting beta-agonists was explored in a 52-week multicenter, prospective, real-world study.ResultsAfter 52 weeks of treatment with therapy regimens used for CA, the mean 5-point Asthma Control Questionnaire (ACQ-5) score decreased markedly from 1.38(first administration) to 0.71 (52 weeks, mean decrease: 0.674, 95%CI: 0.447-0.900, P<.001).The mean asthma quality-of-life questionnaire (AQLQ) score increased from 5.77 (first administration) to 6.20 (52 weeks, mean increase: 0.441, 95% CI 0.258-0.625, P<.001). Furthermore, at week 52, FVC, FEV1 %, the diurnal variation in PEFand the PD20-FEV1 were significantly improved. Subgroup analysis revealed that the patients at first administration in the responsive group had higher ACQ-5 scores than those in the nonresponsive group (P < .05).ConclusionIn conclusion, patients with CTVA had a good therapeutic response to the guideline-recommended routine treatment (containing inhaled corticosteroids). The association between the treatment response and the severity of CTVA suggested that CTVA patients with higher ACQ-5 scores had better therapeutic effects.

Bilinguals' Plausibility Judgments for Phrases with a Literal vs. Non-literal Meaning: The Influence of Language Brokering Experience.

Project description:Previous work has shown that prior experience in language brokering (informal translation) may facilitate the processing of meaning within and across language boundaries. The present investigation examined the influence of brokering on bilinguals' processing of two word collocations with either a literal or a figurative meaning in each language. Proficient Spanish-English bilinguals classified as brokers or non-brokers were asked to judge if adjective+noun phrases presented in each language made sense or not. Phrases with a literal meaning (e.g., stinging insect) were interspersed with phrases with a figurative meaning (e.g., stinging insult) and non-sensical phrases (e.g., stinging picnic). It was hypothesized that plausibility judgments would be facilitated for literal relative to figurative meanings in each language but that experience in language brokering would be associated with a more equivalent pattern of responding across languages. These predictions were confirmed. The findings add to the body of empirical work on individual differences in language processing in bilinguals associated with prior language brokering experience.

An Objective Measure of Noseband Tightness and Its Measurement Using a Novel Digital Tightness Gauge.

Project description:Noseband tightness is difficult to assess in horses participating in equestrian sports such as dressage, show jumping and three-day-eventing. There is growing concern that nosebands are commonly tightened to such an extent as to restrict normal equine behaviour and possibly cause injury. In the absence of a clear agreed definition of noseband tightness, a simple model of the equine nose-noseband interface environment was developed in order to guide further studies in this area. The normal force component of the noseband tensile force was identified as the key contributor to sub-noseband tissue compression. The model was used to inform the design of a digital tightness gauge which could reliably measure the normal force component of the noseband tensile force. A digital tightness gauge was developed to measure this parameter under nosebands fitted to bridled horses. Results are presented for field tests using two prototype designs. Prototype version three was used in field trial 1 (n = 15, frontal nasal plane sub-noseband site). Results of this trial were used to develop an ergonomically designed prototype, version 4, which was tested in a second field trial (n = 12, frontal nasal plane and lateral sub-noseband site). Nosebands were set to three tightness settings in each trial as judged by a single rater using an International Society for Equitation Science (ISES) taper gauge. Normal forces in the range 7-95 N were recorded at the frontal nasal plane while a lower range 1-28 N was found at the lateral site for the taper gauge range used in the trials. The digital tightness gauge was found to be simple to use, reliable, and safe and its use did not agitate the animals in any discernable way. A simple six point tightness scale is suggested to aid regulation implementation and the control of noseband tightness using normal force measurement as the objective tightness discriminant.

Comprehending Non-literal Language: Effects of Aging and Bilingualism.

Project description:A pressing issue that the twenty-first century is facing in many parts of the developed world is a rapidly aging population. Whilst several studies have looked at aging older adults and their language use in terms of vocabulary, syntax and sentence comprehension, few have focused on the comprehension of non-literal language (i.e., pragmatic inference-making) by aging older adults, and even fewer, if any, have explored the effects of bilingualism on pragmatic inferences of non-literal language by aging older bilinguals. Thus, the present study examined the effects of age(ing) and the effects of bilingualism on aging older adults' ability to infer non-literal meaning. Four groups of participants made up of monolingual English-speaking and bilingual English-Tamil speaking young (17-23 years) and older (60-83 years) adults were tested with pragmatic tasks that included non-conventional indirect requests, conversational implicatures, conventional metaphors and novel metaphors for both accuracy and efficiency in terms of response times. While the study did not find any significant difference between monolinguals and bilinguals on pragmatic inferences, there was a significant effect of age on one type of non-literal language tested: conventional metaphors. The effect of age was present only for the monolinguals with aging older monolinguals performing less well than the young monolinguals. Aging older bilingual adults were not affected by age whilst processing conventional metaphors. This suggests a bilingual advantage in pragmatic inferences of conventional metaphors.

Idiom comprehension in aphasia: Literal interference and abstract representation.

Project description:We investigate three potential mechanisms underlying the deficit in idiom comprehension seen in aphasia: difficulty inhibiting literal meanings, inability to recognize that a figurative interpretation is required, and difficulty processing abstract words and concepts. Unimpaired adults and PWA read high and moderate familiarity idioms either preceded or followed by a figuratively biasing context sentence. They then completed a string-to-word probe selection task, choosing between a figurative target, a literal lure, and unrelated concrete and abstract lures. PWA chose the figurative target more often for more familiar idioms and after figuratively biasing contexts, suggesting that difficulty accessing figurative meanings may be a key contributor to idiom impairment in aphasia. Importantly, PWA chose abstract lures at the same rate as they chose literal lures, suggesting that abstract lures may be considered equally good matches for weak idiomatic representations in PWA, and therefore that idiomatic figurative meanings may be represented similarly to abstract concepts for PWA. These results have implications for models of idiom comprehension in aphasia, as well as the design of future studies of idiom comprehension in PWA.

Effects of knot tightness at the molecular level.

Project description:Three 819 knots in closed-loop strands of different lengths (?20, 23, and 26 nm) were used to experimentally assess the consequences of knot tightness at the molecular level. Through the use of 1H NMR, diffusion-ordered spectroscopy (DOSY), circular dichroism (CD), collision-induced dissociation mass spectrometry (CID-MS) and molecular dynamics (MD) simulations on the different-sized knots, we find that the structure, dynamics, and reactivity of the molecular chains are dramatically affected by the tightness of the knotting. The tautness of entanglement causes differences in conformation, enhances the expression of topological chirality, weakens covalent bonds, inhibits decomplexation events, and changes absorption properties. Understanding the effects of tightening nanoscale knots may usefully inform the design of knotted and entangled molecular materials.

Tightness-looseness across the 50 united states.

Project description:This research demonstrates wide variation in tightness-looseness (the strength of punishment and degree of latitude/permissiveness) at the state level in the United States, as well as its association with a variety of ecological and historical factors, psychological characteristics, and state-level outcomes. Consistent with theory and past research, ecological and man-made threats--such as a higher incidence of natural disasters, greater disease prevalence, fewer natural resources, and greater degree of external threat--predicted increased tightness at the state level. Tightness is also associated with higher trait conscientiousness and lower trait openness, as well as a wide array of outcomes at the state level. Compared with loose states, tight states have higher levels of social stability, including lowered drug and alcohol use, lower rates of homelessness, and lower social disorganization. However, tight states also have higher incarceration rates, greater discrimination and inequality, lower creativity, and lower happiness relative to loose states. In all, tightness-looseness provides a parsimonious explanation of the wide variation we see across the 50 states of the United States of America.
